Pharmacological interventions for non-ulcer dyspepsia.

BACKGROUND: The commonest cause of upper gastrointestinal symptoms is non-ulcer dyspepsia (NUD) and yet the pathophysiology of this condition has been poorly characterised and the optimum treatment is uncertain. It is estimated that 450 million pounds is spent on dyspepsia drugs in the UK each year. OBJECTIVES: This review aims to determine the effectiveness of six classes of drugs (antacids, histamine H2 antagonists, proton pump inhibitors, prokinetics, mucosal protecting agents and antimuscarinics) in the improvement of either the individual or global dyspepsia symptom scores and also quali…

Chronic lower limb ulcer? No--chloroma!

Extra medullary deposits are often associated with myeloproliferative disorders, especially with myeloid neoplasia. These are called granulocytic sarcoma, aleukemic leukemia cutis, myeloblastoma, chloroleukemia, extra-medullary myeloid tumor, and chloroma. They commonly present in the bone, periosteum, soft tissue, lymph nodes, and the skin but can occur anywhere. The authors present their experience with a patient with no history of neoplasm presented with a nonhealing ulcer of the lower limb that was a chloroma associated with chronic myeloid leukemia.

Microsurgical treatment of Mooren's corneal ulcer.

This study evaluated the effect of microsurgery of lamellar keratoplasty (LKP) on Mooren's corneal ulcer. The surgical effect, postoperative astigmatism, postoperative vision, postoperative ulcer recurrence, and surgical complications of 2 groups of consecutive Mooren's ulcer inpatients treated respectively by non-microsurgery of LKP and microsurgery of LKP were analyzed. There were significant differences of the effect, postoperative astigmatism, postoperative vision, postoperative ulcer recurrence, and surgical complications between the 2 groups. The effect and postoperative vision of the mi…

[Gastric ulcer and Helicobacter pylori infection].

Helicobacter pylori infection and the use of non-steroidal anti-inflammatory drugs (NSAID) are considered to be the two major risk factors implicated in the development of gastric ulcer. Helicobacter pylori infection related chronic gastritis is known to be the underlying condition which may lead to gastric ulcer. Development of gastric ulcer as the consequence of underlying chronic gastritis is caused by many factors. Treating Helicobacter pylori infection entails the healing of gastric ulcer, it concomitantly prevents recurrences and complications of gastric ulcer, primarily bleeding, and ch…

[Surgical treatment of perforated gastric ulcer].

INTRODUCTION: Peptic ulcer perforation is a complication of ulcer disease which requires urgent surgical treatment. The aim of this paper was to point out our experience in surgical treatment of perforated peptic ulcer. MATERIAL AND METHODS: This retrospective study analyzes results of surgical treatment in 365 patients with perforated peptic ulcer during the period January 1996 to December 2000. RESULTS: During the last 5-year period 365 patients were treated following peptic ulcer perforation. The average age was 43.53 +/- 8.26, with the span from 18 to 86. The most frequent surgical proce…

[Peptic ulcer hemorrhage in the elderly].

BACKGROUND: The aim of this study was to investigate the efficacy of endoscopic injection therapy on the clinical outcome of elderly patients with peptic ulcer bleeding. METHODS: From January 1995 to December 1998, 738 patients with acute peptic ulcer bleeding were observed in the First Division of General Surgery, University of Verona: 359 aged <70 years and 379 =/>70 years. History, clinical and endoscopic findings and outcome were prospectively collected and analyzed comparing old (=/>70 yrs) and young (<70 yrs) patients. Ulcers with active or sign of recent bleeding were submitted to inje…

Risk factors for duodenal ulcer disease.

OBJECTIVE: To evaluate the role played by some of the traditional risk factors on the prevalence of duodenal ulcer disease in Jordan, where the prevalence of Helicobacter pylori is high. METHODS: This study was carried out in the Endoscopy unit at the Health Center, Jordan University of Science and Technology, Irbid, Jordan, during the period January 1998 through to july 1999. The patients' group consisted of 290 subjects with endoscopically proven duodenal ulcers and 161 subjects who were endoscoped for various indications and found to have no ulcers were considered as controls. Validated qu…

[Surgical treatment of gastroduodenal ulcer hemorrhage].

The results of treatment of patients with gastroduodenal ulcerous bleeding over 20-year period were analyzed. From 1997 active individualized policy was used in the treatment of 522 patients. In duodenal ulcer the drug-thermal vagotomy was applied as well as traditional organ-saving operations, in combination with extraduodenisation of ulcer when it penetrates into pancreatic head. In gastric ulcer sparing resection or methods of ulcer's taking outside gastrointestinal tract (extragastration of ulcer) with preservation of gastric angio-, and neuroarchitectonics were preferable. In preoperative…

[Risk factors for peptic ulcer recurrence].

UNLABELLED: Prevalence of peptic ulcer (PU) and risk factors associated with its recurrence are unknown among the Mexican population. OBJECTIVE: To determine clinical features and risk factors associated with peptic ulcer recurrence. MATERIAL AND METHODS: Five-year follow-up study that included 211 men and 140 women 20 years age or older. PU diagnosis was made by clinical and endoscopic criteria. Associated risk factors such as NSAIDs use, smoking, alcohol consumption, and H. pylori infection were assessed. Clinical recurrence was defined as the time between basal diagnosis and the first PU …

[Gastroduodenal ulcer associated with Helicobacter pylori].

Helicobacter pylori is associated with chronic peptic ulcer disease. The high prevalence of H. pylori infection tends to decrease in Western countries suggesting that the incidence of gastroduodenal ulcers not related to antiinflammatory drugs will fall. Abundant data support the leading role of H. pylori in peptic disease through dysregulation of acid production, induction of gastric metaplasia, and development of chronic gastritis. Endoscopy is the key to diagnosis, allowing direct assessment of H. pylori infection. H. pylori eradication is the major aim of peptic ulcer treatment, dramatical…

Solitary rectal ulcer syndrome in children.

Solitary rectal ulcer syndrome (SRUS) is rare in children. Few reported cases have undergone detailed investigations, treatments have been extremely varied and outcome poorly documented. We report two cases of SRUS in children, each with a different macroscopic pathology. The diagnosis was delayed in both cases. The importance of appropriate investigation and the need to tailor treatment to the type of lesion are emphasised. One case of SRUS was associated with a complete, full thickness rectal prolapse and symptoms improved after an abdominal sutured rectopexy. The other patient responded wel…

Marjolin's ulcer presenting in the neck.

'Marjolin's ulcer' is a term that has come to be used for epidermal squamous cell carcinomas that have developed in areas of chronic inflammation. To our knowledge this is the first ever report of a Marjolin's ulcer developing following long-term irritation of the skin of the neck. The history of the eponym is traced and reveals that Marjolin probably never actually described this pathological process.

Invasive mucormycosis in benign gastric ulcer.

Fungal elements are frequently noted overlying the base of chronic peptic ulcers of the stomach and it has been suggested that the fungi enhance the degree of necrosis and that these cases have protracted disease and deeper ulcers with more perforations. It has also been postulated that the number of fungal elements might be increased in the stomach of patients who are receiving potent medications such as H2-receptor antagonists to reduce gastric acidity, but there have not been adequate control studies, and the deleterious effects from the presence of the fungi in these cases have not been su…
