New forceps and spatula for easy retropupillary implantation of iris claw lenses in aphakia: experience in 4 years of use.

PURPOSE: Retropupillary implantation of an iris claw lens offers a rapid and atraumatic approach for rehabilitation of aphakic eyes. The difficulty in the implantation process arises because of the possibility of losing the intraocular lens (IOL) into the vitreous cavity and the need to change the hand holding the forceps during the enclavation. METHODS: The new forceps design radically changes the method of grasping an IOL. It contains an extendable supporting plate with a hook-like end. The instrument achieves a horizontal three-point fixation at the edge of the implant in the 6 and 12 o''c

Visual results after primary intraocular lens implantation or contact lens correction for aphakia in the first year of age.

PURPOSE: The optimal role of intraocular lenses (IOLs) in infants remains a controversial topic. Some ophthalmologists advocate correction w … with 83% of children (19/23) in the CL group (p=0.039). CONCLUSIONS: We suggest that correction of aphakia after unilateral congenital cataract surgery with primary IOL implantation results in improved visual acuity, improved binocular vision outcome and less occurrence of strabismus, but a higher rate of complications requiring reoperation. Further stud …

Extended wear hydrogel and daily wear hard contact lenses for aphakia. Success and complications compared in a longitudinal study.

The success and complications of extended wear hydrogel (EWSCL) and daily wear hard contact lens (DWHCL) correction for aphakia were evaluated retrospectively in two comparable groups of unilaterally aphakic patients (64 with EWSCL, 86 with DWHCL). At the end of the follow-up period, 55% of patients in each group were still using contact lenses. Suppurative keratitis occurred in two (3%) patients using EWSCL and none with DWHCL. The relative risk to all patients of a complication developing severe enough to result in a loss of wearing time was 9.8 times higher for the EWSCL users compared with…

Rate of ocular trauma in children operated on for unilateral cataract in infancy-data from the Infant Aphakia Treatment Study.

To determine whether the fellow eye of children who have undergone unilateral cataract extraction in the first year of life are at increased risk of injury and vision loss, the 10.5-year data on 109 of 114 children enrolled in the Infant Aphakia Treatment Study were examined. Based on this limited data, it was estimated that the fellow eye is at greater risk of injury than the operated eye. Our data do not support the risk being higher in children with the worst vision in the treated eye.

Parenting Stress and Adherence to Occlusion Therapy in the Infant Aphakia Treatment Study: A Secondary Analysis of a Randomized Clinical Trial.

PURPOSE: Using data from the Infant Aphakia Treatment Study, we examined the relationship between adherence to patching and parenting stress. METHODS: Caregivers completed the Parenting Stress Index 3 months after surgery (n = 106), after a visual acuity assessment at 12 months of age (n = 97), and at 4.25 (n = 96) years of age. Patching was reported in quarterly telephone interviews and annual 7-day patching diaries, and averaged across all assessments prior to and in the 6 months following the first stress assessment, and for 6 months before and after the other two stress assessments. The a…

[Mutant gene expression in murine aggregation chimeras. 7. The effect of the aphakia gene--disorder in the formation of the crystalline capsule].

An analysis of aphakia (ak) gene expression in 16 day ak/ak C/C----+/+ c/c chimaeric embryos has shown, that ak gene, acting in developing lens, blocks lens cell differentiation and disturbs the formation by these cells of the extracellular matrix composing the lens capsule material. The dependence of capsule structure in chimaeras on the genotype of underlying cells indicates that lens cells are responsible for the formation of lens capsule.

Use of inverted telescopic corrections incorporating soft contact lenses in the (partial) correction of aniseikonia in cases of unilateral aphakia.

Soft contact lenses have been successfully adapted to inverted telescope units used to restore effective binocularity to selected unilateral aphakic patients. Techniques for adapting such units to the patient have been outlined and results have been reviewed. Differences between hard and soft contact lens techniques have been contrasted. The practitioner is encouraged to utilize these methods when binocularity would be a clear advantage to the patient with unilateral aphakia. The author hopes to adapt these techniques to the treatment of infants and young children in the future.

Correction of aphakia using biometry-based contact lens calculation in pediatric cataract extraction: introducing the aphakic contact lens formula.

PURPOSE: To develop and test a formula for contact lens calculation in aphakic infants. This formula, termed the "aphakic Contact Lens" (aCL) formula, utilizes preoperative axial length and keratometry data. METHODS: This retrospective, consecutive, two-center study used data derived from a development cohort (Freiburg, Germany) to create and optimize the aCL formula in aphakic eyes. A separate validation cohort (Frankfurt, Germany) was used to validate its predictive performance. The accuracy of the aCL formula was compared against: retinoscopy, a previously reported formula (Trivedi and Wil

Restricting feeding to dark phase fails to entrain circadian activity and energy expenditure oscillations in Pitx3-mutant Aphakia mice.

Metabolic homeostasis is under circadian regulation to adapt energy requirements to light-dark cycles. Feeding cycles are regulated by photic stimuli reaching the suprachiasmatic nucleus via retinohypothalamic axons and by nutritional information involving dopaminergic neurotransmission. Previously, we reported that Pitx3-mutant Aphakia mice with altered development of the retinohypothalamic tract and the dopaminergic neurons projecting to the striatum, are resistant to locomotor and metabolic entrainment by time-restricted feeding. In their Matters Arising article, Scarpa et al. (2022) challe…
