[Endogenous endophtalmitis in a patient withStaphylococcus aureusbacteraemia].

BACKGROUND: Staphylococcus aureas bacteraemia can spread to cause endocartitis, spondylodiscitis or infection of vascular or joint prostheses. Endogenous endophtalmitis is a serious complication which may result in permanent visual loss of the affected eye. Treatment consists of intra-ocular injection of antibiotics or a vitrectomy. CASE DESCRIPTION: A 79-year-old man attended the emergency department because of septic arthritis. Blood cultures were positive for a S. aureus bacteraemia. On the second day of admission he developed a blurred vision and he was diagnosed with an endophtalmitis. T

Retinal racemose hemangioma with retinal artery macroaneurysm: Optical coherence tomography angiography (OCTA) findings.

PURPOSE: To describe a rare case of retinal racemose hemangioma (RRH) with retinal artery macroaneurysm (RAM) and its optical coherence tomography angiography (OCT-A) finding before and after treatment. OBSERVATIONS: Congenital arteriovenous (AV) communications or RRH is a rare developmental anomaly associated with various ocular conditions. RRH alone is usually asymptomatic, and vision loss occurs when associated with other ocular complications like retinal vein occlusion, hemmorage, macular edema and macroaneurysm. In this communication we describe a case of a 44-year-old female having RRH

[Ocular syphilis: a warning sign of an HIV infection].

A 29-year-old male presented at the ER with a red and painful left eye with sudden and progressive loss of vision within a week. His visual acuity was 1/60. Slit lamp investigation showed inflammatory cells throughout the anterior chamber and vitreous body, and a hyperaemic and swollen optic disc with retinal lesions, indicating a panuveitis combined with multifocal chorioretinitis. Due to the progressive nature, an infectious cause was suspected: positive syphilis serology and HIV serology confirmed this. The diagnosis of ocular syphilis was a first manifestation of a syphilis infection. More

A Prospective Analysis of Lesion-Symptom Relationships in Acute Vestibular and Ocular Motor Stroke.

Background: Diagnosing stroke as a cause of acute vertigo, dizziness, or double vision remains a challenge, because symptom characteristics can be variable. The purpose of this study was to prospectively investigate lesion-symptom relationships in patients with acute vestibular or ocular motor stroke. Methods: Three hundred and fifty one patients with acute and isolated vestibular or ocular motor symptoms of unclear etiology were enrolled in the EMVERT lesion trial. Symptom quality was assessed by the chief complaint (vertigo, dizziness, double vision), symptom intensity by the visual analog s

Potential therapeutic applications of stem cells in animal models of ocular affections.

BACKGROUND: Ocular affections are serious damage to the ocular tissue that results in impaired vision or blindness. Cell-based therapies are a potentially effective therapeutic technique that entails using stem-like precursor cells to induce differentiation of specific cell types and implanting the cells to improve vision in the affected tissue area. METHODS: Numerous clinical trials were started to investigate the potential benefits of stem cells for treating ocular affections, based on several encouraging findings from the preclinical research. Following our review, data were collected from

Bilateral Exudative Chorioretinopathy in All-trans Retinoic Acid-Associated Differentiation Syndrome.

This report presents a rare case of bilateral exudative retinal detachment (ERD) as an early manifestation of differentiation syndrome (DS) in a woman in her early 30s undergoing induction therapy for acute promyelocytic leukemia (APL) with all-trans retinoic acid (ATRA) and arsenic trioxide. Nineteen days into therapy, she presented with bilateral blurring of vision. Ocular examination revealed bilateral multiple small closely spaced ERDs. Fundus autofluorescence and optical coherence tomography revealed focal retinal pigment epithelial disruptions. No systemic features of DS were recognised

Eye injury: prevalence and prognosis by setting.

Although an estimated 1 million Americans suffer ocular injuries each year, the setting of injury and its prognostic implications have not been closely examined. Using data compiled by the Eye Injury Registry of Alabama (EIRA) from 514 cases of serious eye injury, we examined the demographics and prognosis of ocular injury by setting of injury. Work-related injuries accounted for only 28% of total injuries, and injuries occurring at home accounted for 27%, followed by situations related to recreation (25%), assault (11%), travel (5%), and "other" (school, unknown, etc) (4%). The poorest initia

Efferent Vision Therapy.

BACKGROUND: Traditional orthoptic therapy used by ophthalmologists, orthoptists, and optometrists is directed at improving visual acuity, ocular alignment, or both. For example, convergence exercises are used to treat convergence insufficiency (CI). However, other forms of "vision therapy" are directed at improving "visual processing and efficiency." The therapeutic regimen often entails repetitive ocular motor tasks performed during multiple office visits with a behavioral optometrist. These ocular motor tasks are used to treat diverse conditions such as learning disabilities, poor reading ab

Correlation Between Refractive Asymmetry and Eye Dominance According to the Sensory and Fixation Method.

PURPOSE: The eye dominance distribution in humans varies according to the assessment method. Although the correlation between refractive asymmetry and eye dominance has long been investigated, the results are inconsistent. Here, we analyzed the relationships between refractive asymmetry and eye dominance measured by fixation and sensory methods and assessed the concordance of the two methods. METHODS: One hundred and twelve subjects were enrolled to evaluate their refractive asymmetry and eye dominance. Refractive error was determined through objective and subjective refraction. The assessmen

Creating correct blur and its effect on accommodation.

Blur occurs naturally when the eye is focused at one distance and an object is presented at another distance. Computer-graphics engineers and vision scientists often wish to create display images that reproduce such depth-dependent blur, but their methods are incorrect for that purpose. They take into account the scene geometry, pupil size, and focal distances, but do not properly take into account the optical aberrations of the human eye. We developed a method that, by incorporating the viewer's optics, yields displayed images that produce retinal images close to the ones that occur in natura

Relationship between clinical signs and symptoms of convergence insufficiency.

PURPOSE: The percentage of children who are symptomatic has been shown to increase with the number of signs of convergence insufficiency (CI). Our goal was to investigate whether there is a relationship between the severity of the clinical signs of CI and symptom level reported in children with a three-sign symptomatic CI. METHODS: The Convergence Insufficiency Treatment Trial enrolled 221 children with symptomatic CI from ages 9 to 17 years. Inclusion criteria included the following three signs of CI: (1) exophoria at near at least 4Δ greater than at distance, (2) insufficient positive fusio

Emmetropisation in normal and strabismic children and the associated changes of anisometropia.

The purpose of this study was to measure whether emmetropisation failed in children who had strabismus irrespective of their refraction in infancy, and to record simultaneous changes in anisometropia. We also report how often hypermetropia increased before these children presented with esotropia. A total of 2920 infants had a cycloplegic retinoscopy at age 5-7 months and again at 42 months or when defective vision was identified. Changes of refraction in 210 children with strabismus are compared with the remaining 2710 who did not. When the spherical equivalent of the fixing eyes was > +2.75 D

[The accommodation-convergence bar (A=C bar)].

The accommodation-convergence bar was developed as a simple measuring and exercise instrument. For the ophthalmologist, it is a convenient means of measuring accommodation and convergence in the office, while patients can use it for home convergence and fixation exercises which they can monitor themselves. The A = C bar has also been used experimentally to eliminate the functional central scotoma in dyslexic children.

Novel Application of Point-of-Care Ocular Ultrasound in a Left Central Retinal Artery Occlusion.

Central retinal artery occlusion represents a vision-threatening entity in those presenting with monocular painless vision loss, especially in the elderly and those with cardiovascular comorbidities. While confirmation of this diagnosis requires consultation with an ophthalmologist, prompt recognition is the crucial action of the emergency physician to help reverse retinal ischemia and save vision. Here we describe the case of a central retinal artery occlusion identified on point-of-care ocular ultrasound and confirmed by fluorescein angiography.
