Initial Single-Center Experience With the Fully Repositionable Transfemoral Lotus Aortic Valve System.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) has become the standard therapy for patients with severe symptomatic aortic stenosis and unacceptable high risk for surgical aortic valve replacement. Several different devices for TAVR have been introduced so far, each of them with unique features. AIMS: To analyze our first real-world experience with the second- generation, mechanically expanded, fully repositionable transfemoral Lotus TAVR device (Boston Scientific). RESULTS: Between August 2014 and January 2016, a total of 298 patients were scheduled for transfemoral TAVR at our i

Computed Tomographic Angiography-Derived Risk Factors for Vascular Complications in Percutaneous Transfemoral Transcatheter Aortic Valve Implantation.

Transfemoral aortic valve implantation (TAVI) has become a viable alternative to surgical valve implantation, particularly for higher risk patients; however, vascular complications (VCs) remain a concern in transfemoral TAVI. We aimed to determine clinical and computed tomographic angiography-derived risk factors associated with Valve Academic Research Consortium (VARC)-2 criteria VCs in patients who underwent TAVI. From 2011 to 2017, 481 patients underwent percutaneous transfemoral TAVI at the Minneapolis Heart Institute and were screened for procedural and postprocedural access-related VC ac

Percutaneous decannulation with a suture-mediated closure device versus open surgical repair for peripheral veno-arterial extracorporeal membrane oxygenation: A systematic review and meta-analysis.

OBJECTIVE: Peripheral veno-arterial (VA) extracorporeal membrane oxygenation (ECMO) is commonly used to treat patients with severe cardiorespiratory dysfunction. Percutaneous decannulation (PD) using suture-mediated closure devices has emerged as an alternative to open surgical repair (OSR) following peripheral VA-ECMO. We performed a systematic review and meta-analysis to review and compare outcomes after VA-ECMO decannulation using PD versus OSR. METHODS: Six electronic databases were searched to 25 December 2025 for comparative studies evaluating PD versus OSR. Primary outcomes were access

Percutaneous Versus Surgical Femoral Cannulation in Minimally Invasive Cardiac Surgery: A Systematic Review and Meta-Analysis.

OBJECTIVE: Minimally invasive cardiac surgery (MICS) is increasing worldwide. In most cases, the surgical technique includes cannulation of the groin for the establishment of cardiopulmonary bypass, requiring a second surgical incision (SC) for exposure and cannulation of the femoral vessels. With the introduction of arterial closure devices, percutaneous cannulation (PC) of the groin has become a possible alternative. We performed a meta-analysis and systematic review to compare clinical endpoints between the patients who underwent PC and SC for MICS. METHODS: Three databases were assessed.

Incidence, predictors, impact, and treatment of vascular complications after transcatheter aortic valve implantation in a modern prospective cohort under real conditions.

OBJECTIVE: Vascular complications (VCs) occurring in transcatheter aortic valve implantation (TAVI) procedures have frequently been reported in the past. Considering significant technical improvements in delivery systems and vascular closure devices, the goal of this study was to determine the incidence, impact, and prognostic factors of VCs in a recent real-world cohort. METHODS: We report a bicentric prospective analysis of 479 consecutive patients who underwent TAVI between January 2017 and December 2017. VCs were defined according to criteria set out by the Valve Academic Research Consort…

Anesthetic management of Platypnea-orthodeoxia syndrome in a young patient with residual atrial septal defect following congenital heart surgery.

BACKGROUND: Platypnea-orthodeoxia syndrome (POS) is a rare condition causing dyspnea and hypoxemia that worsen in the upright position and improve when supine. While often reported in elderly patients, POS in young adults, particularly due to residual atrial septal defects (ASD) after congenital heart surgery, is uncommon. CASE PRESENTATION: We present a 24-year-old woman with POS caused by a residual ASD following congenital heart surgery. Agitated saline contrast transthoracic echocardiography confirmed marked right-to-left shunting exacerbated by positional change and Valsalva maneuver. An

Transvenous occlusion of patent ductus arteriosus in a domestic chicken (Gallus gallus).

A 2.5-year-old intact female Marans domestic chicken was presented for lethargy, open beak breathing, and hyporexia. Echocardiography noted left atrial and left ventricular enlargement and computed tomography angiography revealed a type III left-sided patent ductus arteriosus. Retrograde catheterization of the ductus was performed via percutaneous access of the right external jugular vein, and transvenous ductal occlusion was achieved using an 8-mm Amplatzer™ Vascular Plug 4. Transient bradycardia and hypotension occurred during right heart catheterization, which were successfully treated with

Endoscopic Pericranial Flap Repair of Occipital-Frontal Electrode Erosion.

INTRODUCTION: Occipital-frontal nerve stimulation is an off-label therapy for treating chronic refractory migraine and orofacial pain. Though effective, patients experience a high rate of complications including lead migration and erosion through the overlying skin. CASE DESCRIPTION: We present a case of frontal electrode erosion that was revised via pericranial flap repair. The patient presented with multiple lead migrations, necessitating multiple revision surgeries with eventual frontal wound dehiscence. The choice was made to wrap the electrode in a pericranial flap to prevent recurrent l

Alternative percutaneous management of iatrogenic femoral artery pseudoaneurysms using plug-based closure devices after failed us-guided compression and evaluation of risk factors for compression failure.

PURPOSE: Pseudoaneurysms (PAs) arise from the disruption of the arterial wall, resulting in extravasation of blood into the surrounding tissue. Iatrogenic sources are a leading cause of PA formation. There is an upward trend in the incidence of PAs due to the growing number of endovascular procedures. This study identifies the risk factors associated with the failure of Ultrasound-guided compression repair (UGCR) and evaluates the efficacy of vascular closure devices (VCDs) in the percutaneous treatment of iatrogenic femoral artery PAs. There are case reports in the literature, with a few case…

Surgical Explantation of a Fenestrated Endovascular Abdominal Aortic Aneurysm Repair Device Complicated by Aorto-Enteric Fistula.

INTRODUCTION: Alarming outcomes have been reported following infected endovascular aortic aneurysm repair (EVAR) device explantation. Infected fenestrated EVAR (FEVAR) exposes patients to even worse procedural risks. REPORT: A 67 year old man with a prior history of FEVAR presented with impaired general condition, abdominal and back pain, and increased C reactive protein. Computed tomography angiography revealed a collection around the aortic graft bifurcation and 18F-fluorodeoxyglucose-positron emission tomography (FDG-PET) revealed increased FDG uptake at this level, confirmed by labelled w

Endovascular Treatment of Vertebral-Venous Fistula with Flow-Diverting Stent.

BACKGROUND: Vertebral-venous fistulas (VVFs) are a rare vascular condition and are commonly associated with trauma but may also be idiopathic, spontaneous, or congenital. A current literature review reveals there is no level 1 evidence or established guidelines on optimal treatment for VVF. Diagnosis is often confirmed by catheter cerebral angiogram. Treatment goals are complete occlusion of the fistula, which can be achieved by both endovascular closure and/or surgical ligation treatment. Telescopic Pipeline flow-diversion stents are a common treatment modality for aneurysms and have also bee

Carotid artery direct access for mechanical thrombectomy: the Carotid Artery Puncture Evaluation (CARE) study.

BACKGROUND: In acute ischemic stroke due to anterior large vessel occlusion (AIS-LVO), accessing the target occluded vessel for mechanical thrombectomy (MT) is sometimes impossible through the femoral approach. We aimed to evaluate the safety and efficacy of direct carotid artery puncture (DCP) for MT in patients with failed alternative vascular access. METHODS: We retrospectively analyzed data from 45 stroke centers in France, Switzerland and Germany through two research networks from January 2015 to July 2019. We collected physician-centered data on DCP practices and baseline characteristic

Hybrid procedures can reduce the risk of congenital cardiovascular surgery.

BACKGROUND: Minimally invasive operations and percutaneous interventions are well-accepted options in the treatment of congenital heart defects. However, percutaneous interventions may be associated with an increased risk due to limited vascular access or a very tortuous catheter course. In these cases, combining operative and interventional approaches with direct puncture of the heart or the great vessels may facilitate implantation of even large devices. Furthermore, in some situations, cardiopulmonary bypass or circulatory arrest can be omitted when doing a hybrid procedure. PATIENTS: Betw
