[Miliary, ganglionar, pancreatic and costal tuberculosis. Clinical presentation and literature review].

A 42-year-old man was admitted to the hospital with fever, abdominal pain and miliary radiological pattern. The amylase and lipase in serum, and the amylase in urine were increased. The tuberculin skin test was negative. The thoracic and abdominal CT scan showed costal affectation, multiple focal hepatic lesions and large necrosing peripancreatic lymphadenopathies. The transbronchial biopsy and the pancreatic punction demonstrated granulomatous necrotic lesions, and M. tuberculosis grew in the bronchial washing culture. The specific treatment normalized the pulmonary, costal and pancreatic les

Effects of Solanum lycopersicum L. (tomato) against isoniazid and rifampicin induced hepatotoxicity in wistar albino rats.

Anti-tuberculosis drugs are reported to cause hepatotoxicity, which varies from asymptomatic rise of the hepatic enzymes. Hepatoprotective plants plays important role to protect liver. This study investigated the hepatoprotective potential of the Solanum lycopersicum in rats intoxicated with Isoniazid and Rifampicin (INH+RIF) to induce hepatotoxicity. Thirty wistar albino rats were divided into five groups of six animals each. Group 1 rats were kept control while groups II, III, IV and V were administered with INH+RIF (75+150 mg/kg) orally, for seven consecutive days. For treatment, rats in gr

Hepatic granulomas in Northern Ireland: a thirteen year review.

The finding of epithelioid cell granulomas within liver biopsies is a not uncommon occurrence. We undertook this study to investigate the underlying conditions responsible for a diagnosis of granulomatous hepatitis in Northern Ireland during the thirteen year period 1980-1992. One hundred and sixty-three patients with hepatic granulomas were identified, accounting for 4% of all liver biopsies undertaken during the period of the study. In 145 cases (89%) a definite clinical diagnosis was established. The most common clinical diagnoses were primary biliary cirrhosis which accounted for 90 cases

[Portal and splenic veins thrombosis reveling a miliary tuberculosis of the liver].

The etiologies of the portal vein trombosis are dominated by the neoplasic forms with hepatocellular carcinoma; we report a rare case of portal and splenic veins thrombosis revealing a liver military tuberculosis occuring in a HIV 1 infected patient. A 42 years old senegalese woman with no personal or family history of thrombosis was admitted for abdominal upper right quadran, and epigastric pain, with fever and important weight loss. Ultrasound identified endoluminal echogenic images in the portal and splenic veins. There were no lymph nodes or liver tumor. Evaluations of proteins C and S wer

Unusual Presentation of Tuberculous Thyroid Abscess in a Background of Hashimoto's Thyroiditis in a Chronic Hepatitis B Carrier.

Tuberculosis of thyroid gland is a very rare disease. It has variable presentations and may be sometimes associated with autoimmune thyroiditis. We report a case of 45-year-old male, with left sided painless neck swelling, with a purulent discharging sinus over it associated with night sweats and loss of appetite. Thyroid imaging disclosed heterogeneous enhancement of left lobe of thyroid gland with internal vascularity and coarse calcifications. Core needle biopsy revealed caseous necrosis and AFB positivity. Patient had thyroid peroxidase antibody and thyroglobulin antibody positivity and th

Tuberculosis in orthotopic liver transplant patients: increased toxicity of recommended agents; cure of disseminated infection with nonconventional regimens.

BACKGROUND: Because increased hepatotoxicity was observed with first line antituberculous agents using four drug standard induction therapy in orthotopic liver transplant patients, we evaluated the efficacy and adverse effects of a novel continuation regimen for the treatment of tuberculosis in orthotopic liver transplant patients at a University Hospital in New York City. METHODS: The hospital records of all patients who were referred to Mount Sinai Hospital (n=924) and who underwent orthotopic liver transplant between September 1988 and May 1998 were reviewed. Data were collected from patie

Baseline abnormal liver function tests are more important than age in the development of isoniazid-induced hepatoxicity for patients receiving preventive therapy for latent tuberculosis infection.

BACKGROUND: One of the cornerstones of Australia's public health programmes to eliminate tuberculosis (TB) is the identification and treatment of latent tuberculosis infection (LTBI). AIMS: The main aim of this study is to determine the demographics, compliance, completion rates and adverse events of patients on preventive therapy (PT) for LTBI at our institution. The secondary aim is to determine the rates of isoniazid (INH) hepatotoxicity and identify any contributory factors. METHODS: The method used was an audit using medical records of 100 consecutive patients (2010-2014) treated with P

Actinomycosis With Pulmonary and Hepatic Involvement: A Case Report and Clinical Insights.

Actinomycosis is a rare, chronic infectious disease caused by Actinomyces spp., characterized by an indolent and slowly progressive course. It represents a diagnostic challenge since its nonspecific clinical features often lead to misdiagnosis, mimicking pathologies such as solid neoplasms, active tuberculosis, nocardiosis, fungal infections, or other granulomatous diseases. This study describes a 56-year-old male with abdominal and thoracic pain, weight loss, fever, and dyspnea, over a two-week period. Imaging revealed the presence of hepatic abscesses and right-sided pleural effusion. Actino

[Diagnosis and treatment of latent tubercular infections].

Despite dramatic advances in diagnosis and treatment, tuberculosis (TB) remains one of the leading causes of morbidity and mortality worldwide. World Health Organization (WHO) reports that one-third of the global population is infected by M. tuberculosis. Direct identification of individuals who are latently infected with live M. tuberculosis without active disease is currently not possible (Latent TB Infection, LTBI). Available immunodiagnostic tests, i.e. Tuberculin Skin Testing and Interferon-gamma Release Assay, ascertain a state of M. tuberculosis specific immune response; they have sever

Clinical aspects and treatment outcome in HIV-associated pulmonary tuberculosis: an experience from a Thai referral centre.

This case-control study examined the impact of HIV infection on clinical presentation, response to treatment, and outcome of pulmonary tuberculosis. Symptoms, radiographic pattern, sputum direct smear, drug susceptibility, treatment outcome and adverse reactions of 88 HIV-infected patients with newly-diagnosed, culture-proved, untreated pulmonary tuberculosis were compared with those of age and gender-matched HIV-seronegative patients. No differences in the frequency of pyrexia, dyspnoea, cough or haemoptysis were evident. Cavitary lesions and upper zone infiltrates were observed significantly

[Unusual therapeutic approach in treatment of pulmonary tuberculosis in six year old girl].

We report about interesting case of treatment of pulmonary tuberculosis in 6-year old girl. Antituberculotic therapy induced toxic hepatitis (rise of transaminases, indirect hiperbilirubinemia). She was treated with streptomycin, etambutol, ciprofloxacyn and systemic corticosteroides. Two gallbladder stones and high-density sediment were diagnosed after ultrasound examination which could be a side effect of the therapy or previous findings.

"Bypass arthritis" after gastroileostomy.

We describe a patient who underwent inadvertent gastroileostomy, and subsequently developed malabsorption, granulomatous hepatitis, arthritis, and reactivation of tuberculosis accompanied by circulating immune complexes and lowered C1q. Surgical correction resulted in marked improvement, with disappearance of arthritis, return to normal of complement levels, and absence of demonstrable circulating immune complexes. Arthritis has not been documented in the spectrum of abnormalities described after gastroileostomy.

Late-onset paradoxical response 11 years after treatment of tuberculous hepatic abscesses in an HIV-negative patient: a case report and literature review.

Paradoxical responses (PRs) to anti-tuberculosis (anti-TB) treatment refer to the worsening of pre-existing tuberculous lesions or the emergence of new lesions in patients whose clinical symptoms initially improved with therapy. PRs are less common in HIV-negative patients, and presentations as tuberculous hepatic abscesses are rare. Furthermore, PRs occurring after completion of TB treatment are uncommon, making it difficult to distinguish them from TB relapse. We report herein a case of late-onset post-treatment PR, presenting 11 years after completion of treatment for a tuberculous hepatic

Liver involvement in tuberculosis--an autopsy study.

OBJECTIVE: To study the spectrum of histopathological changes in the liver in cases of tuberculosis. MATERIALS & METHODS: This is an autopsy study consisting of 150 adult cases of tuberculosis over a period of 3 years. The diagnosis of tuberculosis was made on the basis of caseating granulomas or acid fast bacilli in the tissue. Cases showing only healed foci of tuberculosis at autopsy were excluded. Clinical details of all cases were obtained from hospital records. At autopsy, gross examination was done meticulously with special emphasis on the liver and extrahepatic biliary tree. Sections f
