Using pressure-limited mechanical ventilation in caring for organ donors.

Pressure-limited (controlled) ventilation is commonly employed to provide mechanical ventilation in the intensive care unit when lung compliance is poor or when airway resistance is irreversibly high. Modification of the inspiratory-expiratory ratio to include inspiratory-expiratory ratio reversal and permissive hypercapnia can also be used when lung disease or injury is severe. Because other donor organs often can be saved for transplantation even when the lungs have been badly damaged, the organ procurement coordinator should adopt pressure-limited ventilation as well as inspiratory-expirato

A Palliative Approach is Adopted for Many Patients Dying in Hospital with Chronic Obstructive Pulmonary Disease.

Severe chronic breathlessness in advanced chronic obstructive pulmonary disease (COPD) is undertreated and few patients access specialist palliative care in the years before death. This study aimed to determine if symptom palliation or a palliative approach were delivered during the final hospital admission in which death occurred. Retrospective medical record audits were completed at two Australian hospitals, with all patients who died from COPD over 12 years between 1 January 2004 and 31 December 2015 included. Of 343 patients included, 217 (63%) were male with median age 79 years (IQR 71.4-

Illusion of choice? Implicit coercion and assisted dying in clinical practice.

In this article, we argue that the narrow focus on explicit coercion in assisted dying obscures more pervasive forms of implicit coercion operating at interpersonal and institutional levels. Drawing on complex adaptive systems theory, we conceptualise healthcare systems as environments in which coercive dynamics emerge non-linearly and cannot be adequately mitigated through linear procedural safeguards. We look at the potential for implicit coercion within the unique context of terminal illness and consider how end-of-life emotional distress and attachment style shape individuals' responses to

Sex-related differences in end-of-life decision-making in patients with traumatic brain injury: a systematic review and meta-analysis.

BACKGROUND: Families and medical professionals caring for traumatic brain injury (TBI) patients are often faced with decisions regarding the limitation of life-sustaining treatment (LLST). Considering difficulty of outcome prognostication and the resulting lack of objectivity, these decisions remain challenging. Although it is recognized that sex influences end-of-life care, research on sex differences in palliative care (PC) is scarce. To better understand the decision-making process at the end of life in TBI, we investigated potential sex-differences in the frequency of LLST and PC involveme

Implantable cardioverter defibrillator deactivation and end-of-life: British Heart Rhythm Society practical consensus guideline.

Implantable cardioverter defibrillators (ICDs) are implanted in increasing numbers of patients with the aim of treating ventricular arrhythmias in high-risk patients and reducing their risk of dying. Individuals are also living longer with these devices. As a result, a greater number of patients with an ICD will deteriorate either with worsening cardiac failure, another non-cardiac condition or general frailty and will have a limited prognosis. Frequently they will be cared for by non-cardiac teams who may be less familiar with ICDs. Therefore, to ensure the person receives high-quality end-of

First generation migrants' experiences of terminal illness: a systematic review of diasporic dying.

BACKGROUND: Migration is an established global phenomenon. While many newly arrived migrants have better health than the general population of the country they have moved to, migrants also have their own healthcare needs and face particular issues when diagnosed with a terminal illness. First generation migrants are less likely to have social, financial, and medical supports when faced with a terminal illness. These factors make first generation migrants an important group to understand in order to inform service commissioning and delivery. METHODS: The systematic review was an international

Places of end-of-life care and death in health policies of four countries (EOLinPLACE Project).

BACKGROUND: Place of death and its concordance with patient preference is a key indicator for end-of-life care, studied cross-nationally and flagged as a priority by the OECD. However, it is unclear if and how 'place' is considered in health policy in relation to end-of-life care. This study aims to examine if and how health policies in different nations consider places of end-of-life care and death. METHODS: We conducted a comparative qualitative study across the US, the Netherlands, Portugal, and Uganda, of health policy documents following the READ (i.e., Ready materials, Extract data, Ana

Towards a shared and supported decision-making model: fostering relational autonomy in end-of-life care.

Respect for autonomy is a central ethical principle in end-of-life care. However, due to cognitive decline, emotional distress, and the existential weight of end-of-life decisions, many patients may struggle to articulate or sustain their preferences without appropriate forms of support. The question, then, is not solely how to respect patients' autonomy in end-of-life care, but how autonomy, understood in relational terms, can be genuinely fostered. To address this question, the article advances a normative and practice-oriented model of decision-making that integrates two approaches often tr

Coping strategies of nurses caring for children with life-limiting and life-threatening conditions: An integrative review.

INTRODUCTION: Nurses care for children with life-limiting and life-threatening conditions across a variety of settings, often managing complex needs and unpredictable illness trajectories. As key providers of physical and emotional support to the child and family, they face significant emotional challenges. Therefore, it is essential to explore how nurses cope with these demands. The review aimed to synthesise and critically appraise the evidence on coping strategies used by nurses caring for these children. METHODS: Studies exploring coping strategies used by nurses caring for children with

Prognostic awareness and its association with health outcomes in the last year of life.

BACKGROUND: Little research has examined changes in prognostic awareness (PA) in the last year of life and the extent PA change was associated with anxiety, depression, and spiritual well-being among metastatic cancer patients. METHODS: Two surveys were administered in the last year of life to 176 conveniently sampled Singaporean patients with stage 4 solid cancers. PA was assessed by asking patients whether they were aware that their treatments were unlikely to cure their cancer. Multivariable linear regression models were used to investigate the association of PA change with anxiety, depres

Physicians' Beliefs and Attitudes Toward Hypoactive Delirium in The Last Days of Life.

CONTEXT: The perspective toward hypoactive delirium in the last days of life could be different among physicians. OBJECTIVES: To clarify the attitudes, beliefs, and opinions of palliative care physicians and liaison psychiatrists toward hypoactive delirium in the last days of life and to explore the association among these factors. METHODS: A nationwide cross-sectional questionnaire survey was conducted among 1667 physicians who were either certified palliative care specialists or liaison psychiatrists. Physicians' agreement with the appropriateness of pharmacological management (e.g., antip

Racial Disparities in End-of-Life Care Between Black and White Adults With Metastatic Cancer.

CONTEXT: The comfort of patients with cancer near the end of life (EOL) is often undermined by unnecessary and burdensome treatments. There is a need for more research examining racial disparities in EOL care, especially in regions with a history of racial discrimination. OBJECTIVES: To examine whether black adults received more burdensome EOL care than white adults in a population-based data set of cancer decedents in Louisiana, a state with a history of slavery and long-standing racial disparities. METHODS: This was a retrospective analysis of EOL care from the Research Action for Health N

Elucidating the End-of-Life Experience of Persons With Amyotrophic Lateral Sclerosis.

Amyotrophic lateral sclerosis (ALS) is a fatal motor neuron disease that occurs in 4 among 100 000 people in the United States. Individuals with ALS gradually lose their ability to control voluntary muscles, diminishing their ability to communicate. A comprehensive multidatabase search retrieved 31 qualitative research articles that addressed persons with end-of-life experiences with ALS. Inclusion/exclusion criteria were applied and a critical appraisal was applied for the final 8 included articles. First-person data extraction from the final articles represented emergence of 3 themes signifi

Prevalence and Implications of Abnormal Laboratory Results in Patients in the Terminal Phase of Life.

BACKGROUND: Pathophysiological changes at the end of life may affect pharmacokinetics of drugs. However, caregivers typically do not extensively monitor patients' laboratory parameters at the end of life. OBJECTIVE: Our aim was to describe laboratory parameters of hospice patients in the week before death. METHODS: A cohort study was conducted on available laboratory results in the week before death, including clinical chemistry and hematology tests. RESULTS: Laboratory data of 125 patients in a palliative care center were included, assessed at a median of 3 days before death. Eighty percen

Dignity therapy for older people in care homes: a qualitative study of the views of residents and recipients of 'generativity' documents.

AIM: To report the findings of a study exploring the views and experiences of care home resident's family on Dignity Therapy. BACKGROUND: As the proportion of older people dying in care homes increases, it is important to enhance their dignity, reduce distress at the end-of-life, and provide bereavement support to their families. Pilot studies show that hospice patients and care home residents feel Dignity Therapy had or would help their families; however, there are no qualitative studies of their views. DESIGN: Qualitative exploration. METHODS: Qualitative interviews were conducted between
