We evaluated a new IRMA developed commercially for the measurement of corticotropin (ACTH) in human plasma. The assay involves purified poly … TH-producing tumors, hypopituitarism, steroid suppression, surgical adrenalectomy, Nelson syndrome, Addison disease, and corticotropin-releasing hormone stimulation. We conclude that the new IRMA is technically simple to perform and provides a specific and sensitive method for evaluating of adrenocortical function.

Primary adrenal insufficiency (PAI), or Addison's disease, is a rare and potentially life-threatening condition that requires prompt diagnosis and management. While most cases are autoimmune, other causes include infections such as Salmonella, though this is rarely reported. We describe a case of bilateral adrenal abscesses and adrenal insufficiency secondary to disseminated Salmonella Saintpaul infection in a previously healthy 20-year-old male following travel to Thailand. This case highlights the importance of early recognition of adrenal involvement in septic patients with persistent fever

We describe a patient with sicca syndrome, Raynaud's phenomenon and fixed waxy skin lesions. Investigations confirmed both Sjögren's syndrome and sarcoidosis. The patient had previously had ulcerative colitis and other antibody evidence of autoimmunity. The spectrum of disorders reported in association with Sjögren's syndrome and sarcoidosis is wider than suggested in the acronym TASS syndrome (thyroiditis, Addison's disease, Sjögren's syndrome and sarcoidosis), and we suggest that the acronym TOASSUC (thyroiditis, other autoimmunity, Sjögren's syndrome, sarcoidosis, ulcerative colitis) includ

In developing countries tuberculosis is a common cause of constrictive pericarditis and its involvement of other organs may sometimes complicate the surgical management of such patients. A case is reported of constrictive pericarditis in which the patient developed protracted hypotension which was unresponsive to inotropic therapy, following pericardiectomy. On investigation the cause of hypotension was diagnosed as acute adrenocortical insufficiency, secondary to adrenal tuberculosis. The patient was successfully treated with corticosteroids. Low cardiac output after pericardiectomy, which is

Addison's disease secondary to metastatic cancer to the adrenal gland is underdiagnosed. Prompt diagnosis and treatment is essential and could enhance the quality of life. Cases of adrenal insufficiency produced by metastatic carcinoma are unusual, despite the frequency of carcinomatous metastases to the adrenal glands. The clinical features of adrenal insufficiency are relatively nonspecific and can be easily overlooked in a patient with a malignant neoplasm. We report herein the case of a middle-aged man who presented with adrenal insufficiency in association with pancreatic carcinoma. To ou

A case of bilateral adrenal tumoral formation associated with Addison's disease is reported, and the possible cause of this association is discussed. The short-term deleterious course of metastatic malignancies, hemopathies or severe infections when left without specific treatment, and the long-term course of our patient's tumoral pathology excluded these diagnoses. The spontaneous regression of a bilateral pseudotumoral mass was suggestive of 2 causes: haematoma or tuberculosis. Haematoma was unlikely in the absence of causal factors, and this, combined with the slow regression of the adrenal

In two patients with extensive pulmonary tuberculosis who developed hypercalcaemia and hypokalaemia the hypercalcaemia appeared related to the use of small doses of vitamin D, which suggested patients with tuberculosis were hypersensitive to vitamin D. They were thus similar to patients with sarcoidosis, and it is interesting that the Kveim test result was positive in both cases. The hypercalcaemia was quickly suppressed with steroids. Hyperparathyroidism, thyrotoxicosis, Addison's disease, and multiple myeloma were excluded on clinical grounds and by the appropriate tests. The hypokalaemia wa

The dentist is in a strategic position to evaluate early color changes of the head and neck. Certain oral and perioral changes of pigmentation are amenable to early diagnosis. Systemically acquired generalized melanoses and nonmelanotic color changes should be periodically reviewed by all examining physicians and dentists. The multiple forms of acanthosis nigricans are described, and the triad of Albright's syndrome is compared with the findings of Peutz-Jegher's syndrome, multiple basal cell nevi, neurofibromatosis, and Addison's disease. The spectrums of changes seen in abnormal nonmelanotic

We describe two patients with liver kidney microsomal antibody type 1 (LKM1)-positive autoimmune hepatitis (AIH) with associated endocrinopathies. The first patient had insulin-dependent diabetes (IDDM), and the second patient had Addison's disease and hypoparathyroidism, and is also positive for islet cell antibodies, without overt diabetes. To account for the existence of multiple endocrinopathy in these patients, we investigated whether there is sequence similarity between the target of LKM1 antibodies, cytochrome P4502D6 (CYP2D6), and other human proteins, and if so, whether this structura

Type I polyglandular autoimmune syndrome is characterized by the triad of hypoparathyroidism, Addison's disease and chronic mucocutaneous candidiasis. Chronic active hepatitis has been associated with this syndrome but its incidence and severity have not been well documented. We describe a sibship of two patients with type I polyglandular autoimmune syndrome who presented with autoimmune chronic active hepatitis. The first patient presented to us with advanced disease and died despite an emergent liver transplant, while the second patient responded to steroids. Autoimmune chronic active hepati

The authors report 195 cases of endocrine diseases diagnosed in a department of internal medicine at the National Hospital of Niamey (Niger) from January 1982 to December 1986. Diseases of the thyroid gland were largely predominant (70.8%), with simple (34.9%) and nodular (19%) goiters. Hypothyroidism and thyroiditis were not frequent (2 and 2.6%, respectively). The rarity of thyroid cancers (1%) was in relation to the direct hospitalization of these patients in the surgical department. They were not included in this study. In spite of the magnitude of tuberculosis endemicity in Niger, primary

Porphyrias are metabolic disorders of heme biosynthesis, which encompass a broad range of symptoms and signs, neurologic, cutaneous or mixed. Because of lack of specificity and polymorphous clinical picture, porphyrias can mimic either neuropsychiatric, dermatologic, or gastrointestinal diseases. We present the case of a 58 years old man to whom clinical presentation suspicious of Addison's disease (melanoderma, fatigue, weight loss, intermittent abdominal pain) was the disguise of porphyria cutanea tarda. A general background of porphyrias and differential diagnosis with other forms of hepati

INTRODUCTION: In sera of pituitary disease patients and other autoimmune endocrine disease are detectable pituitary autoantibodies. Until now characterization of pituitary antigen is still unknown. The aim of our study was isolation and characterization of pituitary autoantigen by affinity chromatography. For isolation we have used microsomal fraction of human pituitary.
MATERIAL AND METHODS: For immunoglobulins isolation have been used sera of pituitary disease, Addison disease and Graves-Basedow disease patients with detectable pituitary autoantibodies. For pituitary antigen isolation have …

Autoimmune polyglandular syndrome (APS) is a rare disease that is characterized by autoimmune reactions to multiple endocrine and non-endocrine organs, which can be divided into four main types. The principal manifestations of APS-3 are autoimmune thyroid disease and other autoimmune diseases, such as type 1 diabetes, atrophic gastritis, pernicious anemia, vitiligo, alopecia, and myasthenia gravis, but not Addison's disease or hypoparathyroidism. Here we report a case demonstrating the rare coexistence of growth hormone deficiency and hyperthyroidism with sexual dysgenesis, secondary amenorrhe

Primary adrenal insufficiency is the result of impaired steroid synthesis, adrenal destruction or abnormal development of the adrenal gland affecting the adrenal cortex. Tuberculosis is one of the main causes in developing countries. We present the case of a 10-year-old male patient diagnosed with adrenal insufficiency two years earlier with regular treatment, who was admitted for adrenal crisis. A thoraco-abdominal tomography was carried out during hospitalization, which showed cylindrical and traction bronchiectasis associated with cicatricial atelectasis in the right upper pulmonary lobe, e

BACKGROUND: Given the sparse nature of acute mania or psychosis in primary adrenal insufficiency (PAI), physicians may not be aware of the association of these two entities.
OBJECTIVE: To conduct a systematic review of the literature for the purpose of identifying all studies reporting mania and/or psychosis in individuals with PAI.
METHOD: We conducted a systematic review according to PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines using the PubMed, Embase, and Web of Science databases from June 22, 1970 to June 22, 2021, for the purpose of identifying

Cardiac tamponade is an important and severe consequence of pericardial effusion. Patients with haemodynamically significant pericardial effusions present with signs and symptoms relating to the degree of their impaired cardiac function. Although autoimmune disease is a recognised cause of pericardial effusion, cardiac tamponade as a clinical presentation of primary adrenal insufficiency (PAI) is infrequently reported. We present a case of a woman, in her early 50s, who was admitted to the coronary care unit with cardiac tamponade caused by an acute adrenal crisis from unrecognised PAI. We hop
