BACKGROUND: Surgical resection is the only potentially curative treatment for pancreatic and periampullary cancer. A considerable proportion of patients undergo unnecessary laparotomy because of underestimation of the extent of the cancer on computed tomography (CT) scanning. Bronchogenic cysts are usually discovered only incidentally in the adult. A giant bronchogenic cyst in a 19-year-old woman presenting with pain and shortness of breath was mistaken for tension pneumothorax and initially treated with tube thoracostomy. Giant bullae were diagnosed by computed tomography. Bullae resection was undertaken, but the remaining lung tissue required pneumonectomy. Pathologic examination of the specimen confirmed bronchogenic cyst.

We report the case of a gentleman referred with apparent worsening function in a hydronephrotic left-sided pelvic kidney. Imaging with Tc-99m-Mercaptoacetyltriglycine (MAG 3) in 1999, showed that the left kidney contributed 46% of renal function with no evidence of obstruction. Three years later, repeat scanning showed ectopic kidney function had decreased to 20% and with worsening pain, nephrectomy had been suggested. Benign fibrous histiocytoma is a rare bone tumor that usually presents with localized pain and/or a pathological fracture. We report on a patient with adenocarcinoma of the prostate who had a radionuclide bone scan for staging, which was suspicious for metastatic disease. Histologic examination of left eleventh rib revealed a Benign Fibrous Histiocytoma (BFH). To our knowledge, this is the first BFH found solely on bone scan.

The free autogenous gingival graft is an excellent surgical procedure for treating gingival extension. It is a procedure that can be performed rapidly with little postoperative trauma to the patient. Its greatest feature is the high degree of predictability of success in producing a specific dimension of masticatory mucosa. However, we have a moral and ethical obligation to our patients to be more selective in its use in treating gingival recession. Too many patients have been subjected to unnecessary surgery, especially in cases in which the recession has been of long duration and is stable.

OBJECT: Tethering of the spinal cord is thought to increase the chance of neurological injury when scoliosis correction is undertaken. All patients with myelomeningocele (MM) are radiographically tethered, and untethering procedures carry significant morbidity risks including worsening neurological function and wound complications. No guidelines exist as regards untethering in patients with MM prior to scoliosis correction surgery.
