Factors affecting bone mineral density in renal transplant patients.

Bone disease is a major cause of morbidity in end stage renal failure. This study is aimed to assess the prevalence of abnormal bone mineral density (BMD), measured by dual photon absorptiometry (DPA) in the renal transplant population. Subjects consisted of 110 patients followed up after transplantation for between 1 and 17 years. Variables analyzed included age, sex, ethic origin, years and type of dialysis prior to transplantation, date of transplant, total steroid dose, number of rejection episodes, use of Cyclosporin, and biochemical/hormonal variables such as serum calcium, phosphate, ma

Reduced bone mineral content in adult patients with growth hormone deficiency.

Bone mineral content was measured in a follow-up study of adult patients with hypopituitarism and growth hormone deficiency. There were 95 patients (59 males, mean age 54.0 years, range 21-74 years; 36 females, mean age 53.5 years, range 31-73 years). Routine replacement therapy with cortisone acetate and L-thyroxine was given. All males that were gonadal deficient were on proper testosterone therapy, except in four patients who were treated separately. Bone mineral content (g/cm) was measured using dual-photon absorptiometry in the third lumbar vertebra. Bone mineral content in the patients w

Longitudinal study of bone mass in end-stage renal disease patients: effects of parathyroidectomy for renal osteodystrophy.

The effectiveness of parathyroidectomy (PTHX) for the control of secondary hyperparathyroidism was assessed in 46 adult end-stage renal disease (ESRD) patients whose bone mineral content at the midshaft and distal radius was measured using single-photon absorptiometry (SPA) every 6 months before and after the surgery. They were compared to 46 age-, race-, and sex-matched ESRD patient controls who had not undergone surgery but who had had at least five SPA studies at similar intervals. Presurgery midradius bone mass was significantly lower for PTHX patients compared to controls. Comparing chang

Bone mineral density in Turkish postmenopausal women.

OBJECTIVE: To determine the bone mineral density curve in Turkish postmenopausal women and to evaluate the efficacy of estrogen replacement therapy in the prevention of bone loss. METHOD: Spinal bone mineral density was measured using dual photon absorptiometry in 118 postmenopausal women, prospectively. Parity, previous and current use of estrogen preparations were correlated with BMD using Student's t-test and the chi 2-test. RESULTS: With a mean BMD value of 0.967 +/- 0.013 g/cm2, 62.5% of our postmenopausal study population had a mean BMD value of < 1.0 g/cm2 and 33.7% were already osteo

Long-term adjuvant tamoxifen in early breast cancer: effect on bone mineral density in postmenopausal women.

The decrease in sex steroid hormone levels after the onset of menopause is associated with bone loss and subsequent osteoporosis. Tamoxifen has antiestrogenic properties and may thus theoretically decrease bone mineral density, particularly after long-term treatment. Bone mineral density (BMD) was assessed in 75 recurrence-free postmenopausal breast cancer patients included in a randomized trial of adjuvant tamoxifen (40 mg daily) for 2 or 5 years versus no adjuvant endocrine therapy. The measurements were done about 7 years after the initial randomization. BMD was measured with single-photon

Calcium balance in uremia: longitudinal study of long-term survivors.

Twelve male patients surviving uremia of 9-15 years duration were studied sequentially over a mean period of 9.2 years. Radiographs, bone biopsies, bone mineral mass (BMM) measured by photon absorptiometry, and total body calcium (TBC) determined by in vivo neutron activation analysis (NAA) were used. Pathologic fractures correlated with TBC corrected for height: 4 of 5 patients without fractures had TBC above 80% predicted, whereas 5 of 7 patients with fractures fell below this value. Unlike previous studies, negative calcium balance was seen in 3 patients with prolonged hyperparathyroidism.

Physical exercise as prophylaxis against involutional vertebral bone loss: a controlled trial.

1. The skeletal effects of physical training were studied in a controlled trial involving 31 healthy women (aged 50-73 years) with previous Colles' fracture of the forearm. The bone mineral content of the lumbar spine and both distal forearms was measured by dual-photon (153Gd) absorptiometry. 2. The participants were allocated to either a physical exercise group or a control group. The former group followed a standardized exercise programme, exercising for 1 h twice weekly during 8 months. 3. Twenty-seven women completed the study. Lumbar spine bone mineral content of the exercise group incre

Assessment of bone turnover in postmenopausal osteoporosis by measurement of serum bone Gla-protein.

Controversy persists regarding the abnormality of bone turnover responsible for bone loss in women with postmenopausal osteoporosis. To evaluate this, we measured serum bone Gla-protein (BGP), a specific marker for bone turnover, in 62 untreated patients with postmenopausal osteoporosis. Results were compared with those in 142 normal women and were expressed as standard deviations from the age-adjusted predicted mean (Z score). Serum BGP was increased (+0.48 S.D., p = 0.002) in the osteoporotic patients; 9.7% of patients were greater than 2 S.D. above but none were greater than 2 S.D. below th

Reduction of bone density: an effect of gonadotropin releasing hormone analogue treatment in central precocious puberty.

Gonadal steroids drive the significant bone mineral increase that occurs at puberty. Oestrogen deprivation in women results in bone loss. We investigated bone mineralization by single photon absorptiometry in girls with central precocious puberty (n = 13, age 3.8-8.5 years) before and during 1 year of treatment with gonadotropin releasing hormone analogue (GnRH-a = long-acting D-Trp6-GnRH, 60 micrograms i.m. every 28 days). Before GnRH-a therapy, bone mineral density (BMD) was significantly higher in patients than in ten control girls matched for chronological age (patients 0.575 +/- 0.097 g/c

Longitudinal changes in lumbar bone density among thyrotoxic patients after attainment of euthyroidism.

Hyperthyroidism increases bone turnover, which, in turn, may lead to bone loss from the spine and hip. Treatment of thyrotoxicosis may restore bone mass, but there are few long term prospective studies. We examined lumbar bone mineral density (BMD) in 21 subjects [11 with hyperthyroidism (HT) and 10 controls of similar age] by dual photon absorptiometry in 1986 and by dual energy x-ray densitometry in 1991. All 11 HT patients were successfully treated and remained euthyroid (mean TSH, 2.25 +/- 0.80 mU/L) for more than 3 yr. Lumbar BMD increased 11.03 +/- 2.38% (P < 0.001) in HT patients, but o

Vitamin D, parathyroid hormone, and bone mineral content of lumbar spine and femur in primary biliary cirrhosis.

In order to elucidate the pathogenesis and degree of osteopoenia in primary biliary cirrhosis (PBC) we conducted a cross-sectional study of 47 non-selected female patients with biopsy-proven PBC. Bone mineral content (BMC) of the lumbar spine, femoral neck and femoral shaft was determined using dual photon absorptiometry. Compared to healthy females of corresponding decades the PBC patients exhibited significantly decreased mean BMC-values in lumbar spine (88%, P less than 0.05) and femoral neck (92%, P less than 0.05) but not in femoral shaft (96%, NS). Bone mineral content was not significan

Vertebral bone density in non-amenorrhoeic hyperprolactinaemic women.

Recently, a decrease in bone mineral content (BMC) in hyperprolactinaemic women with long-lasting amenorrhoea has been reported, and attributed either to a direct effect of PRL on bone or secondary to the oestrogen deficiency. To verify if PRL by itself has a direct effect on bone, we have studied BMC at the lumbar level by double-photon absorptiometry in 22 patients with hyperprolactinaemia, selected on the basis of normal or near-normal oestradiol levels. The results were compared with those obtained in 28 healthy closely-matched women, and seven hyperprolactinaemic patients with long-lastin

Skeletal remodelling and bone related hormones in two adults with increased bone mass.

This study of two cases documents increased bone mass in a 30-yr old man and osteopetrosis in a 38-yr-old woman and examines the patients in terms of radiologic features, bone photon absorptiometry and bone biopsy analyses which include tetracycline kinetics. Both patients had elevated bone mass based on quantitative bone histology of iliac crest biopsies normal, or low PTH, and normal calcitonin values. However, they differed markedly from each other in other respects: One patient possessed significantly increased rates of bone formation and mineralization, had elevated serum alkaline phospha

Application value of FRAX in high-altitude areas in China: a systematic review and meta-analysis.

BACKGROUND: High-altitude environments in China are associated with altered bone metabolism and an increased risk of osteoporotic fractures. Given the limited availability of dual-energy X-ray absorptiometry (DXA) in these regions, the FRAX tool provides a practical alternative for fracture risk assessment. However, the screening performance and diagnostic performance of FRAX in plateau populations remain unclear. This study systematically evaluated the diagnostic accuracy of FRAX-probability of major osteoporotic fracture (PMOF), probability of hip fracture (PHF), and FRAX high-risk threshold

Low bone mineral density and pain impact in adults with sickle cell disease.

Low bone mineral density (BMD) is a prevalent skeletal finding in sickle cell disease (SCD), but its clinical consequences are poorly understood. We hypothesized that low BMD, independent of osteonecrosis (ON), would associate with pain in adults with SCD. In the SCD Bone Pain study, 53 ambulatory adults (64% females; mean age, 38 ± 11 years; 66% hemoglobin [Hb] SS/Sβ0 thalassemia) underwent dual-energy X-ray absorptiometry scans of their lumbar spine, hip, forearm, and whole body. They also completed the Adult Sickle Cell Quality of Life Measurement Information System pain impact questionnair

A Novel Dual-Energy X-Ray Absorptiometry Protocol for the Proximal Humerus Reveals a Fatty Infiltration Dependent Reduction in Bone Mineral Density.

Localized reduction in bone mineral density (BMD) within the proximal humerus may compromise anchor fixation strength and tendon-to-bone healing after rotator cuff repair; however, standardized dual-energy X-ray absorptiometry (DXA) protocols and diagnostic thresholds are lacking. This study aimed to develop a reproducible proximal humerus DXA protocol, evaluate regional BMD differences between affected and contralateral shoulders, and investigate the relationship between fatty infiltration and regional bone loss. A retrospective analysis was performed on 224 patients who underwent unilateral
