First case report of topical tacalcitol for vitiligo repigmentation.

We present a 11-year-old girl with an 8-month history of vitiligo vulgaris on her postcervical, periocular, and periauricular regions. The region topically treated with 0.0002% tacalcitol ointment (vitamin D3 ointment) and sunlight exposure healed completely, while the region treated with the ointment alone did not improve. A combination therapy with tacalcitol ointment and sunlight exposure instead of UV irradiation can be recommended as an alternative therapy for vitiligo vulgaris because the treatment is safe and increases compliance in children.

Novel drug delivery systems: potential in improving topical delivery of antiacne agents.

Acne is the most common cutaneous disorder of multifactorial origin with a prevalence of 70-85% in adolescents. The majority of the acne sufferers exhibit mild to moderate acne initially, which progresses to the severe form in certain cases. Topical therapy is employed as first-line treatment in mild acne, whereas for moderate and severe acne, systemic therapy is required in addition to topical therapy. Currently, several topical agents are available that affect at least one of the main pathogenetic factors responsible for the development of acne. Although topical therapy has an important posi

Transdermal administration of bromocriptine.

Bromocriptine (BRC) has been mainly used for the inhibition of lactation, treatment of menstrual disorders, Parkinson disease, breast tumours, infertility and brain tumours as a dopamine agonist in clinics. But current BRC formulations have some side effects and bioavailability problems because of hepatic first pass effect. Transdermal application could be an alternative route to overcome all these problem and penetration properties of BRC has not been studied yet. Therefore, it was aimed to investigate the effectiveness of transdermal formulation of BRC which is applicable to the skin. For th

High-frequency ultrasound in the evaluation of pharmacological effects on the skin.

In 1987, the introduction of two 20-MHz B- (brightness-) scan ultrasound systems with axial and lateral resolution of 200 microm and 75-80 microm, respectively, rendered two-dimensional sonographic imaging in dermatology possible. Since then cutaneous sonography has increasingly been used to investigate drug effects on human skin. Both unwanted and wanted effects have been studied, including skin thinning due to repeated glucocorticoid application and reduction of skin thickening in hyperplastic disorders such as psoriasis vulgaris due to various treatment approaches. The resolution of 20-MHz

Successful treatment of perianal warts in a child with 5% imiquimod cream.

Treatment of anogenital warts in children is difficult. Commonly used therapeutic regimes can be painful, variably effective and recurrence rates are high. Imiquimod is a recently developed imidazoquinolin heterocyclic amine that is an immune response modifier. Topical imiquimod has been used successfully to treat anogenital warts in adults. This case documents the effective use of topical imiquimod in the treatment of perianal warts in children.

Arrest of preterm labour and prolongation of gestation with glyceryl trinitrate, a nitric oxide donor.

13 women admitted consecutively in preterm labour were recruited to an observational study of the effect of glyceryl trinitrate (GTN) patches on uterine contractions and prolongation of pregnancy. All twenty episodes of preterm labour (at 23-33 weeks' gestation) responded. 1 patient delivered because of cervical incompetence despite cessation of uterine contractions. The mean prolongation of pregnancy was 34 days. 8 babies have been born so far, and are progressing well. GTN patches appear to be a safe, well-tolerated, and non-invasive method of suppressing uterine contractions in preterm labo

Clinical controversy on the effect of topical ciclosporin: what is the target site?

In recent years attempts have been made to treat T-cell-mediated skin diseases with topical therapeutics. Based on clinical data on the local treatment of recalcitrant erosive lichen planus (LP) with ciclosporin (CS) we discuss in vitro and in vivo studies on percutaneous absorption of CS, drug localization and drug metabolism in the skin as well as clinical data. Clinically relevant immunosuppressive activity depends not only on drug distribution in the target organ skin. The inhibition of T cell response is also dependent upon T cell subsets involved and the activation stage of the T cell. T

Cellular and molecular basis of barrier function in oral epithelium.

The use of the oral mucosa for drug delivery and the erroneous belief that it is a nonkeratinized tissue have given rise to the suggestion that the oral mucosa is a permeable tissue. Such an assumption is not supported by studies which indicate that permeability differs significantly in different oral regions, depending on the pattern of epithelial differentiation. Keratinized regions such as hard palate and gingiva have a permeability which is significantly less than nonkeratinized regions like buccal mucosa and floor of mouth. Nevertheless, all oral regions are more permeable than skin. Asso

Effect of polyoxyethylene glycols (PEG) on properties of the matrix model of transdermal therapeutic system (TTS) with testosterone.

Polyoxyethylene glycols (PEG) are nontoxic substances, which ones do not irritate the skin, therefore can be used as pharmaceutical excipients to differentiate the properties of various matrices. The effect of variable content and molecular mass of PEG in cellulose acetate matrices on the testosterone solubility, the rate of water sorption and the release rate of testosterone from the matrix model of TTS was studied. It may be concluded that the testosterone solubility in matrices, the rate of water sorption by systems and the pharmaceutical availability of drug can be regulated on a relativel

In-vivo pharmacokinetic characteristics of a transdermal phenylpropanolamine (PPA) preparation.

Phenylpropanolamine hydrochloride (PPA-HCL) is a synthetic phenylisopropanolamine sympathomimetic agent which is structurally related to ephedrine and amphetamine. Although its precise mechanism of action has not been conclusively determined, PPA is known to exert cardiovascular effects possibly related to indirect stimulation of beta-adrenergetic receptors in the heart. In addition, through CNS stimulation, PPA is known to act as an appetite suppressant, the anorexigenic effect being much weaker than that of amphetamine. In order to develop a convenient dosage of PPA, a transdermal preparatio

Propylene glycol dermatitis.

Propylene glycol is a commonly used vehicle for topical preparations. Although it is well suited for this purpose, it is capable of producing both primary irritant skin reactions and allergic sensitization. The literature on propylene glycol is reviewed, with particular attention to the nature of these adverse cutaneous effects. Guidelines for patients sensitive to propylene glycol are discussed.

[Efficacy of percutaneous caffeine in the treatment of apnea in the premature infant].

In 18 prematures (gestational age less than 32 weeks), an aqueous monohydrated caffeine colloidal gel was administered topically. High plasma levels were rapidly obtained with a loading dose divided in 4 applications by massaging, 1 ml of gel per kg body weight, at 12 hour intervals (18 mg caffeine base/kg). Maintenance was then instituted with a dose of 0.5 ml/kg/day. Treatment was initiated before 8 days of life, in children under assisted ventilation but without severe pulmonary involvement, during the period of weaning from the respirator. A good local and general tolerance was invariably

Side effects of transdermal scopolamine.

Motion sickness can be experienced as the result of air or sea travel. Scopolamine can be used before travel to prevent this illness. This report describes two patients who used transdermal scopolamine preparations and experienced hallucinations and then confusion as side effects. The discussion concerns the recognition and treatment of these adverse effects.
