Early experience with the refractive keratoplasty techniques of José Barraquer--keratophakia and hypermetropic keratomileusis is presented. In contradistinction to the alloplastic lens substitutes currently being employed for the integral correction of aphakia, Barraquer's techniques would seem to offer a more permanent, more physiologic, full-time optical correction of the aphakic state. Sixty-five patients less than 10 years of age had cataract surgery on 99 eyes. The visual prognosis depended on the cataract type and whether the treatment was undertaken during the critical period for development of the fixation reflex. All patients received the same surgical technique, aggressive optical correction, and occlusion therapy, if needed. Since October 1977, the authors have attempted 32 keratophakias (refractive corneal surgery using an interlamellar homograft disc). Twenty-nine of these were primary and three secondary on aphakic eyes. These cases were divided into two series. In the first series, the dioptric correction was 28.5% less than the amount calculated to correct the aphakic error. The penetration of a pseudophakic eye by a high-speed metallic foreign body is reported. The path of the steel missile was interrupted by the intraocular lens, which was fixed within the capsular remnant, preventing missile penetration into the posterior segment. Although the foreign body was removed from the anterior chamber without immediate sequelae, several months later a peripheral retinal detachment occurred. Prompt recognition and treatment resulted in the retention of excellent pseudophakic vision. The importance of postoperative protective eyewear is emphasized.

There is increasing suspicion that topical ocular miotic therapy in selected persons may precipitate retinal detachments. This is a rare event and probably does not occur in patients free of retinal pathology. Data supporting a possible cause and effect relationship, obtained from the National Registry of Drug-Induced Ocular Side Effects, a survey of the Retina Society, medicolegal decisions, and a review of the literature, is presented.

The authors have studied the vitreous in 150 aphakic eyes after uneventful operations. They were examined by the slit-lamp and the Goldmann three-mirror glass. Two types of lesions were revealed: (1) 42% showed rupture of the anterior hyaloid membrane; this is not considered dangerous. (2) 69.3% showed posterior detachment of the vitreous, a more serious lesion. It is often associated with retinal lesions such as haemorrhage, pigmentary anomalies, and retinal tears. In 64 eyes the combined procedure (group I) and in 67 aphacic eyes a trabeculotomy (group II) was performed. There were different types of glaucoma. Re-examinations wer done 6 months and 1--10 years postoperatively. No definite differences of operative or postoperative complications existed between the two groups. PURPOSE: The aim of this review is to examine the techniques, complications, and outcomes of Descemet membrane endothelial keratoplasty (DMEK) in aphakic, aniridic, and vitrectomized eyes.
METHODS: A literature search was conducted in the MEDLINE database (via PubMed), using as keywords "(DMEK) AND (aphakia OR aniridia OR vitrectomy OR vitrectomized)." The research was limited to 10 years (January 2014-March 2024), in view of lack of literature before 2014. Articles, including case reports and case series, were included.

PURPOSE: Achieving near normal vision after unilateral congenital cataract (UCC) surgery is possible but requires early surgery, optical correction, and consistent patching. BACKGROUND AND OBJECTIVES: Congenital cataract is a condition compromising the eye's crystalline lens in infants and is usually diagnosed at birth. It can lead to irreversible vision loss if not promptly detected and treated, especially in unilateral cases. Nowadays, children with congenital cataracts can undergo surgical removal of their opacified crystalline lenses, and visual rehabilitation is mandatory to prevent deep amblyopia. We aim to describe a technique for intraocular lens (IOL) implantation with suture clamping for aphakic capsules. This prospective, noncomparative, interventional case study included three eyes of three patients with an aphakic capsule after vitrectomy, in which the IOL was clamped by suturing. The suturing procedure was transconjunctival or subconjunctival performed without scleral flaps. The IOLs were implanted without difficulty, and visual acuity improved after IOL implantation.
