Dysphagia due to isoniazid therapy for tuberculosis in a patient with Lewy body dementia.

Dementia with Lewy bodies (DLB) potentially causes dysphagia; however, the features and underlying mechanism causing dysphagia have still not been clarified. We are the first to report a case of dysphagia resulting from isoniazid therapy for tuberculosis in a DLB patient. A 74-year-old woman with DLB developed dysphagia during treatment for tuberculous pleurisy. Oral videoendoscopic and videofluorographic swallowing examinations showed oropharyngeal dysphagia. The increased administration of levodopa successfully ameliorated dysphagia. Therefore, dysphagia was ascribed to diminished levodopa e

Births, deaths and medical emergencies in the district: a rapid participatory appraisal in Nepal.

Two qualitative rapid appraisal techniques were used in a community-based study to investigate health beliefs, attitudes and practices in a rural district in Nepal. Pregnancy and childbirth remain non-medical activities managed at home. Deaths may be avoided by better access to hospital emergency services. Health awareness of some problems, e.g. oral rehydration and diarrhoeal disease, is high; but of others, e.g. association of haemoptysis with tuberculosis, is poor. These methods require few resources, are efficient in time and manpower required, and generate useful relevant information on a

Tuberculosis in hemodialysis: A diagnostic and therapeutic challenge.

Tuberculosis (TB) is a significant clinical and public health problem for patients with chronic kidney disease (CKD), particularly those requiring hemodialysis (HD). This population is particularly vulnerable due to the profound alteration of the immune response characteristic of CKD, which significantly increases the risk of reactivation of latent TB. The Mantoux test, for example, is not suitable for diagnosis due to the low sensitivity of these patients and the high rate of false positives due to vaccination. In contrast, IGRAs (interferon-gamma release assays) show superior sensitivity and

Proapoptotic Bcl-2 inhibitor as potential host directed therapy for pulmonary tuberculosis.

Mycobacterium tuberculosis establishes within host cells by inducing anti-apoptotic Bcl-2 family proteins, triggering necrosis, inflammation, and fibrosis. Here, we demonstrate that navitoclax, an orally bioavailable, small-molecule Bcl-2 inhibitor, significantly improves pulmonary tuberculosis (TB) treatments as a host-directed therapy. Addition of navitoclax to standard TB treatments at human equipotent dosing in mouse models of TB, inhibits Bcl-2 expression, leading to improved bacterial clearance, reduced tissue necrosis, fibrosis and decreased extrapulmonary bacterial dissemination. Using

MmpL3 as a Target for the Treatment of Drug-Resistant Nontuberculous Mycobacterial Infections.

Nontuberculous mycobacterial (NTM) pulmonary infections are emerging as a global health problem and pose a threat to susceptible individuals with structural or functional lung conditions such as cystic fibrosis, chronic obstructive pulmonary disease and bronchiectasis. Mycobacterium avium complex (MAC) and Mycobacterium abscessus complex (MABSC) species account for 70-95% of the pulmonary NTM infections worldwide. Treatment options for these pathogens are limited, involve lengthy multidrug regimens of 12-18 months with parenteral and oral drugs, and their outcome is often suboptimal. Developme

A review of the use of ethionamide and prothionamide in childhood tuberculosis.

Ethionamide (ETH) and prothionamide (PTH), both thioamides, have proven efficacy in clinical studies and form important components for multidrug-resistant tuberculosis treatment regimens and for treatment of tuberculous meningitis in adults and children. ETH and PTH are pro-drugs that, following enzymatic activation by mycobacterial EthA inhibit InhA, a target shared with isoniazid (INH), and subsequently inhibit mycolic acid synthesis of Mycobacterium tuberculosis. Co-resistance to INH and ETH is conferred by mutations in the mycobacterial inhA promoter region; mutations in the ethA gene ofte

An attenuated Salmonella-vectored vaccine elicits protective immunity against Mycobacterium tuberculosis.

There is a need to develop protective vaccines against tuberculosis (TB) that elicit full immune responses including mucosal immunity. Here, a live attenuated Salmonellatyphimurium aroA SL7207 vector TB vaccine, namely SL(E6-85B), harboring the Mycobacterium tuberculosis (M. tb) H37Rv ESAT6-Ag85B fusion gene was developed. The experimental data demonstrated that this SL(E6-85B) vaccine, or when it is combined with BCG vaccination, induced the strongest TB Ag-specific mucosal, humoral, and cellular immune responses comprised of increased proliferation of T cells, IFN-gamma expression, granzyme

Vaccination of cattle with Mycobacterium bovis BCG by a combination of systemic and oral routes.

Mycobacterium bovis bacille Calmette-Guérin (BCG) vaccine delivered to calves by the subcutaneous (s.c.) or by the oral route in a formulated lipid matrix has been previously shown to induce similar levels of protection against bovine tuberculosis. The current study was aimed at determining whether a combination of delivering BCG by s.c. and oral routes would enhance levels of protection, compared to only one route of vaccination. Forty calves were randomly divided into four groups (10/group). Calves were vaccinated with 10(6)colony forming units (CFU) of BCG Pasteur by the s.c. route or orall

[Public health problems in relation to refugees].

In Hessen, obligatory health examination of refugees includes the following: Clinical examination, faecal bacteriological examination for salmonella and shigella, chest x-ray of persons aged 3 years and older, lues- and hepatitis B serology in case of persons of 16 years of age and older. The results obtained with 3453 refugees were analysed: There were remarkable differences according to country of origin. In connection with Public Health, only two diseases are important: tuberculosis and lues. Early diagnosis and immediate treatment minimise the risks. Faeco-orally transmitted diseases (for

Isolated Forearm Abscess with Bilateral Psoas Abscesses: An Unusual Presentation of Extrapulmonary Preextensively Drug-resistant Tuberculosis without Pulmonary Involvement.

Extrapulmonary tuberculosis (TB) accounts for 15%-20% of all TB, but soft-tissue involvement of the extremities is distinctly uncommon. Concurrent forearm and bilateral psoas abscesses from drug-resistant Mycobacterium tuberculosis, in a patient without pulmonary disease, have not - to our knowledge - been described. A 16-year-old immunocompetent girl reported right-sided chest pain, a progressively enlarging swelling on the left forearm, anorexia, and weight loss over roughly 2 weeks. Fever, cough, and night sweats were absent, as was any known TB contact. On examination, the forearm lesion w

Linezolid-induced peripheral neuropathy in a patient with multidrug-resistant pulmonary tuberculosis: a case report and comprehensive literature review.

BACKGROUND: Multidrug-resistant tuberculosis (MDR-TB) remains a major global health challenge due to the limited availability of effective therapeutic options. Among second-line agents, linezolid has demonstrated excellent efficacy in both MDR-TB and extensively drug-resistant tuberculosis (XDR-TB), owing to its high oral bioavailability, good tissue penetration, and low resistance profile. However, its long-term use is frequently limited by adverse effects-most notably peripheral neuropathy, along with optic neuritis and myelosuppression-which often lead to early treatment discontinuation and

Challenges in causality assessment of renal dysfunction during sequential treatment for presumed rifampicin-resistant tuberculosis in an elderly patient.

BACKGROUND: Renal impairment during treatment for rifampicin-resistant tuberculosis (RR-TB) is uncommon but clinically significant, especially in elderly patients with limited physiological reserve. Determining whether renal dysfunction is attributable to anti-tuberculosis drugs or patient-related factors remains a major clinical challenge, particularly following sequential regimen changes. CASE PRESENTATION: A 74-year-old man with a low body mass index (17.5 kg/m²) was treated for presumed RR-TB based on a single Xpert MTB/RIF assay showing rifampicin resistance despite persistently negative

Oral isoniazid causes oxidative stress, oocyte deterioration and infertility in mice.

Isoniazid (INH), a synthetic first-line tuberculosis antibiotic, has been widely used in clinical treatment. It has been reported to cause toxic effects at multiple tissue sites and also increases the incidence of adverse pregnancy outcomes; but the mechanism of action of INH on the reproductive system of female mammals remains unclear. Here, we demonstrate that oral INH (40 mg/kg/day every other day for 28 days) severely affects oocyte maturation and fertilization, late blastocyst development and fertility. We found that INH could disrupt standard spindle assembly, chromosome arrangement, and
