During the 1970s, parietal cell vagotomy (PCV) gradually became accepted as a suitable method in the surgical treatment of prepyloric, pyloric and duodenal ulcer disease. This study reports the data from a study of 405 consecutive patients with chronic ulcer disease treated with PCV. Mortality was low (0.5%) and there were few postoperative sequelae (periodic loose stools in 2% and mild dumping in 2%). Twenty-two patients with gastric ulcers located at the incisura angularis underwent parietal cell vagotomy without drainage and an intraluminal excision of the ulcer through a small gastrotomy on the greater curvature. These patients were followed up for two to five years. Results were excellent to good in 87% and fair in 5%; 9% of the procedures were failures. One failure was due to recurrent ulcer, possibly related to ingestion of a dopamine antagonist. 343 white Wistar rats were divided into three groups of 113, 115 and 115 animals in each group. A control group received an injection of acetic acid into the wall of the antrum whereas the two other groups underwent first, in one group, a vagotomy and pyloroplasty, in the other, a pyloroplasty alone. Thirteen mongrel dogs equipped with four bipolar electrodes and three strain gauges on the anterior wall of the stomach underwent three types of vagotomy: truncal vagotomy (TV)-6 dogs; selective proximal vagotomy (SPV)-3 dogs; and antral vagotomy (AV)-4 dogs. 107 patients were examined in different terms after vagotomy (up to 6 years) performed for ulcerous disease of the duodenum, 43 of them--prior to and after the operation. It is found that the suppression of the acid- and enzyme secretory function of the stomach is not related to the development of the mucosa atrophy, but it is conditioned by eliminating the resolving role of vagus nerves and by a reduced sensitivity of fundic glands to humoral stimuli.
