[Granulomatous spondylodiscitis: due mainly to tuberculosis but lymphoma cannot be excluded].

Lower back pain is due to multiple etiologies that make diagnosis difficult. Primitive spinal lymphoma is rare and its diagnosis often requires ultrasound-guided biopsy. A 30-year old man hospitalized for inflammatory lumbago evolving within the context of an impaired general condition. Phisical examination revealed pain on palpation of the L2-L3 vertebral apophysis without peripheral tumor syndrome. Laboratory tests showed an inflammatory syndrome. Morphological assessment was in favour of a spondylodiscitis. The first biopsy showed granulomatous osteitis.Clinical and radiological worsening d

British Infection Society guidelines for the diagnosis and treatment of tuberculosis of the central nervous system in adults and children.

SUMMARY AND KEY RECOMMENDATIONS: The aim of these guidelines is to describe a practical but evidence-based approach to the diagnosis and treatment of central nervous system tuberculosis in children and adults. We have presented guidance on tuberculous meningitis (TBM), intra-cerebral tuberculoma without meningitis, and tuberculosis affecting the spinal cord. Our key recommendations are as follows: 1. TBM is a medical emergency. Treatment delay is strongly associated with death and empirical anti-tuberculosis therapy should be started promptly in all patients in whom the diagnosis of TBM is sus

Central nervous system mycobacteriosis caused by Mycobacterium genavense in degus (Octodon degus).

Degus (Octodon degus) that were kept at a breeding facility presented with neurological or respiratory symptoms and died. Necropsies were performed on 9 individuals, and no significant gross lesions were found. Histologically, spinal cord necrosis was observed in all 9 cases and granulomatous myelitis in 5 of the 9 cases. Locally extensive necrosis of the brain and encephalitis were observed in 7 of the 9 cases. Acid-fast bacteria were found in the spinal cords, brains, and lungs from all 9 cases. Immunohistochemically, Mycobacterium tuberculosis antigen was observed in the spinal cords, brain

A Rare Complication of Tuberculosis: Acute Paraplegia.

BACKGROUND: Tuberculous radiculomyelitis(TBRM) is one of the complications of neurological tuberculosis and includes cases of arachnoiditis, intradural spinal tuberculoma or granuloma, and spinal cord complications of tuberculous meningitis (TBM). Here, we report a case of TBRM which presented with acute paraplegia. CASE DETAILS: Neurological examination on admission revealed flaccid paralysis, bilateral extensor plantar responses, and exaggerated deep tendon reflexes. Cerebrospinal fluid analysis showed xanthochromic fluid that contained 600 cells/mm3, 98% lymphocytes, protein 318 mg/dl and

Basal Ganglia Ischemic Stroke: The Unforeseen Progression of Tuberculosis Epididymo-Orchitis.

Tuberculosis is an infectious disease with broad pulmonary and extrapulmonary clinical manifestations. Central nervous system tuberculosis (CNS-TB) is a complex extrapulmonary infection known for its diverse clinical features including meningitis, tuberculoma, and spinal arachnoiditis. Particularly, tuberculosis meningitis can further lead to complications such as ischemic stroke.  This article presents a challenging case of a 35-year-old male patient initially diagnosed with epididymo-orchitis, followed by viral-like central nervous system symptoms, ultimately complicated by tuberculosis meni

[One stage posterior vertebral column resection for the treatment of thorciclumbar tuberculosis with kyphotic deformity].

OBJECTIVE: To evaluate the clinical outcome of one stage posterior vertebral column resection in patients with spinal tuberculosis combined with kyphotic deformity. METHODS: Thirty-six patients with spinal tuberculosis combined with kyphotic deformity underwent posterior one-stage vertebral column resection reducing tension on the spinal cord from 1998 to 2006. The patients were mobilized with a thoracolumbar orthosis for 3 months. All patients had a minimum of a 2-year follow-up, and clinical examinations and radiographs were obtained at 6-month intervals. RESULTS: No perioperative mortalit

Tuberculous spondylodiscitis with atypical presentation: A diagnostic challenge: Case report and review of literature.

Spinal tuberculosis is the most common form of osteoarticular tuberculosis and presents significant diagnostic challenges, especially in atypical cases. It may clinically and radiologically mimic neoplastic lesions, particularly when disc involvement is absent. We report a case of a 60-year-old woman with a history of treated lymph node tuberculosis who presented with chronic inflammatory dorsolumbar pain without systemic symptoms. Initial imaging revealed lytic lesions of the D11-D12 vertebrae with preservation of the intervertebral disc, raising suspicion of a neoplastic process. A PET scan

The radiological diagnosis of tuberculosis of the adult spine.

Tuberculosis remains endemic in the United States with an estimated incidence of 15.9 cases per 100,000 population. In North America and Saudi Arabia tuberculosis of the spine is primarily a disease of adults. In Saudi Arabia the average age on presentation is 41 years. Spinal tuberculosis begins classically in the anterior inferior portion of a vertebral body. The infection spreads beneath the anterior longitudinal ligament to involve adjacent vertebral bodies. Disc space narrowing is a secondary phenomenon, occurring when destruction of the cancellous bone permits herniation of the disc into

[Elastic materials in surgery for inflammatory diseases of the vertebral column].

When 17 patients were surgically treated for spinal tuberculosis and osteomyelitis, anterior spine fusion was performed by A. Ye. Garbuz's procedure that was based on the replacement of a spinal defect by a carbon-carbonic implant that had rifampicin containers in the frontal sections, as well as by autologous bone grafts. The carbon-carbonic implants reliably fix an operated spinal portion, prevent the progression of kyphotic deformity, and create good conditions for autologous bone grafts to knit. An osteocarbonic block forms in the late postoperative period.

Magnetic resonance imaging of spinal infections.

The manifestations of infectious disease of the spine are numerous. Viruses commonly affect the spinal cord and meninges, while tuberculosis and various bacterial and fungal organisms may involve the vertebrae, intervertebral disks, epidural and paraspinal regions, the leptomeninges, and, rarely, the cord itself. Magnetic resonance imaging, with its multiplanar capability and superb contrast resolution, is definitely the modality of choice in evaluating the patient with suspected infection of the spinal column or spinal cord and its coverings. The addition of postcontrast images is often helpf
