HYPOTHESIS: The aim of this study is to investigate how a mastoidectomy surgery affects bone conduction (BC) sound transmission using a whole head finite element model.
BACKGROUND: Air conduction (AC) and BC hearing thresholds are normally used to evaluate the effect of an ear surgery. It is then assumed that the BC hearing thresholds are unaffected by the surgery. Moreover, BC hearing aids are used in cases of unilateral or conductive hearing loss in heads that have undergone a mastoidectomy surgery. Given the invasiveness of the surgery, the BC hearing sensitivity may be altered by the surg

Tinnitus is a debilitating perception of sound in the absence of external auditory stimuli. It may have either a central or a peripheral origin in the cochlea. Experimental studies evidenced that an electrical stimulation of peripheral auditory fibers may alleviate symptoms but the underlying mechanisms are still unknown. In this work, a stochastic neuron model is used, that mimics an auditory fiber affected by tinnitus, to check the effects, in terms of firing reduction, of different kinds of electric stimulations, i.e., continuous wave signals and white Gaussian noise. Results show that both

Previous research has noted that music can improve gait in several pathological conditions, including Parkinson's disease, Huntington's disease and stroke. Current research into auditory-motor interactions and the neural bases of musical rhythm perception has provided important insights for developing potential movement therapies. Specifically, neuroimaging studies show that rhythm perception activates structures within key motor networks, such as premotor and supplementary motor areas, basal ganglia and the cerebellum - many of which are compromised to varying degrees in Parkinson's disease.

This article examines four disorders of auditory processing that can result from selective brain damage (cortical deafness, pure word deafness, auditory agnosia and phonagnosia) in an effort to derive a plausible functional and neuroanatomical model of audition. The article begins by identifying three possible reasons why models of auditory processing have been slower to emerge than models of visual processing: neuroanatomical differences between the visual and auditory systems, terminological confusions relating to auditory processing disorders, and technical factors that have made auditory s

Two experiments were conducted to examine the intelligibility of 72 passages of connected discourse prepared by Cox and McDaniel in their development of the Speech Intelligibility Rating (SIR) test. Intelligibility was assessed with a method-of-adjustment (MOA) procedure in which listeners adjusted the level of a multi-talker babble until they could just understand 50% of a passage; the measure of intelligibility was the signal-to-babble ratio, dB S/B. The objective was to develop a Revised Speech Intelligibility Rating (RSIR) test that would comprise a large number of equivalent passages that

Measurements of psychophysical two-tone suppression in a number of subjects are described. Levels of the stimulus components (suppressee, L1, and suppressor, L2) were the primary experimental variables. In all experiments the pulsation threshold was used with the probe frequency fr fixed at the suppressee frequency f1. In an initial experiment f1 was fixed at 1 kHz. The suppressor frequency f2 ranged from 0.2 to 1.4 kHz. At appropriate levels all subjects showed significant suppression. Suppression was found to decrease to zero as f2 approached f1. The amount of suppression depended on both L1

To study the whole dynamic range of the stapedius reflex the intensity of the soundstimulus with fixed frequency is continuously changed from 70 to 115 dB. Thus we get results concerning the real reflex eleciting intensity level. the gradient caused by continuous intensity change and a possible limit of contraction. To investigate time resolution phenomenons the stimulating signal is pulsated in a second trial. We used this method of stapedius reflex registration with 48 normal ears and a group of patients with sensorineural hearing loss. The results and the diagnostic possibilities of this me

Responding to multiple stimuli of different modalities has been shown to reduce reaction time (RT), yet many different processes can potentially contribute to multisensory response enhancement. To investigate the neural circuits involved in voluntary response initiation, an acoustic stimulus of varying intensities (80, 105, or 120 dB) was presented during a visual RT task to a patient with profound bilateral cortical deafness and an intact auditory brainstem response. Despite being unable to consciously perceive sound, RT was reliably shortened (~100 ms) on trials where the unperceived acousti

The present study investigates effects of current focusing and pulse shape on threshold, dynamic range, spread of excitation and channel interaction in the time domain using cochlear implant stimulation. The study was performed on 20 adult guinea pigs using a 6-channel animal cochlear implant, recording was performed in the auditory midbrain using a multielectrode array. After determining the best frequencies for individual recording contacts with acoustic stimulation, the ear was deafened and a cochlear implant was inserted into the cochlea. The position of the implant was controlled by x-ray…

An active actuator of a middle-ear implant coupled to the round window membrane (RWM), which transmits vibration to the cochlea, has been used to compensate for hearing loss in patients. However, various factors affect the coupling condition between the actuator and the RWM, resulting in coupling leakage. In this study, a coupling impedance model of the human ear and the actuator was used to investigate the effect of inefficient coupling during reverse stimulation. First, the three-port circuit network model of the actuator was coupled with the acoustic impedance model of human ear reverse sou

In the nervous system, synchronization processes play an important role, e.g., in the context of information processing and motor control. However, pathological, excessive synchronization may strongly impair brain function and is a hallmark of several neurological disorders. This focused review addresses the question of how an abnormal neuronal synchronization can specifically be counteracted by invasive and non-invasive brain stimulation as, for instance, by deep brain stimulation for the treatment of Parkinson's disease, or by acoustic stimulation for the treatment of tinnitus. On the exampl…

OBJECTIVE: Although enormous attention has been directed to the localization and preservation of the facial nerve in acoustic neuroma surgery, the nervus intermedius has largely been ignored. In this article, we describe a method for intraoperative electrophysiologic identification of the nervus intermedius.
STUDY DESIGN: Retrospective case review.
SETTING: University hospital (tertiary care center).
PATIENTS: Thirty-three patients who underwent intraoperative facial nerve monitoring for various cerebellopontine angle procedures. Recording electrodes were placed in the orbicularis oculi and

Previous studies have shown that residual acoustic hearing supplements cochlear implant function to improve speech recognition in noise as well as perception of music. The current study had two primary objectives. First, we sought to determine how cochlear implantation and electrical stimulation over a time period of 14 to 21 months influence cochlear structures such as hair cells and spiral ganglion neurons. Second, we sought to investigate whether the structures that provide acoustic hearing also affect the perception of electrical stimulation. We compared psychophysical responses to cochlea

In each of 24 randomly selected outpatient the size of the impedance changes elicited by touching the skin in the ipsilateral cavum conchae with cotton wool were compared with those elicited by electrical stimulation of the same skin area. Pulse trains of 0.5 s duration, consisting of individual squarewave constant current pulses of frequency 200 Hz and duration 1 ms, were used. In all ears a current of 0.5-1 mA produced impedance change of about the same size as the tactile stimulus. Therefore, in testing the middle ear function observation of impedance changes elicited by tactile as well as

Neuromodulation as a treatment strategy for psychiatric and neurological diseases has grown in popularity in recent years, with the approval of repetitive transcranial magnetic stimulation (rTMS) for the treatment of depression being one such example. These approaches offer new hope in the treatment of diseases that have proven largely intractable to traditional pharmacological approaches. For this reason, neuromodulation is increasingly being explored for the treatment of Alzheimer's disease. However, such approaches have variable, and, in many cases, very limited evidence for safety and effi

The long-term course of the EEG in the Rett syndrome is documented with reference to past studies. The monorhythmic theta activities in the waking state which were characteristically seen in adults with the Rett syndrome were investigated in patients aged between 27 and 28, using auto power spectra. They were often suppressed when the patients were excited or strained. Then the suppression was thought to elevate their conscious levels. The theta activities were often suppressed by calling, but not easily suppressed by other stimuli, especially the somatosensory stimuli of pain and touch. There

Driving the cochlea in reverse via the round window membrane (RWM) is an alternative treatment option for the hearing rehabilitation of a nonfunctional or malformed middle ear. However, cochlear stimulation from the RWM side is not a normal sound transmission pathway. The basilar membrane (BM) motion elicited by mechanical stimulation of the RWM is unknown. In this study, the BM movement at the basal turn was investigated in both reverse via RWM drive and acoustic stimulation in the ear canal or forward drive in postmortem isolated temporal bone preparations of guinea pigs. During reverse driv…
