Necrotizing stomatitis in the developed world.

We present a case of a malnourished 68-year old man with occult hypothyroidism who presented with malaise, pyrexia, tongue swelling, oral ul … ement, intravenous metronidazole and nutritional support. Healing occurred with marked fibrosis and trismus, which has slowly improved with mouth-opening exercises. Necrotizing stomatitis is more commonly encountered in malnourished children in developing countries, and may subsequently result in devastating facial defects and death. Patients in the devel …

Alcohol injection in the management of paroxysmal trigeminal neuralgia: a report of six cases.

The report revisits the role of alcohol in the treatment of paroxysmal trigeminal neuralgia (PTN). The study included 6 patients, 3 male and … ined of oral dysaesthesia. Another patient with similar involvement failed to respond and developed trismus. Responders remained pain-free for 9 months on average. While paroxysmal trigeminal neuralgia (PTN) has been recognised for centuries, the aetiology and definitive treatment remain obscure. Since this is a debilitating condition, management should a …

Ludwig's angina. Report of a case and review of the literature.

In the preantibiotic era, Ludwig's angina frequently caused asphyxiation and death. Recognized less often today, this rapidly progressive su … evidence of dental disease. Submandibular swelling, elevation of the tongue, fever, dysphagia, and trismus were each present in more than one half of patients. Streptococci and anaerobes were most frequently isolated from soft-tissue cultures. Untreated, this illness is fatal in one half of patients. Early recognition is therefore essential. Appropriate …

Platelet-rich plasma and platelet-rich fibrin in oral surgery: A narrative review.

Platelet-rich plasma (PRP) and platelet-rich fibrin (PRF) are biological products derived from the plasma fraction of autologous blood that … atelet-rich fibrin is used after third molar extractions, in the treatment of alveolar osteitis and trismus, and in implant surgery. Platelet-rich plasma is utilized in sinus lift procedures, after tooth extractions, and in patients undergoing the treatment of bisphosphonate-related osteonecrosis of the jaw. Based on this review, plenty of data indicates …

Thirty-year follow-up of a TMD case treated based on the neuromuscular concept.

AIM: Occlusal therapy is employed to alleviate the symptoms of a temporomandibular disorder (TMD) at times. However, the long-term effect of occlusal therapy in the masticatory system is not well understood. This case study aims to present a 30-year follow-up of a TMD case. METHODOLOGY: The patient developed TMD with intermittent closed lock of the left temporomandibular joint (TMJ). Chief complaints included trismus, pain, and noise of the left TMJ during function. The patient's occlusal disharmony was assessed with use of electronic instruments and corrected based on the neuromuscular conce…

The perilobule approach to subcondylar fractures.

Several approaches to the management of subcondylar fractures have been used, including the submandibular approach, the preauricular approac … p revealed correct reduction and fixation in all the cases. There were no occlusal disturbances, no trismus, no lateral deviations of the mandible, and no nerve lesions. Our findings indicate that the short perilobe approach is an easy and safe technique for displaced subcondylar fractures.

Arthroscopic management of septic arthritis of temporomandibular joint.

This article reports on 7 patients with septic arthritis of the temporomandibular joint (TMJ) who were managed with arthroscopy between 1998 and 2007. The common symptoms were trismus and pain. A series of imaging studies showed widening of joint space in 1 patient with plain film; MRI demonstrated increased joint effusion in 4 patients; accompanying cellulitis in adjacent tissues was discerned by CT in 2 patients. Under the arthroscope, a reddened and swollen synovial membrane was found in 2 patients who were in the acute stage, whereas strong adhesions, destruction of cartilage, and bony def…

Facial swelling in a sickle cell patient.

Sickle cell disease (SCD) is characterized as a chronic hemolytic anemia with vaso-occlusive crises that result in multisystem organ damage. Bone marrow is one of the more common sites of these crises, presumably due to marrow hypercellularity that impairs blood flow- leading to regional hypoxia and subsequent infarction. Infarcts of facial bones are considered an uncommon complication of SCD. When infarcts occur in facial bones, the mandible and orbital bones are the most commonly affected. Overall, the clinical presentation of facial bone infarctions may mimic an infectious process, such as

Morphine-associated wooden chest syndrome in palliative care: a case report.

Wooden chest syndrome (WCS) is a rare, life-threatening complication of opioid use, characterized by chest wall rigidity and respiratory compromise. While most commonly associated with rapid intravenous fentanyl, we report a case of WCS following intravenous morphine and midazolam administration in a palliative care patient with metastatic lung sarcoma. The patient, who had a "do-not-intubate" (DNI) order, received morphine and midazolam for severe pain and dyspnea. Shortly after administration, he developed acute rigidity, including neck extension and trismus, consistent with WCS. Despite fur…

Perioperative Care of a Pediatric Patient With Beals Syndrome.

The trismus pseudocamptodactyly syndrome (Beals syndrome) is an uncommon autosomal dominant condition first described in 1971. The disorder shares phenotypic similarities with Marfan syndrome. Affected patients classically present with two main physical features: limited excursion of the mandible and flexion deformity of the fingers that occurs with wrist extension (pseudocamptodactyly). The primary cellular defect is a mutation of the fibrillin-2 (FBN2) gene on chromosome 5q23. Mutations to this gene change the structure of the FBN2 protein, decreasing the elasticity and altering the strength…

Isolated fracture of the coronoid process following a molar extraction: A rare case report.

Isolated coronoid process fractures are uncommon, and iatrogenic isolated fractures are extremely rare. This case describes a displaced fracture of an isolated coronoid process thought to be due to excessive force applied by a dentist that had been overlooked and left untreated for about a month. The patient was a woman in her late 50's and she had undergone a molar extraction. Her dentist had confused her symptoms of trismus, pain, and facial oedema with the complex tooth extraction procedure. Following a cone-beam computed tomography (CBCT) scan we showed that the mandibular coronoid process…

Navigating Challenges in Cervicofacial Actinomycosis: A Case of Diagnostic Persistence and Multidisciplinary Collaboration.

Actinomycosis is a rare, chronic bacterial infection caused by Actinomyces species, characterized by granulomatous inflammation, abscesses, and sinus tracts. It primarily affects the cervicofacial region and often mimics other conditions such as malignancies or tuberculosis, complicating early diagnosis and treatment. This case report details an 18-year-old male with no known comorbidities, who presented with a two-week history of facial swelling, trismus, and discharging fistulas following an insect bite. The delay in seeking medical attention was due to initial symptom subsidence and challen…
