In a previous study (1) it was found that in healthy subjects Ocular (Counter) Rotation is mainly due to otolith stimulation and only to a minor extent induced by slanted visual structures. Stimulation of the neck by tilting the trunk laterally upwards did not result in a systematic rotation of the eyes. In the present study it was found that subjects with bilateral loss of vestibular function showed a higher visually induced ocular rotation. Tilting the head (cervical stimulation) or the whole body (somatosensory stimulation) also led to a considerable OCR, demonstrating substitution of other

The authors report on a series of 18 patients operated through the posterior fossa approach for selective vestibular neurectomy. The indication for surgery consisted of intractable vertigo in cases of Ménière's disease. Results were excellent with a 94% cure of vertigo and 100% improvement. Postoperative hearing results were good with 78% of patients maintaining their discrimination score and 94% their pure tone average threshold. The results and the advantages of this approach are discussed and compared with the known procedures as reported in the literature.

Three patients with spasmodic torticollis following VIII nerve lesions (VIII-ST) underwent quantitative assessment of their sternomastoid EMG during vestibular (otolith and semicircular canal) stimulation. The results were compared with a normal control group and with six patients with idiopathic spasmodic torticollis (ST). Backwards tilt of the VIII-ST patients resulted in a marked increase in the EMG, especially in the more affected sternomastoid, whereas this manoeuvre did not have a significant effect in normal subjects, or had a variable effect in the ST group. These results suggest that

The activity of single motor units of the splenius-capitis muscle of healthy human subjects and patients with unilateral vestibular lesions was recorded by means of microelectrodes. The major part of these units reacted to vestibular stimulation in the same way as primary and secondary vestibular neurones of other vertebrates. The reactions of these units to trapezoidale stimuli is identical to that of vestibular neurones. An angular acceleration of more than 0.6 degrees/s2 is for these units a stimulus which is above threshold, this too corresponds to findings in the vestibular nerve. In pati

AIM: Diabetes mellitus (DM), a systemic disease, is a major public health concern. It is possible that micro and macrovascular changes in the inner ear due to DM could affect both auditory and vestibular functioning. However, there is a lack of conclusive evidence for vestibular impairment in individuals with DM. The current study investigated the effects of Type 2 diabetes mellitus (T2DM) on the functioning of the vestibular system and its duration dependency.
METHODS: Eighty individuals in the age range of 40-70 years (40 with T2DM and 40 age-matched healthy controls) underwent cervical ves

INTRODUCTION: Bilateral vestibulopathy (BVP) is a chronic, disabling disorder characterized by bilateral loss of vestibular function, leading to imbalance, oscillopsia, and falls. Despite established diagnostic criteria, clinical expression and rehabilitation potential vary widely across etiologies. Understanding these functional signatures is crucial for tailoring future neurostimulation strategies. The main objective of this study is to decode the etiological and functional heterogeneity of BVP and define clinical-functional profiles guiding vestibular or cochleo-vestibular implant candidacy

For 40 years, the amino acid acetyl-DL-leucine (or isoleucine - Tanganil) has been used in clinical practice to reduce imbalance and autonomic manifestations associated with acute vertigo crises. In animal models, acetyl-DL-leucine accelerates vestibular compensation following unilateral labyrinthectomy, and has only minor effects on normal vestibular function. Our work in animal models suggested that acetyl-DL-leucine acted mainly on abnormally hyperpolarized and/or depolarized vestibular neurons by restoring their membrane potential towards a mean value of -65 to -60 mV. Acute vestibular dis

BACKGROUND: The present study investigated whether prochlorperazine affects vestibulo-ocular reflex (VOR) and vestibulo-perceptual function.
METHODS: We studied 12 healthy naïve subjects 3 h after a single dose of oral prochlorperazine 5 mg in a randomised, placebo-controlled, double-blind, crossover study in healthy young subjects. Two rotational tests in yaw were used: (1) a threshold task investigating perceptual motion detection and nystagmic thresholds (acceleration steps of 0.5°/s(2)) and (2) suprathreshold responses to velocity steps of 90°/s in which vestibulo-ocular and vestibuloperc

Background: Cochlear implantation (CI) helps patients with severe or profound sensorineural hearing loss (SNHL) restore hearing and speech abilities. However, some patients exhibit abnormal vestibular functions with symptoms such as dizziness or balance disorders, after CI. Whether age at CI and CI approach (unilateral or sequential bilateral) affect vestibular functions in users with cochlear implants remains unclear. Objectives: To investigate the vestibular functions in children and adults before and after unilateral or sequential bilateral CI. Materials and Methods: Thirty-seven patients w

The vestibulocochlear nerve is a sensory nerve that serves the organs of hearing and equilibrium. Neuropathies of the nerve, particularly auditory neuropathy may be caused by primary demyelination or by axonal diseases. In disorders affecting the cochlear nerve, it is probable that the vestibular nerve is involved as well. There are isolated reports of the involvement of the inferior vestibular nerve (using vestibular-evoked myogenic potentials) in individuals with AN. However, there is a dearth of information on the involvement of the superior vestibular nerve and other functions such as opto

HYPOTHESIS: A vasopressin-induced endoymphatic hydrops model can represent an acute vertiginous attack in Menière's disease (MD).
BACKGROUND: Previous animal models are not appropriate to evaluate the efficacy of new treatments for hydrops because they cannot represent an acute attack of MD. Recently, a new dynamic model was introduced for acute hydrops exacerbation using the vasopressin type 2 receptor agonist, desmopressin (1-deamino-8-D-Arginine vasopressin, VP); however, resulting changes in vestibular function have not been investigated.
METHODS: A total of 37 guinea pigs were used. Two

CONCLUSIONS: The findings demonstrate that an enduring unilateral utricular dysfunction, possibly together with canal hypofunction, can occur after labyrinthine disease or injury. They also suggest that unilateral, isolated utricular dysfunction - or utricle paresis - can occur, representing a novel entity in the differential diagnosis of peripheral vestibular function. The occurrence of subjective visual vertical (SVV) asymmetry in the presence of symmetric vestibular evoked myogenic potentials (VEMPs) also confirms that the information from the utricles, rather than the saccules, dominates S

BACKGROUND: Do central mechanisms account for the variability of clinical recovery following peripheral vestibulo-cochlear lesions?
OBJECTIVE: To investigate structural (morphological) plasticity in the human brain following unilateral vestibulo-cochlear lesions which might contribute to central vestibular compensation.
METHODS: The authors compared regional grey matter volume (GMV) changes in patients after surgical removal of unilateral acoustic neuroma with age-matched control subjects, and hypothesised morphometric changes in the vestibular and auditory cortices which may be related to f

PURPOSE: We aimed to assess the ability of a head-shaking test (HST) to reflect vestibular compensation in patients after unilateral peripheral vestibular loss and to provide missing evidence and new insights into the features of head-shaking-induced nystagmus (HSN) over a 2-year follow-up.
BACKGROUND: HSN may occur after a prolonged sinusoidal oscillation of the head. HSN is frequently observed in subjects with vestibular function asymmetry; it usually beats toward the functionally intact or "stronger" ear and can be followed by a reversal of its direction.
STUDY DESIGN: A prospective obser
