Gray-Scale Inversion on High Resolution Computed Tomography of the Temporal Bone: An Observational Study.

OBJECTIVES: This is a qualitative study to explore the utility of gray-scale inversion or the "invert" function of high-resolution computed tomography (HRCT) scans in the diagnosis of temporal bone anatomy and pathology. METHODS: This is a case series describing an innovative application of an existing image processing tool to visualize temporal bone anatomy and pathology. Illustrative patients at a tertiary referral center with otologic symptoms and findings leading to HRCT scans of the temporal bone were included. Diagnostic HRCT scans were evaluated utilizing the gray-scale inversion funct

Perilymphatic fistula and pneumolabyrinth without temporal bone fracture: a rare entity.

Post-traumatic pneumolabyrinth is an uncommon clinical entity, particularly in the absence of temporal bone fracture. We report the case of a patient who presented to our emergency department with a headache, sudden left hearing loss and severe dizziness which began after a traumatic brain injury 3 days earlier. On examination, the patient presented signs of left vestibulopathy, left sensorineural hearing loss and positive fistula test, normal otoscopy and without focal neurological signs. The audiometry confirmed profound left sensorineural hearing loss. Cranial CT revealed a right occipital

Gap junctional connections between hair cells, supporting cells and nerves in a vestibular organ.

The pattern of gap-junctional connections between cells in the vestibular neuroepithelium of the posterior semicircular duct of the alligator lizard are described based upon the study of freeze fracture replicas and ultrathin sections with a transmission electron microscope. Both type I and type II hair cells are coupled to adjacent supporting cells by a series of small macular gap junctions located in a ring around the hair cell at the level of the apical circumferential belt of actin filaments. Adjacent supporting cells are extensively interconnected by gap junctions. A few cases of gap junc

Inner ear damage due to lipoid nephrosis.

Inner ear pathology was studied in adult rats with lipoid nephrosis induced by puromycin aminonucleoside. Although no abnormality was observed in auditory brain-stem responses, significant changes were noted in the stria vascularis. The most striking observation was that intermediate cells were markedly swelled, there-by pressing adjacent marginal cells. Severely affected marginal cells have vacuoles and increased lysosomes and protruded toward the endolymphatic space. The organ of Corti remained virtually intact. Although the vestibular maculae were relatively normal, type I hair cells in the

Hereditary vestibulo-cochlear dysfunction and vascular disorders.

A family is described with a progressive autosomal dominant vestibulocochlear dysfunction resulting in a Dandy syndrome, head movement dependent oscillopsia and hearing loss. The history was negative for other neurological or otological diseases (including infectious diseases) or use of neuro-ototoxic drugs, except for a high incidence of vascular disorders (hypertension, stroke, and heart infarction).

Medical problems of space flight.

Several consistent medical problems have been encountered by astronauts during space flights. These include vestibular dysfunction, weight loss, increase in height, upward fluid shift, anemia, cardiovascular deconditioning, muscle atrophy, and bone loss. Almost all of these alterations can be attributed to the absence of gravitational force. Most are adaptive in nature and therefore reversible, but readaptation after returning to earth may cause further problems (e.g., in the case of vestibular dysfunction). The most recalcitrant and disturbing of all these problems is the relentless bone loss

Cogan's syndrome: acute vestibular and auditory dysfunction with interstitial keratitis.

Two patients with Cogan's syndrome had acute bilateral hearing loss and vestibular dysfunction. Interstitial keratitis was discovered by slit-lamp examination. Both cases responded to corticosteroid treatment. Immediate recognition of this syndrome and the institution of high-dose corticosteroid therapy should lead to resolution of the hearing loss in patients with Cogan's syndrome.

The effects of the "vestibular sedative" drug, Flunarizine upon the vestibular and oculomotor systems.

The effects of the "vestibular sedative" drug Flunarizine upon the oculomotor functions of pursuit and voluntary saccades and upon the vestibular response (to rotational stimuli) were assessed in twenty volunteer subjects. The study was then extended to three patients with chronic imbalance of central origin who had reported a beneficial symptomatic response to the drug. Three of the volunteer subjects were found to have a directional preponderance (presumed to arise from peripheral dysfunction). In the remaining seventeen normal subjects Flunarizine was found to reduce the amplitudes of fast

[Lipoproteins in cochleovestibular disorders (author's transl)].

Forty-nine patients with neuro-otologic symptoms were examined with regard to their risk factors, especially their lipoproteins. No essential differences were found in the serum triglyceride and serum cholesterol levels. LDL-cholesterol and the quotient LDL-cholesterol/HDL-cholesterol were significantly higher in the group of patients. We consider this an indication for a relation between arteriopathy and cochleovestibular disorders.

Stapes mobilization for otosclerotic deafness; the monitored peribasal technique.

Direct surgical operation on the stapes footplate region in the treatment of hearing loss due to otosclerosis has been revived. In the last 100 cases of a total series of 600, the peribasal stapes mobilization (stapedolysis) technique was completely successful in 62 per cent, partially successful in 29 per cent and a failure in 9 per cent. Stapes mobilization requires precise microsurgical technique monitored by audiometry during the surgical procedure. It appears that mobilization is the preferred approach to the surgical treatment of otosclerosis, but fenestration of the vestibular labyrinth

Surgical treatment of vertigo by utriculostomy: an experimental study in sheep.

The objective of this study was to develop and test an endolabyrinthic microsurgical procedure for treatment of vertigo, the utriculostomy. This involves the application of local heat for obtaining a fistula in the membranous labyrinth, so as to establish communication between the endolymphatic and perilymphatic spaces at the utricle level. Before the procedure, an experimental model using quail eggs was built for pre-evaluation, and macroscopic and histological studies were performed in the temporal bones of three healthy sheep. Following this, the utriculostomy was performed through the oval

Endolymphatic hydrops in guinea pigs after cauterizing the sac with silver nitrate.

There have been several reports of perisaccular fibrosis of the endolymphatic sac in some autopsied cases of Meniere's disease. We attempted to initiate perisaccular fibrosis of the sac in guinea pigs by injecting a small amount of 10% silver nitrate solution that cauterizes the tissue readily to produce scar, ie, fibrosis. Ninety-nine guinea pigs out of 108 (92%) that were treated with silver nitrate showed the endolymphatic hydrops in varied extent, from slight to profound. The extent of hydrops might depend on the extent of destruction of the sac following the cauterization. The lumen of th

[Comparative study in acute unilateral vestibular disorders].

The results of follow-up examinations on 52 patients suffering from acute unilateral vestibular paralysis are compared with the data of 295 patients with the same illness, compiled from 10 publications, and the successes of the treatment methods are compared. Function was restored, independent of the age of the patient, intensity of the symptoms at the onset of the illness and the extent of the functional damage. Concomitant diseases and so-called high-risk factors had no influence on the prognosis. The infusion-therapy carried out with rheologic or vasoactive drugs showed no better result tha

[Fluctuating vestibular deficit--a new Ménière disease related internal ear disorder].

A third inner ear disorder similar to Menière's disease and fluctuant hearing loss seems to be due to endolymphatic hydrops. This disorder is characterised by attacks of vertigo without any cochlear symptoms. During the course of the disease, which lasts up to 8 years, the vestibular organ loses its function in most cases. Clinical and electronystagmographic findings are presented, and the possible pathophysiological mechanisms are discussed.

Diagnostic accuracy of rotation testing vs. standard vestibular test battery--a long-term study.

Interpretation of vestibular test results derived from rotational and other electronystagmography studies often give confusing diagnostic and prognostic information. We have reviewed 90 consecutive patients seen in the Vestibular Clinic at the University of Texas Medical Branch comparing the accuracy of the damped torsion swing test (DTS) with a conventional vestibular test battery (VTB) to predict vestibular function abnormality (peripheral or central). These predictions were compared with vestibular diagnoses obtained by long-term follow-up (24-36 months). VTB and DTS results were evaluated

[Feasibility of high resolution CT in the demonstration of the vestibular aqueduct (Ménière) and otospongiosis of the labyrinth].

Ever since the introduction of temporal bone imaging by means of high-resolution CT, it appears that the combination of high spatial resolution, high density resolution and the freedom of patient positioning for scanning of optimal otological planes may play a unique role in the diagnosis and follow-up of a number of otological disorders. Two examples are described here. The first is the possibility of determining whether the vestibular aqueduct in idiopathic Ménière's disease is obliterated or not, and if so, whether it is a bony or a fibrous obliteration. Although the results are preliminary
