Experiences and perspectives during the transition from paediatric to adult care in type 1 diabetes mellitus: systematic review of qualitative studies.

UNLABELLED: When Type 1 diabetes mellitus (T1DM) patients transition from paediatric to adult healthcare providers, the risk of deteriorating health outcomes rises. The aim of this study is to integrate the experiences and perspectives regarding interventions for the management of T1DM during the transition from paediatric to adult care, from a patient and professional perspective. A systematic review was conducted with a framework analysis guided by the Cochrane Qualitative and Implementation Methods Group approach. Three online databases (Medline, Embase and Web of Science) were searched. We

Associations Between Health Screening Rates and Visits to Their Primary Care Provider in Young Adults Transitioning From Pediatrics to Adult Medicine.

INTRODUCTION: This study examines associations between a young adult's visit to a designated primary care physician (PCP) in the 18 months after transitioning from pediatrics and their completion of preventive healthcare screenings. The period from 18 to 25 years is marked by transition and continued brain development, making it an ideal period for impactful intervention on healthy behaviors and outcomes. METHODS: Completion of screening for alcohol use, mental health issues, exercise habits, and tobacco use was measured in members upon turning 18 during 2020-2021. These members had transitio

mHealth Support in Cardiac Care Pathways for Patient Self-Management During Transitions From Hospital to Rehabilitation: Exploratory Field Study.

BACKGROUND: Cardiac rehabilitation (CR) is essential for recovery from cardiovascular disease. However, patients often encounter challenges in navigating the transition from acute hospital care to CR. Mobile health (mHealth) technologies may support this critical phase; however, evidence regarding their clinical practice remains limited. The HERO app (developed by REDOX GmbH) was developed to address the needs of patients with cardiovascular disease for orientation, emotional support, and motivation during this transition. OBJECTIVE: This study aims (1) to explore how mHealth technologies tai

Understanding barriers and facilitators of transition to adult care among adolescents with Congenital Heart Disease (CHD): A qualitative study of adult perspectives.

BACKGROUND/OBJECTIVES: Congenital heart disease (CHD) refers to a spectrum of structural abnormalities of the heart at birth. Survival and outcomes have improved due to advances in surgery and post-operative care, resulting in more patients transitioning from paediatric to adult healthcare settings. This study aimed to explore varied experiences of this transition and identify ways to improve processes to enable continuity of care in Australia. METHODS: Purposive sampling was used to recruit a diverse sample of CHD patients from a larger study. In semi-structured online interviews, adult part

"I want to stay here": Patient and staff perspectives on transitioning from a low-threshold buprenorphine program to clinic-based care.

BACKGROUND: The Project Connections At Re-Entry (PCARE) Van is a low-threshold buprenorphine program operating outside the Baltimore City Detention Center. Like other low-threshold programs, PCARE seeks to engage a vulnerable population in care, stabilize patients, then transition patients to longer-term care; however, <10% of patients transition to clinic-based buprenorphine treatment. Our goal was to better understand these low transition rates and center patient perspectives in discussion of broader low-threshold program design. METHODS: From December 2022 to June 2023, semi-structured int

A Novel Pathogenic IGSF1 Variant in a Patient with GH and TSH Deficiency Diagnosed by High IGF-I Values at Transition to Adult Care.

IGSF1 deficiency is a rare X-linked condition characterized by central hypothyroidism and a wide variety of other clinical features with variable prevalence, including a delayed pubertal testosterone rise and growth spurt in the context of normal or accelerated testicular growth, and adult macroorchidism with relatively low serum testosterone concentrations. Other features include increased waist circumference, attention deficit, prolactin deficiency and transient partial growth hormone (GH) deficiency in childhood, contrasting with an increased GH secretion in adulthood. Patients with this di

An Initiative to Improve 30-Day Readmission Rates Using a Transitions-of-Care Clinic Among a Mixed Urban and Rural Veteran Population.

BACKGROUND/OBJECTIVE: Hospital readmissions in the United States, especially in patients at high-risk, cost more than $17 billion annually. Although care transitions is an important area of research, data are limited regarding its efficacy, especially among rural patients. In this study, we describe a novel transitions-of-care clinic (TOCC) to reduce 30-day readmissions in a Veterans Health Administration setting that serves a high proportion of rural veterans. METHODS: In this quality improvement initiative we conducted a pre-post study evaluating clinical outcomes in adult patients at high

Patterns and Predictors of Transitions Across Residential Care Settings and Nursing Homes Among Community-Dwelling Older Adults in the United States.

BACKGROUND AND OBJECTIVES: Older adults prefer to age in place, but sociodemographic, health, and socioeconomic factors may influence their decision to remain in the community. Guided by Andersen's behavioral model, we characterize incident transitions out of the community into residential care settings or nursing homes and identify predictors of these transitions. RESEARCH DESIGN AND METHODS: Study participants include 2,725 (weighted n = 13,704,390) community-dwelling U.S. older adults of the National Health and Aging Trends Study from 2011 to 2018. We examined the associations between soci

'A confident parent breeds a confident child': Understanding the experience and needs of parents whose children will transition from paediatric to adult care.

Transitional care for young people with long-term conditions emphasizes the importance of supporting parents, particularly in relation to promoting adolescent healthcare autonomy. Yet, little practical guidance is provided, and transitional care remains suboptimal for many families. This study aimed to examine how parents understand and experience their caregiving role during their child's transition to adult services, to identify parents' needs, and to inform service improvements. Focus groups were undertaken with parents of young people with brittle asthma, osteogenesis imperfecta, or epilep…

The Effectiveness of Standardized Handoff Tool Interventions During Inter- and Intra-facility Care Transitions on Patient-Related Outcomes: A Systematic Review.

Improving physician handoffs is a patient safety priority. The authors hypothesize that standardized handoff interventions during care transitions improve patient-related outcome measures. PubMed, Cochrane, PsycINFO, CINAHL, Embase, and Web of Science were searched for publications from 2000 to May 2016. Eligible studies compared standardized handoff intervention(s) with no standardized handoff intervention and measured patient-related outcomes. Studies were evaluated independently for eligibility for inclusion by at least 2 authors in a 2-stage process; 14 articles met inclusion criteria. Onl
