Ultraviolet (UVR) phototherapy is widely used to treat an array of skin conditions, including psoriasis, eczema and vitiligo. For such interventions, a quantified dose is vital if the treatment is to be both biologically effective and to avoid the detrimental effects of over-dosing. As dose is absorbed at surface level, the orientation of patient site with respect to the UVR lamps modulates effective dose. Previous investigations have modelled this behaviour, and examined the impact of shaped anodized aluminium reflectors typically placed around lamps in phototherapy cabins. These mirrors are

Melasma is a common acquired hypermelanosis that primarily affects women of skin type IV-VI. It tends to appear on sun-exposed areas of face and neck. The exact pathogenesis is linked with many factors include ultraviolet radiation, pregnancy, hormonal activity, thyroid abnormalities, and medications. Melasma, which involves increased melanin production and melanocytosis, may be principally epidermal, dermal, or mixed. Mainstays of treatment include topical hypopigmenting agents, lasers, chemical peels, and dermabrasion.

Psoralen and ultraviolet light A (PUVA) was the mainstay of vitiligo treatment before the introduction of narrowband UVB(NB-UVB). Eighteen patients treated with NB-UVB in Hospital Kuala Lumpur were retrospectively analyzed. Eight patients had Fitzpatrick skin phototype III and 10 with phototype IV. The mean duration of disease was 7.3 +/- 5.4 years, and the mean body surface area affected was 19.3 +/- 16.2%. Mean duration of treatment was 14.3 +/- 9.6 months, number of session 113.5 +/- 88.6 times and cumulative dose 111.7 +/- 108.5 J/ cm2. Concomitant topical steroid was used in 88.9% patient

Nevus depigmentosus is an uncommon hypopigmented macule or patch that is congenital and stable in its relative size and distribution throughout life; it occurs sporadically and may be localized, segmental or, less often, systematized. We report the case of a 17-year-old girl with a segmental achromic nevus of the left leg and a patch on the lower back with late age of onset who developed lentigines after prolonged intense ultraviolet B exposure as a consequence of an incorrect diagnosis of segmental vitiligo.

UVR effects on human health are considered. The critical organs for UVR are skin, eye, and the immune system. In the skin, UVR can induce erythema, tanning, vitamin D photosynthesis, premature aging, and cancer. UVR has a specific immunosuppressive effect on the skin secondary to altered lymphoid cells function. It results in inhibition of the anti-neoplastic activity and suppression of contact hypersensitivity and delayed hypersensitivity to various antigens. UV can induce photokeratoconjunctivitis and play an active role in etiology of cataracts, corneal and retinal degeneration, pterigium a

Polymorphous light eruption (PLE) is a common disorder characterized by a delayed, abnormal response to ultraviolet (UV) radiation, with a varied morphology of itching efflorescences on sun-exposed areas of the skin. Thirty-one PLE subjects were treated with either UVA (340-400 nm) or UVA and UVB (300-400 nm) phototherapy during spring 1987 (10 exposures to UV light). They were randomly allocated to these 2 groups. For subjects of the UVA group, the applied dose corresponded to their individual minimal tanning dose; for subjects of the UVA and UVB group it corresponded to approximately 3/4 of

A severe case of solar urticaria induced by UVA with positive results of passive transfer test (3 recipients) and reverse passive transfer test (2 recipients) is reported. Desensitization treatment by irradiation of particular parts of the body with UVA and UVB (to accelerate tanning) was fully effective. A marked increase of the tolerance to UV remained during the period of over five years of follow-up.

The beneficial effect of sunburn-spectrum ultraviolet (UVB) phototherapy on uremic pruritus was studied. Seven patients were treated twice weekly for 4 weeks with UVB to one half of the body and placebo phototherapy to the other half. All patients noted generalized improvement without localization of benefit to the UVB side, suggesting a systemic effect of UVB. A comparison of three schedules varying from one to three treatments weekly showed that the percentage of patients responding was not influenced by frequency of UVB exposure, although patients treated more intensively improved faster. I

Two groups of patients with subtotal to universal areate type alopecia were serially treated, 6 with intralesional triamcinolone acetonide suspension and 6 with contact irradiation by ultraviolet light from a high-pressure mercury-arc source with a quartz envelope (Kromayer lamp). The treatment was limited to one half of the scalp while the untreated half served as a control. Triamcinolone acetonide suspension gave in 2 of the 6 treated patients a temporary stimulating effect on hari growth; later, however, hair growth inhibition was noted in 3 patients. Contact irradiation with ultraviolet li

Electron microscopy was used to define the ultrastructure of the capillary loops in the dermal papillae of psoriatic lesions. Pustular psoriasis of von Zumbusch and psoriasis vulgaris were studied before and after treatment with the Goeckerman regimen. Capillary loops were reconstructed from 1-mum plastic-embedded sections. Ultrathin sections were taken at intervals for correlation with the 1-mum sections. There were no ultrastructural differences between the capillary loops in psoriasis vulgaris and pustular psoriasis. The intrapapillary portion of the loop was predominantly a venous capillar
