European clinical practice guideline: managing and treating laryngopharyngeal reflux disease.

OBJECTIVE: To propose a European consensus for managing and treating laryngopharyngeal reflux disease (LPRD) to guide primary care and specialist physicians. METHODS: Twenty-three European experts (otolaryngologists, gastroenterologists, surgeons) participated in a modified Delphi process to revise 38 statements about the definition, clinical management, and treatment of LPRD. Three voting rounds were conducted on a 5-point scale and a consensus was defined a priori as agreement by 80% of the experts. RESULTS: After the third round, 36 statements composed the first European Consensus Report

Metabolomic and sphingolipidomic profiling of human hepatoma cells exposed to widely used pharmaceuticals.

Pharmaceutical compounds have become one of the main contaminants of emerging concern (CECs) due to their high usage and increased release into the environment. This study aims to assess the effects caused by three widely consumed hepatotoxic pharmaceutical compounds: an antibiotic (amoxicillin), an antiepileptic (carbamazepine), and an antidepressant (trazodone), on human health when indirectly exposed to toxicologically relevant concentrations (30, 15, and 7.5 μM for amoxicillin and carbamazepine, and 4, 2, and 1 μM for trazodone). A combination of semi-targeted metabolomic and targeted sphi

On-demand radiofrequency-controlled drug release from microcapsule depot in ex vivo tissue model.

Passive antibiotic release from implanted depot systems often leads to sub-therapeutic dosing, which in some cases may increase the risk of bacterial resistance. Addressing this limitation requires precise, controllable dosing strategies that do not rely on additional surgical interventions. To this end, this work presents a core-shell depot microcapsule system that enables externally controlled, on-demand antibiotic release using radiofrequency (RF) heating. The microcapsules (d ≈ 500 µm) consist of a norfloxacin-loaded wax solid core and a dually crosslinked methacrylated alginate shell cont

Assessment of Healing Potential of Alginate and Chitosan-Based Biomaterials for Cranial Bone Defects in Experimental Model.

The healing of bone defects in the skull vault is a serious clinical problem. The aim of this study is to evaluate the effect of two osteoplastic materials on the healing of a cranial vault experimental defect by computed tomography. Two material systems are analyzed in detail: (i) a sodium alginate-based matrix modified with hydroxyapatite microparticles and zinc ions and (ii) a chitosan-based matrix containing in situ-formed brushite nanoparticles; both sample types were loaded with cholecalciferol (VitD3). A total of 36 rats aged 6 months showed that the alginate-based biomaterial promotes

Modification of chitosan/calcium alginate/Fe3O4 hydrogel microsphere for enhancement of Cu(II) adsorption.

Copper in drinking water causes a significant environmental problem. Composite material based on alginate hydrogel has been gaining attention in the field of Cu(II) adsorption. However, alginate-based hydrogel exhibits poor mechanical property and relative low adsorption capacity which limit their application. The present study is devoted to the modification of chitosan/calcium alginate/Fe3O4 (CAF) hydrogel microsphere by NaOH solution for enhancement of Cu(II) adsorption. Results reveal that modification of CAF via NaOH solution significantly improves the mechanical strength and Cu2+ adsorpti

Spontaneous Coassembly of Biologically Active Nanoparticles via Affinity Binding of Heparin-Binding Proteins to Alginate-Sulfate.

Controlled delivery of heparin-binding (HB) proteins represents a challenge in regenerative medicine strategies. Here, we describe the features of novel nanoparticles (NPs), spontaneously coassembled due to affinity interactions between HB proteins and the semisynthetic anionic polysaccharide, alginate-sulfate. The NPs efficiently encapsulated and protected the proteins from proteolysis. Injection of a combination of NPs encapsulating multiple therapeutic growth factors promoted effective and long-term tissue repair in animal models of severe ischemia (murine model of hindlimb ischemia and acu

Honey and larvae in the treatment of foot gangrene: a reflection.

With the introduction of the Nursing and Midwifery Council's (NMC) (2015a) revalidation directive, nurses are required to demonstrate an ongoing commitment to providing safe and effective care by continually combining sound empirical evidence with reflective practice ( Sackett et al, 1996 ; Rolfe et al, 2011 ). Using Gibbs' (1998) model, I will reflect on an episode of care undertaken while I was on a recent placement. This reflective account will discuss the clinical use of honey and larvae therapy in the treatment of foot gangrene following meningococcal septicaemia. The psychosocial impact

Calcium alginate dressing-related hypercalcemia.

To date, no adverse effects or contraindications have been observed in association with the use of calcium alginate dressing on an exuding wound. This is the first case in the literature in which it is suggested that using a large quantity of calcium alginate dressing could be harmful. The scenario leading to the complication and suggestions to prevent similar conditions in the future are described.

Microencapsulation of Liver Spheroids with Poly(Vinyl Alcohol) Enhances Function Compared with Alginate.

Background and Aims: Cell therapy approaches to treating chronic liver disease provide only transient improvements, mainly due to loss of hepatocytes after infusion. Microencapsulation in alginate has been shown to protect transplanted cells from physical stress and rejection, but the poor biocompatibility of alginate can lead to graft failure. This study aimed to evaluate a biocompatible poly(vinyl alcohol) (PVA)-based microcapsule against standard alginate for improved transplantation outcome of liver spheroids. Materials and Methods: Human hepatocyte spheroids were microencapsulated in algi

Impact of yeast cell wall incorporation on the mucoadhesion, stability, oral permeability and release profile of alginate/whey protein beads loaded with insulin.

This study aimed to evaluate the impact of incorporating yeast cell wall (YCW) into alginate/whey protein (ALG/WP) particles as a strategy to improve oral insulin delivery. Insulin-loaded particles were produced by an extrusion-gelation process with or without YCW, and their physicochemical, mucoadhesive, and permeability properties were assessed in vitro, ex vivo, and in vivo. The inclusion of YCW increased the viscosity of the polymeric solution, resulting in more cohesive particles and a significant reduction in insulin loss during coating. Encapsulation efficiencies ranged from 65 to 99%.
