Combined transnasal/transoral endoscopic odontoid resection in pediatric patients: Otolaryngologic considerations to airway management, endoscopic exposure, and complication management.

BACKGROUND: Anterior brainstem compression from odontoid pathology can occur in patients with craniocervical disorders. Occasionally, odonto … videolaryngoscopy for intubation, with or without fiberoptic endoscopic assistance. Seven underwent adenoidectomy, two underwent adenotonsillectomy, and one required adenoidectomy with midline palatal split and inferior turbinate outfracture. Four patients had undergone prior adenotonsillectomy. Presenting symptoms included extremity weakness (n = 9), dysphagia …

Nasopharyngeal Hairy Polyp Causing Pediatric Sleep-Disordered Breathing.

A rare congenital nasopharyngeal hairy polyp was incidentally identified during an attempted adenoidectomy in a 15-month-old female. The patient presented with snoring and intermittent apnea concerning for upper airway obstruction. Magnetic resonance imaging (MRI) revealed a 2.5-cm partially enhancing nasopharyngeal mass along the soft palate, likely originating near the left nasopharyngeal wall or Eustachian tube orifice. The lesion was excised transorally without complications. Histopathological analysis confirmed a benign hairy polyp composed of mature adipose tissue, skeletal muscle, and c…

Limitations of the Apnea-Hypopnea Index in pediatric obstructive sleep apnea. A critical review: Toward a multiparametric approach.

OBJECTIVES: The Apnea-Hypopnea Index (AHI) is considered the gold standard for diagnosis and severity classification of pediatric obstructive sleep apnea (OSA). This review critically analyzes the scientific foundations of this paradigm and its clinical implications. MATERIALS AND METHODS: A systematic literature review in the PubMed and Cochrane databases was conducted according to the SWiM methodology (Synthesis Without Meta-analysis) following EQUATOR network guidelines. Studies were grouped according to 5 themes: methodological foundations of the AHI, clinical correlations, alternative pa

Residual OSA in Down syndrome: does body position matter?

STUDY OBJECTIVES: To examine children with Down syndrome with residual obstructive sleep apnea (OSA) to determine if they are more likely to have positional OSA. METHODS: A retrospective chart review of children with Down syndrome who underwent adenotonsillectomy at a single tertiary children's hospital was conducted. Children with Down syndrome who had a postoperative polysomnogram with obstructive apnea-hypopnea index (OAHI) > 1 event/h, following adenotonsillectomy with at least 60 minutes of total sleep time were included. Patients were categorized as mixed sleep (presence of ≥ 30 minutes

Effect of surgical intervention for mild childhood obstructive sleep apnoea on attention and behavioural outcomes: A randomized controlled study.

BACKGROUND AND OBJECTIVE: We evaluated inattention and behavioural outcomes following surgery versus watchful waiting (WW) in school-aged children with mild obstructive sleep apnoea (OSA). METHODS: A prospective randomized controlled study was performed in pre-pubertal children aged 6-11 years with polysomnography (PSG)-confirmed mild OSA. They were assigned randomly to early surgical intervention (ES) or WW. The surgical intervention consisting of tonsillectomy with or without adenoidectomy and turbinate reduction was carried out within 4-6 weeks after randomization. Both groups underwent PS…

Adenotonsillectomy for the treatment of obstructive sleep apnoea in extreme paediatric obesity.

OBJECTIVE: Adenotonsillectomy is a recognised treatment for paediatric obstructive sleep apnoea. Recent literature highlights the need to assess outcomes in the extremely obese subpopulation. This study reviewed the efficacy of adenotonsillectomy performed in patients with extreme obesity. METHOD: A retrospective analysis of attendees at a tertiary paediatric obesity clinic was performed, identifying patients with a body mass index z-score equal to or more than three who had undergone adenotonsillectomy for obstructive sleep apnoea. Electronic patient records, including respiratory polygraphy

Can pediatric sleep questions be incorporated into a risk model to predict respiratory complications following adenotonsillectomy?

BACKGROUND: Adenotonsillectomy, one of the most frequent surgical procedures in children, is usually performed for sleep-disordered breathing, a disease spectrum from primary snoring to obstructive sleep apnea. Children undergoing an adenotonsillectomy may be at risk for perioperative respiratory complications, necessitating intervention or escalation of care. However, there is no effective preoperative screening or risk-stratification model for perioperative respiratory complications that incorporates not only clinical history and physical examination but also sleep question responses for chi

Effect of adenotonsillectomy in children with obstructive sleep apnea and major psychiatric disorders on obstructive apnea-hypopnea index and Epworth Sleepiness Scale scores.

STUDY OBJECTIVES: This study evaluates the effectiveness of adenotonsillectomy in the treatment of obstructive sleep apnea in children with major psychiatric disorders as measured by polysomnography and the Epworth Sleepiness Scale (ESS) at a tertiary children's hospital. Adults with major psychiatric disorders often have higher rates of obstructive sleep apnea and decreased response to treatment. The goal was to determine if children with serious mental illness had outcomes similar to their adult counterparts. METHODS: A retrospective chart review was undertaken to identify children with obs

Development of a pediatric obstructive sleep apnea triage algorithm.

INTRODUCTION: Diagnosis and treatment of obstructive sleep apnea (OSA) in children is often delayed due to the high prevalence and limited physician and sleep testing resources. As a result, children may be referred to multiple specialties, such as pediatric sleep medicine and pediatric otolaryngology, resulting in long waitlists. METHOD: We used data from our pediatric OSA clinic to identify predictors of tonsillectomy and/or adenoidectomy (AT). Before being seen in the clinic, parents completed the Pediatric Sleep Questionnaire (PSQ) and screening questionnaires for restless leg syndrome (R…

Is securing normal dentofacial development an indication for tonsil surgery in children? A systematic review and meta-analysis.

OBJECTIVE: Tonsil surgeries are common operations in the field of paediatric otorhinolaryngology. Often, the indication for these operations is hypertrophied tonsils. Paediatric sleep-disordered breathing and mouth-breathing are conventional situations caused by the hypertrophied tonsils. Both of these are further associated with dentofacial development alterations. Securing normal dentofacial development, or restoring it, is often used as an indication for tonsil surgery. In this review and meta-analysis, we assessed the contemporary literature to clarify whether tonsil surgery has an effect

Diagnostic dilemma in a case of obstructive sleep apnoea syndrome with reversible pulmonary hypertension and right heart failure: A case report.

Childhood obstructive sleep apnoea syndrome (OSAS) secondary to adenoid hyperplasia is known to give rise to pulmonary hypertension. However, we present a case of a toddler with pulmonary hypertension (PH) and right heart failure due to OSAS, the cause of which is difficult to identify. After the patient underwent an adenotonsillectomy, OSAS disappeared and the PH eventually resolved. Both paediatricians and otolaryngologists should know that paediatric OSAS can occur even in the setting of mild, clinically insignificant palatine tonsil hypertrophy and adenoid hyperplasia. Surgical interventio

Gravitational forces, negative pressure and facial structure in the genesis of airway dysfunction during sleep: a review of the paradigm.

The recent and distant literature has extensive discussion of how sleep apnea, adeno-tonsillar growth, and facial structural deformity are related. Conventionally, the order of cause and effect is as follows: (1) Inflammatory/infectious process→tonsillar/adenoid tissue growth→(2) airway obstruction and mouth breathing/Obstructive Sleep Apnea (OSA)→(3) altered facial structure (adenoid facies). Using this same reasoning, adenotonsillectomy is the first line of treatment in the prevention of structural abnormalities. However, through a lifetime of clinical research Christian Guilleminault and hi

[Current aspects of the problem of tonsillar disease].

The paper presents current concepts and the author's data on morphofunctional organization of lympho-epithelial symbiosis of the palatine and pharyngeal tonsils in children operated because of tonsil disease (chronic tonsillitis and chronic adenoiditis). The structure and functional significance of the areas of the epithelium reticulation as immune regulatory tonsil compartments and their possible role in pathogenesis of tonsillogenic complications are discussed.

[Hyperplastic adenoids and growth of the facial skeleton].

A case of a 16-year old female patient with large hyperplastic adenoids is presented. The only subjective symptoms were blocked nasal ventilation and subsequent permanent mouth breathing for 10 years. Concomitant sinusitis and middle ear affection were absent. In addition a gothic palate with malocclusion due to prognathism was found. The possible relationship between hyperplastic adenoids, chronic mouth breathing and craniofacial growth is discussed.

Unconsciousness associated with midazolam and erythromycin.

An 8-yr-old boy suffering from an asymptomatic ventricular septal defect was given erythromycin for antibiotic prophylaxis before adenoidectomy. Sixty minutes after premedication with oral midazolam 0.5 mg kg-1 and oral atropine 0.03 mg kg-1, an infusion of erythromycin 400 mg was started. When 200 mg of erythromycin had been infused, the patient lost consciousness, but other vital functions remained normal. After 45 min, he awakened spontaneously. At the time the plasma concentration of midazolam was 134 ng ml-1. In order to investigate possible interactions between midazolam and erythromycin…
