Enveloping the bladder with displacement of flap of the rectus abdominis muscle for the treatment of neurogenic bladder.

Neurogenic bladder is a frequent occurrence. A new surgical technique has been designed and was used successfully in 18 patients with ideal results. The key point of the operation is to turn over a flap of rectus abdominis muscle to envelop the bladder. Contraction of the muscle flap and the abdominal muscles would enhance voiding ability. In the meantime, the bladder is displaced forward and the resultant change in the bladder-posterior urethral angle also favors voiding. Since the bladder is near the anterior abdominal wall manual compression during voiding is made easier. All of these advan…

Upper gastrointestinal bleeding in a quadriplegic patient.

This paper presents an unusual case of gastrointestinal (GI) bleeding which followed a period of nausea and vomiting in a quadriplegic patient in whom endoscopy revealed a tear in the distal esophagus. Similar lesions have been described as the Mallory-Weiss syndrome. The physiology of vomiting is presented, wherein abdominal muscle contraction plays a partial role. The pathophysiology in the patient who lacks functional abdominal muscles is considered in relation to etiology. It is suggested that mucosal tears similar to those associated with a Mallory-Weiss syndrome be added to the different…

An unusual case of rapidly progressive contractures: Case report and brief review.

An 8-year-old boy, diagnosed as cervical dystonia, was referred to our tertiary center. After a trivial trauma he had developed painful lumps in the axial region, which was followed by restricted movements of neck, shoulder, and abdominal muscles over 4 months. He had kyphoscoliosis, torticollis, rigid abdomen, and multiple muscle contractures. He also had short great toes. A detailed skeletal survey showed calcification in the soft tissues surrounding the shoulder anterior chest wall, thorax, and paraspinal muscles; there was also beaking of vertebrae, which was confirmed by CT thorax. This r…

Mapping of neural pathways that influence diaphragm activity and project to the lumbar spinal cord in cats.

During breathing, the diaphragm and abdominal muscles contract out of phase. However, during other behaviors (including vomiting, postural adjustments, and locomotion) simultaneous contractions are required of the diaphragm and other muscle groups including abdominal muscles. Recent studies in cats using transneuronal tracing techniques showed that in addition to neurons in the respiratory groups, cells in the inferior and lateral vestibular nuclei (VN) and medial pontomedullary reticular formation (MRF) influence diaphragm activity. The goal of the present study was to determine whether neuro…

Clinical and ultrasound evaluation of donor site morbidity after tram-flap for breast reconstruction.

The TRAM-flap has become a well-established method for breast reconstruction. Even though the aesthetic result is superior to implant reconstruction, a main disadvantage is the potential risk to create weakness of the abdominal wall. For evaluation of abdominal wall function, an imaging method has to be used which is able to prove functional properties of the remaining muscle. This study was undertaken in order to verify if ultrasound imaging is a reasonable method to examine muscle movements after TRAM-flap procedures in addition to clinical examination. In 8 patients, a DIEP-flap, in 11 pati

Anatomical Variations of the Iliopsoas Muscle.

Anatomical variations of the iliopsoas muscle are relatively uncommon but are clinically significant because of the muscle's relation to the femoral nerve (FN). Variants of the iliopsoas include accessory iliacus, iliacus minimus, psoas tertius, and psoas quartus muscles. This study describes anatomical findings of these variants. Dissections of the posterior abdominal wall in 95 Japanese cadavers at Kurume University School of Medicine (2015-2017) identified three cases of iliopsoas muscle variants. In the first case, a small variant muscle on the right side of a 91-year-old female cadaver or

Latent profile analysis of pelvic floor muscle pain in patients with chronic pelvic pain.

AIM: Chronic pelvic pain (CPP) is a syndrome of related diagnoses including pain originating from the muscles of the pelvic floor. The objective of this study was to evaluate which muscles are important to examine, in what manner pelvic floor muscle pain contributes to patients' pain experience, or what thresholds should be applied to identify significant pelvic floor muscle pain by comparing exam findings with outcome measures METHODS: A total of 428 patients meeting the definition for CPP were evaluated using a standardized physical examination of the abdominal wall, pelvic floor, and vesti

The efficacy of Pilates-based exercises on rectus abdominis engagement in comparison with the FITNESSGRAM in Collegiate dancers.

Engagement of the core abdominal muscles is essential to maintain proper alignment, technique, and safety. This study compared the muscular amplitude of the upper and lower rectus abdominis muscles' EMG during six Pilates-based abdominal exercises with the FITNESSGRAM in order to determine which may be more effective for dancers. A sphygmomanometer is used as a training tool for neutral pelvis stability; therefore this study also investigated the muscle activation of the Pilates-based exercises with and without the use of a sphygmomanometer to determine which would be more effective in muscula…

Trunk muscle involvement in late-onset Pompe disease: study of thirty patients.

Late-onset Pompe disease is characterized by progressive weakness involving proximal limb and respiratory muscles. Recently, treatment with enzyme replacement therapy (ERT) has been introduced partially improving patients' prognosis, but a standard consensus on when to start ERT is still lacking. There is also a lack of biomarkers related to the clinical progression of the disease. Here we used muscle magnetic resonance imaging (MRI) or computed tomography (CT) to study the abdominal and paravertebral muscles of 30 late-onset Pompe patients at different stages of disease. We observed a selecti

Effects of postural changes and vestibular lesions on diaphragm and rectus abdominis activity in awake cats.

Changes in posture can affect the resting length of the diaphragm, requiring alterations in the activity of both the abdominal muscles and the diaphragm to maintain stable ventilation. To determine the role of the vestibular system in regulating respiratory muscle discharges during postural changes, spontaneous diaphragm and rectus abdominis activity and modulation of the firing of these muscles during nose-up and ear-down tilt were compared before and after removal of labyrinthine inputs in awake cats. In vestibular-intact animals, nose-up and ear-down tilts from the prone position altered re…

Assessment of bioelectrical activity of synergistic muscles during pelvic floor muscles activation in postmenopausal women with and without stress urinary incontinence: a preliminary observational study.

OBJECTIVE: Muscles such as adductor magnus (AM), gluteus maximus (GM), rectus abdominis (RA), and abdominal external and internal oblique muscles are considered to play an important role in the treatment of stress urinary incontinence (SUI), and the relationship between contraction of these muscles and pelvic floor muscles (PFM) has been established in previous studies. Synergistic muscle activation intensifies a woman's ability to contract the PFM. In some cases, even for continent women, it is not possible to fully contract their PFM without involving the synergistic muscles. The primary aim

Heterotopic Ossification of the Abdomen: A Rare Sequela Following Trauma and Damage Control Laparotomy.

Heterotopic ossification (HO) of the abdomen is a rare yet highly morbid complication following blunt and penetrating trauma requiring damage control laparotomy. We present the case of a 22-year-old man, 20 months after life-threatening motor vehicle crash with major vascular injury requiring multiple abdominal surgeries. The patient was initially treated at a community hospital and subsequently developed a chronic left lower quadrant enterocutaneous fistula, accompanied by a gradually worsening diffuse abdominal pain. He was referred to our tertiary care center with extensive skin breakdown a

Muscle involvement in a patient with Henoch-Schönlein purpura nephritis.

Although Henoch-Schönlein purpura (HSP) is a small-vessel vasculitis affecting multiple organ systems, muscle involvement has rarely been reported. This report describes muscle pain and weakness in a patient with HSP nephritis (HSPN). A 13-year-old boy suffered from purpura, abdominal pain, and symmetrical muscle pain and weakness of the extremities. He was diagnosed as having HSP with muscle involvement. His abdominal pain and muscle involvement improved 1 week after commencing oral prednisolone therapy. The patient subsequently developed biopsy proven HSPN. It should be noted that, although

Muscle quality correlates with hearing thresholds: a cross-sectional analysis.

INTRODUCTION: Muscle health, including muscle volume, is an independent risk factor for physical disability and metabolic disorders. Although low muscle mass has been associated with hearing loss, the association between computed tomography (CT)-derived muscle quality and hearing has not been explored. METHODS: This cross-sectional study examined associations between abdominal body composition and hearing thresholds in 7,774 adults (4,537 men and 3,237 women) aged ≥40 years undergoing routine health check-ups. Abdominopelvic CT and pure-tone audiometry were performed, and total abdominal musc

Severe Irreversible Diastasis Recti Abdominis and Abdominal Hernia in Postpartum Women: Rare Case Report.

Diastasis recti abdominis is a condition defined as the separation between the rectus abdominis and the linea alba, which leads to weakness in the abdominal muscles. Diastasis may be slight or severe, sometimes resulting in herniation of the abdominal viscera. Following childbirth, most women develop some extent of muscle separation in the postpartum period. However, if the diastasis recti abdominis in the postpartum period remain severe, it should be corrected to prevent concurrent abdominal hernia, strangulation or incarceration. We herein present two Korean cases of postpartum women with se…

Abdominoplasty of cloacal exstrophy: application of reconstructive methods to recurrent and primary cases.

BACKGROUND: Surgical repair of cloacal exstrophy is still challenging. At birth, patients undergo bladder closure, colostomy, and osteotomy of the pubic bone (if necessary, when the interpubic range is wide and cannot be brought together with the hands). This abdominal defect is closed primarily by urologists or pediatric surgeons, if possible, but the patient may experience a relapse of bladder exstrophy and an abdominal defect. Abdominal reconstruction was performed for a series of recurrent and primary (preventive) cases. METHODS: Abdominal wall reconstruction was performed using the rectu
