Tissue changes around dental implants installed in alveolar ridge preservation sites: A 1-year follow-up randomized controlled clinical trial.

OBJECTIVE: This study aimed to assess radiographic marginal bone changes 22 months post extraction, which is 1 year after implant loading in alveolar ridge preservation (ARP) sites grafted with a combination of collagen-embedded xenogenic bone substitute (DBBM-C) and collagen matrix (CMX), comparing them with implants placed in naturally healed sites. METHODS: This randomized controlled clinical trial was conducted over 22 months. Patients who needed a single tooth extraction and subsequent implant placement in nonmolar areas were enrolled. The test group received deproteinized bovine bone mi

The Bone Core Technique for the Augmentation of Limited Bony Defects: Five-Year Prospective Study with a New Minimally Invasive Technique.

The aim of this study was to evaluate a new minimally invasive surgical technique for the reconstruction of critical-size bony defect with local harvested bone core with simultaneous implant placement. In a prospective study, 186 consecutively treated patients were included and controlled clinically and radiologically for at least 5 years postoperative. Every patient presented a bony defect affecting the buccal, lingual, or palatal wall. In all cases, the alveolar crest was wide enough to allow implant placement inside the bony contours. During implant bed preparation, a trephine bur (3.5 mm e

The bone growing concept: A call for a paradigm shift in bone reconstruction.

Alveolar bone reconstruction has significantly evolved over the years. The transition from bone transfer techniques to bone regeneration methods aimed to reduce morbidity, increase the available reservoir of bone, and enhance volumetric outcomes. However, current bone regeneration techniques are prolonged and yield suboptimal biological results. This is primarily because the process relies heavily on bone substitutes that lack osteoinductive or osteogenic properties, with the supply of cells and growth factors entirely dependent on endogenous sources. This article calls for a new paradigm shif

Implant based rehabilitation options for the atrophic edentulous jaw.

The atrophic and edentulous jaw can pose a number of challenges for the implant clinician. In simple terms, the amount of bone that remains is insufficient for the conventional placement of a dental implant. A variety of treatment strategies can be employed to enable implants to be placed despite the paucity of bone stock in either the mandible or the maxilla. Conceptually these strategies follow one of two pathways: either augmentation of the bone, or the novel utilization of the remaining bone. This article will discuss patient assessment, treatment planning, and the range of contemporary op

Relationship between the buccal bone plate thickness and the healing of postextraction sockets with/without ridge preservation.

In 48 postextraction sites, sockets were grafted with bovine bone mineral plus collagen membrane (test) or had spontaneous healing (control). After 4 months, horizontal ridge width reduction was 0.71 mm in the test group and 4.04 mm in the control group, while vertical ridge loss was 0.58 mm and 1.67 mm, respectively. No correlation was found between the thickness of the buccal bone wall and the alveolar bone loss in the test group, while an inverse correlation was found in the control group. Ridge preservation compensated for postextraction alveolar ridge resorption irrespective of the buccal

An interarch alveolar ridge relationship classification.

Alveolar ridge defects resulting from tooth loss, trauma, periodontal disease, or congenital lesions often require correction prior to dental implant therapy. Numerous classifications proposed to describe alveolar ridge defects have been limited to describe intra-arch relationships. To provide sufficient jaw-to-jaw alveolar ridge (interarch) relationship information for both restorative and surgical treatment decision-making, this article introduces a new interarch alveolar ridge relationship classification system. This article also discusses cone beam computed tomography and multidisciplinary

[Usefulness of hydroxyapatite implants for the management of alveolar bone lesions at tooth hemisection sites].

INTRODUCTION: Hemisection is a surgical method for treating lesions of two- and multi-rooted teeth. This procedure includes amputation of one or two lesioned roots together with the attached dental crown, leaving an endodontically treated root and the remaining dental crown prepared for future prosthetic restoration. The results of this study demonstrate that HA Biocer implants create optimal conditions for new bone growth. MATERIAL AND METHODS: The study was done in 51 patients after hemisection treated with HA Biocer in the form of granules implanted into the alveolus. The control group con

Immediate replacement of a maxillary central incisor associated with severe facial bone loss: use of Bio-Oss collagen--case report.

The loss and replacement of anterior maxillary teeth pose several challenges. Treatment must successfully address immediate tooth replacement, esthetics, patient comfort, and psychologic acceptance. The purpose of this case report is to present a prosthetic and surgical technique for immediate tooth replacement following extraction in an area of severe localized bone loss. Combined full- and partial-thickness flaps associated with the use of Bio-Oss collagen grafting material and a Bio-Gide membrane were used for alveolar ridge reconstruction. To improve the final esthetic result, a connective

Advanced surgical procedures: bone augmentation.

Over the last ten years there has been a significant increase in the range and type of edentulous defects that can be treated using osseointegrated implants. Encouraged by the long-term success of implant reconstructions in the edentulous mandible and maxilla, and the availability of novel implant attachments, clinicians will now undertake more elaborate treatment involving the partially edentate and those with localized or generalized tissue deficiencies. This clinical trend places increasing demands on the predictability, complexity and accuracy of the surgical procedure necessary to allow a

Vascular Structures of the Lateral Wall of the Maxillary Sinus: A Vascular Labeling Technique.

PURPOSE: Sinus floor augmentation is a common procedure in implant dentistry. However, several intraoperative complications can occur during this procedure, such as bleeding from the lateral wall of the maxillary sinus. The aim of this study was to describe the vascular structures of the lateral wall of the maxillary sinus using a vascular labeling technique. MATERIALS AND METHODS: Ten cadaveric specimens were prepared by the vascular labeling technique. Liquid latex was injected into the large vessels of the head, and the lateral wall of the maxillary sinus was exposed by dissection. The dia

The effects of β-TCP and/or xenogeneic bone substitute on alveolar ridge preservation: a randomized clinical trial.

OBJECTIVES: After tooth extraction, the alveolar ridge undergoes significant absorption, and the alveolar ridge preservation (ARP) will alleviate the complexity of implantation operations in specific clinical occasions and then improve the functionality and aesthetics of subsequent restoration. However, the effectiveness of β-tricalcium phosphate (β-TCP) composite materials for ARP needs to be confirmed by more clinical trials and more sufficient evidence. This study was aimed at evaluating the effects of β-TCP coating porous bovine deproteinization bone (β-TCP/PBDB) on ARP when compared with

HVC ridge deficiency classification: a therapeutically oriented classification.

Alveolar ridge defects resulting from tooth extraction, trauma, or periodontal disease often require surgical correction prior to prosthodontic reconstruction. Whether implants or conventional fixed prostheses are planned, without careful consideration and proper treatment planning, hard and/or soft tissue defects may lead to functional, structural, or esthetic compromises in the final prosthesis. This article reviews the etiology and treatment of alveolar ridge defects and introduces a therapeutically oriented classification system of such defects (horizontal, vertical, and combination-HVC-cl

[The maxilla--a poor implant site?].

In the maxilla, it is sometimes necessary to use implants to achieve a good prosthetic result. However, an increased failure rate of maxillary implants seems to be a common clinical experience. This experience was investigated in a retrospective statistical analysis. In a retrospective study of 665 patients between 1987 and 1997, 2484 implants were examined. The implants in the upper jaw were compared to those in the lower jaw. Implants with and without autogenous bone grafts and fixtures in patients with alveolar ridge atrophy, tumor, or trauma were explored. In particular, the data obtained
