Long-term stimulation of cardiac vagal preganglionic neurons reduces blood pressure in the spontaneously hypertensive rat.

BACKGROUND: Arterial hypertension is associated with autonomic nervous system dysfunction. Different interventional strategies have been implemented in recent years for the reduction of sympathetic activity in patients with hypertension. However, the therapeutic benefit of increasing vagal tone in hypertensive patients remains largely unexplored. OBJECTIVE: Here, we describe the effects of long-term activation of vagal neural pathways on arterial pressure, heart rate arterial pressure variability and spontaneous baroreflex sensitivity in spontaneously hypertensive rats (SHR) and normotensive

Adenoviral dominant-negative soluble PDGFRβ improves hepatic collagen, systemic hemodynamics, and portal pressure in fibrotic rats.

BACKGROUND & AIMS: Platelet-derived growth factor (PDGF) is the most potent stimulus for proliferation and migration of stellate cells. PDGF receptor β (PDGFRβ) expression is an important phenotypic change in myofibroblastic cells that mediates proliferation and chemotaxis. Here we analyzed the relationship between PDGFRβ expression, hemodynamic deterioration, and fibrosis in CCl(4)-treated rats. Thereafter, we investigated the effects produced by an adenovirus encoding a dominant-negative soluble PDGFRβ (sPDGFRβ) on hemodynamic parameters, PDGFRβ signaling pathway, and fibrosis. METHODS: Mea

Effect of smooth muscle relaxant drugs on proximal human ureteric activity in vivo: a pilot study.

Drugs are increasingly being used to promote stone passage in renal colic. Diclofenac, nifedipine and tamsulosin cause ureteric smooth muscle relaxation in vitro; however, in clinical trials nifedipine and tamsulosin promote stone passage whereas diclofenac has no apparent benefit. We adapted a ureteric pressure transducer catheter in an attempt to compare the human ureteric response to these drugs in vivo. The catheter was inserted into the contralateral ureter following ureteroscopy for stone disease. Contraction frequency, pressure and velocity measurements were recorded at 24 h. Each patie

Microconvex Probe-Assisted Sonopalpation for Ultrasound-Guided Diagnosis of Cervical Discogenic Pain in Challenging Neck Anatomy: A Technical Report.

Cervical discogenic pain is diagnostically challenging because degenerative changes on magnetic resonance imaging often do not reliably identify the symptomatic disc level. Although provocative discography remains an important reference standard in selected patients, it is invasive and controversial. More recently, ultrasound-guided sonopalpation has been described as a practical adjunct for identifying painful cervical discs before intradiscal intervention. However, in patients with short necks, bulky anterior soft tissues, reduced tissue compliance, or close apposition of the carotid sheath

Effects of Alpha-Blockade on Ureteral Peristalsis and Intrapelvic Pressure in an In Vivo Stented Porcine Model.

INTRODUCTION: Clinical studies have shown beneficial role of oral alpha-blockers for ureteral stent-related morbidity. However, the in vivo effects of oral alpha-blockers on a stented ureter are unclear. We evaluated the effects of alpha-blockade on ureteral dynamics in a stented porcine ureter. METHODS: Twenty-seven female pigs were used in this study. Fourteen pigs received oral alpha-blocker medication (silodosin, 8 mg daily), and 13 pigs received no medication. Under cystoscopic guidance, a 5F ureteral catheter was positioned in the renal pelvis and attached to a pressure monitor. A Foley

Real-Time Injection Pressure Sensing and Minimal Intensity Stimulation Combination During Ultrasound-Guided Peripheral Nerve Blocks: An Exploratory Observational Trial.

BACKGROUND: Nerve damage can occur after peripheral nerve block (PNB). Ultrasound guidance does not eliminate the risk of intraneural injection or nerve injury. Combining nerve stimulation and injection pressure (IP) monitoring with ultrasound guidance has been suggested to optimize needle tip location in PNB. In this prospective observational study, we hypothesized that measured pairs of IP and minimum intensity of stimulation (MIS) might differentiate successive needle tip locations established by high-resolution ultrasound during PNB. METHODS: For this exploratory study, 240 observations f

An Oral Pressure Conversion Ratio as a Predictor of Vocal Efficiency.

Voice production is an inefficient process in terms of energy expended versus acoustic energy produced. A traditional efficiency measure, glottal efficiency, relates acoustic power radiated from the mouth to aerodynamic power produced in the trachea. This efficiency ranges between 0.0001% and 1.0%. It involves lung pressure and hence would appear to be a useful effort measure for a given acoustic output. Difficulty in the combined measurement of lung pressure and tracheal airflow, however, has impeded clinical application of glottal efficiency. This article uses the large data base from Schutt

Comparison of air-charged and water-filled urodynamic pressure measurement catheters.

AIMS: Catheter systems are utilized to measure pressure for diagnosis of voiding dysfunction. In a clinical setting, patient movement and urodynamic pumps introduce hydrostatic and motion artifacts into measurements. Therefore, complete characterization of a catheter system includes its response to artifacts as well its frequency response. The objective of this study was to compare the response of two disposable clinical catheter systems: water-filled and air-charged, to controlled pressure signals to assess their similarities and differences in pressure transduction. METHODS: We characterize

Lip closing pressure and spoon management in passive spoon feeding.

To determine the sources of lip closing pressure (P(LC) ) generation during passive spoon feeding, we used a fine pressure transducer glued into a wooden spoon, as well as electromyography (EMG) of the upper and lower lips and the submental muscle complex, in normal adult volunteers (average age 24·5 years). An assistant fed a seated subject 0·6 mL of yogurt and then withdrew the spoon from the subject's closed mouth. The spoon was held at an angle of 0° (i.e. in the naso-auricular plane) during serving and at either 0° or 60° during withdrawal. We detected simultaneous increases in P(LC) and

Whispering--a single-subject study of glottal configuration and aerodynamics.

Whisper productions were produced by a single adult male subject over a wide range of subglottal pressures, glottal areas, and glottal flows. Dimensional measurements were made of these three variables, including glottal perimeter. Subglottal pressure was directly obtained by a pressure transducer in a tracheal catheter, and wide-band flow with a pneumotach mask. Four types of whispers were used-hyperfunctional, hypofunctional, neutral, and postphonation-in addition to three levels of loudness (soft, medium, loud). Sequences of the /pae/ syllable were used. Video recordings of the larynx were

Accuracy evaluation of a method to partition ground reaction force and center of pressure in cane-assisted gait using an instrumented cane with a triaxial force sensor.

Clarifying the biomechanics of cane-assisted gait in elderly individuals and patients with gait disorders is important for developing better therapeutic interventions in the fields of rehabilitation and orthopedics. However, if the foot and the cane in the ipsilateral hand are placed on the same force plate simultaneously, the force plate cannot separate the forces as it records the sum of the forces. To overcome this indeterminacy problem of the ground reaction force (GRF) and the center of pressure (COP) in cane-assisted gait analysis, a method to partition the GRF and COP using an instrumen

Non-nutritive sucking in the healthy pre-term infant.

In the infant, sucking behaviour is one of the first coordinated muscular activities. It is under the control of the brainstem. In utero sucking is observed from 13 weeks' gestation. The healthy full-term newborn infant has a very stable rhythm of alternating bursts of sucking with pauses in between. The non-nutritive sucking (NNS) patterns of 58 low-risk prematurely born infants (gestational ages between 26 and 35 weeks) were studied using a specially designed computer-based method that analyses and quantifies NNS. A total of 183 observations were analysed. The typical NNS pattern with bursts

Deterministic properties of apnoeas in an abdominal breathing signal.

The paper describes a general set of properties that represent most apnoeas as found in an abdominal breathing signal. An apnoea is a pause in breathing during sleep, and only central apnoeas in infants are considered. Human experts are consulted to determine what properties of the signal they use to recognise apnoeas. A set of deterministic, or shape, properties is developed to represent expert opinion. An apnoea is modelled as a flat region with four properties: flatness, duration, thinness and smoothness. Mathematical descriptions of each property are formulated that discriminate between ap
