The continuity hypothesis: the relationship of long-term alcoholism to the Wernicke-Korsakoff syndrome.

Although alcoholic Korsakoff's syndrome has traditionally been considered an acute disorder related to a nutritional deficiency, recent evidence demonstrating that ethanol may be neurotoxic has raised the possibility that the perceptual, problem-solving, and memory deficits associated with this chronic neurological disorder may develop slowly over decades of alcohol abuse. A review of the recent cognitive literature provides only limited support for this "continuity hypothesis." Long-term alcoholics, as with patients with alcoholic Korsakoff's syndrome, are impaired on numerous visuoconceptual

[Enzyme induction of the liver caused by chronic alcoholism as risk factor in hepatotoxicity].

Abuse of ethanol has shown to induce the drug-metabolizing enzyme activity of the liver in rats and in humans. Therefore, we studied cytochrome P-450 dependent monooxygenase and conjugating enzyme activity in needle liver biopsy specimens of patients using the following substrates and enzyme assays: 7-ethoxycoumarin O-deethylase (EOD), p-nitroanisol O-demethylase (PNA), NADPH-cytochrome c reductase, 1-naphtol glucuronyltransferase (NGT). Among alcoholics induction of EOD, PNA and NGT was only found in patients with active alcoholic liver damage (elevated transaminases and necrosis of the liver

Work-related outcomes of the constructive-confrontation strategy in a job-based alcoholism program.

Most job-based alcoholism policies and programs incorporate a strategy of constructive confrontation. Data from two national samples of over 600 managers in a large corporation were collected to assess the effectiveness of this strategy with both problem drinkers and other problem employees. Results showed that supervisors of problem drinkers took more actions than did supervisors of employees with other problems. Work performance of problem-drinking employees improved more than that of other problem employees. Oral discussions containing both constructive and confrontational topics were posit…

[Macrocytosis and anemia in chronic alcoholism. Correlation with the results of hepatic needle biopsy].

Clinical and laboratory observations were made on 220 chronic alcoholics, regularly taking at least 150 g of alcohol daily. Haematological d … was seen between male and female alcoholics. Macrocytosis may be considered as an early marker for alcoholism but it does not correlate with the type of histological liver damage. Hemoglobin levels were significantly reduced in chronic alcoholics (P less than 0,01): the degree and incidence of anemia were more severe in those patients with advanced liver di …

Variable prognosis in congestive cardiomyopathy. Role of left ventricular function, alcoholism, and pulmonary thrombosis.

Prognosis of 36 patients with congestive cardiomyopathy was studied in relation to various clinical factors. Half life of the survival curve after overt heart failure was about 7 years. Although left ventricular function was a major determinant of clinical course in congestive cardiomyopathy in general, its relation to prognosis was variable according to the type of cardiac involvement. In peripartal cardiomyopathy and in a type of cardiomyopathy named subacute cardiomyopathy with pulmonary thrombosis in this paper, factor(s) other than left ventricular function, possibly including pulmonary t

Peyote, a potential ethnopharmacologic agent for alcoholism and other drug dependencies: possible biochemical rationale.

The authors examine folk psychiatry among Native American Church members from an enthnopharmacologic viewpoint. Alcohol and opiate abuse among Indian and non-Indian are presented in case histories proving to be asymptomatic under Indian guidance and through participation in the peyote ritual. The biochemical alkaloids common in the peyote cactus, rather than just the psychoactive substances (mescaline), are purported to be pharmacologically similar to the neuroamine-derived alkaloids found in the brain during alcohol intoxification. Evidence is reviewed that points out possible common features

Hypothesis: energy wastage in alcoholism and drug abuse: possible role of hepatic microsomal enzymes.

Hepatic microsomal drug oxidation appears to be an energy-wasteful process because it has no known link with energy-conserving mechanisms such as the synthesis of ATP, and in addition it requires the uncoupled oxidation of NADPH. It is postulated that this can appreciably alter the energy balance of the whole body under certain conditions. Such an imbalance would be favored by the repeated intake of drugs that induce hepatic microsomal enzymes and by the provision of ethanol as a major source of calories. The hypothesis is consistent with the changes in body weight in humans and animals after

THE INFLUENCE OF ACUTE ALCOHOLISM ON THE NORMAL VITAL RESISTANCE OF RABBITS TO INFECTION.

THE DEDUCTIONS THAT MAY BE DRAWN FROM THE RESULTS OF THESE EXPERIMENTS ARE AS FOLLOWS: That the normal vital resistance of rabbits to infection by streptococcus pyogenes (erysipelatos) is markedly diminished through the influence of alcohol when given daily to the stage of acute intoxication. That a similar, though by no means so conspicuous, diminution of resistance to infection and intoxication by the bacillus coli communis also occurs in rabbits subjected to the same influences. And that, while in alcoholized rabbits inoculated in various ways with staphylococcus pyogenes aureus, individual

ECG-voltage in alcoholics and non-alcoholics with acute alcohol intoxication.

Alcohol intoxication is probably the most common intoxication worldwide, and may be lethal. The exact mechanism by which ethanol intoxication contributes to death is unknown, although ventricular tachyarrhythmias degenerating into fibrillation is a possible cause. Alcoholics have increased risk of sudden death and, possibly, higher risk than occasional drinkers. In 32 consecutive patients with alcohol intoxication Delta-voltage was the differences in voltage between values at admission (when patients had high blood alcohol levels) and at discharge (when the patients were assumed sober), and wa

Effects of aminooxyacetic acid on learning and memory function and neurochemical changes in chronic alcoholism.

OBJECTIVE: This study aimed to investigate the effect of aminooxyacetic acid (AOAA) on cognitive function, particularly learning and memory, in a rat model of chronic alcoholism. Additionally, the study explored changes in cystathionine β-synthase (CBS), hydrogen sulfide (H₂S), and serotonin (5-HT) levels in the prefrontal cortex to understand the potential neurochemical mechanisms involved. METHODS: Sixty-four male SD rats were randomly divided into four groups, with 16 rats in each: Con, Con + AOAA, Model, and Model + AOAA. The Model group received a 6 % ethanol solution for 28 days. From d…

Alcohol and alcoholism associated neurological disorders: Current updates in a global perspective and recent recommendations.

Alcohol use disorder (AUD) is a medical condition that impairs a person's ability to stop or manage their drinking in the face of negative social, occupational, or health consequences. AUD is defined by the National Institute on Alcohol Abuse and Alcoholism as a "severe problem". The central nervous system is the primary target of alcohol's adverse effects. It is crucial to identify various neurological disorders associated with AUD, including alcohol withdrawal syndrome, Wernicke-Korsakoff syndrome, Marchiafava-Bignami disease, dementia, and neuropathy. To gain a better understanding of the n…

The Lower Survival in Patients With Alcoholism and Hepatitis C Continues in the DAA Era.

BACKGROUND: Alcohol liver disease (ALD) may coexist with hepatitis C (HCV) in many transplant recipients (alcoholic cirrhosis with hepatitis C [AHC]). Our objective was to determine whether there were differences in postliver transplantation outcomes of patients with AHC when compared with those with alcoholic cirrhosis (AC) and/or alcoholic hepatitis (AH). METHODS: Using UNOS explant data sets (2016-2020), the survival probabilities of AC, AH, and AHC were compared by Kaplan-Meier survival analysis. Cox proportional-hazard regression analysis was used to determine outcomes after adjusting fo

Multi-omics integration analysis identifies novel genes for alcoholism with potential overlap with neurodegenerative diseases.

Identification of causal variants and genes underlying genome-wide association study (GWAS) loci is essential to understand the biology of alcohol use disorder (AUD) and drinks per week (DPW). Multi-omics integration approaches have shown potential for fine mapping complex loci to obtain biological insights to disease mechanisms. In this study, we use multi-omics approaches, to fine-map AUD and DPW associations at single SNP resolution to demonstrate that rs56030824 on chromosome 11 significantly reduces SPI1 mRNA expression in myeloid cells and lowers risk for AUD and DPW. Our analysis also i

Reversible lesion in the splenium of the corpus callosum in a patient with chronic alcoholism.

Marchiafava-Bignami disease (MBD) is often diagnosed in chronic alcoholics. The disease processes typically involve the corpus callosum and clinically presents with various manifestations on the basis of clinical condition, extent of the splenium of the corpus callosum involvement at brain magnetic resonance imaging (MRI), and prognosis. We report a patient affected by MBD, who presented an isolated reversible splenial lesion at brain MRI and achieved a favorable recovery.
