Surgical treatment of cardiac involvement in Takayasu arteritis.

Cardiac involvement is a serious disorder in Takayasu arteritis. Surgical treatment of aortic root and coronary artery lesions due to Takayasu arteritis has many potential difficulties due to its inflammatory nature. We operated on 15 patients with cardiac involvement stemming from Takayasu arteritis. These patients are classified into 3 groups depending on the clinical diagnosis and surgical procedures employed: coronary artery involvement alone--coronary artery bypass grafting (CABG), three patients (group A), aortic regurgitation with intact coronary artery--Aortic valve replacement or modi…

Takayasu arteritis in China: a report of 530 cases.

A total of 530 patients with Takayasu arteritis were studied. Among 346 patients who were diagnosed by aortography, the female to male ratio was 2.9 to 1, and the age of onset ranged from 5 to 45 years. Three hundred and eighteen (60%) patients with secondary hypertension, including renovascular hypertension in 281, and 197 (37.2%) with pulseless extremities were found in the series. All the patients were treated with medical or surgical procedures. Surgical treatment is preferred if clinical status of the patient permits. The patients were followed for 1-29 years (average 7.8 years). There we…

Giant cell arteritis in the ocular ischemic syndrome.

Oculomotor nerve paresis, ocular hypotony, anterior segment ischemia, and the ocular ischemic syndrome are uncommon manifestations of giant cell arteritis. Four patients with these findings had giant cell arteritis documented by temporal artery biopsy. Cerebral angiography or ultrasonography, or both, performed in three patients, excluded hemodynamically significant stenosis of the internal carotid artery as the cause of ocular ischemia and cerebral aneurysms as the cause of oculomotor nerve paresis. Corticosteroid treatment, administered to three patients, resulted in resolution of the oculom…

Clinical and laboratory findings with giant cell arteritis.

Out of 66 patients who were diagnosed as suffering from polymyalgia rheumatica (PMR; n = 40), temporal arteritis (AT; n = 14) or both (n = 12) in a 6.5 year period (incidence 3.4/100,000 per year), 9 died and 49 were followed up for an average period of 28 months. Exacerbations of the illness (n = 24) and complications in the course (n = 32) were more frequent with an initial ESR greater than 90 mm/h. Postural vertigo (n = 11), amaurosis fugax (n = 11) and polyneuropathy (n = 8) were the most frequent neurological complications. Persisting unilateral blindness and aromatic anosmia developed in…

Management of severe hypertension in childhood Takayasu's arteritis.

Six children presented with severe hypertension caused by Takayasu's arteritis (TA), of whom four had bilateral renal artery narrowing and two coarctation syndrome. Two presented with hypertensive encephalopathy and four with congestive cardiac failure. All had a strongly positive skin reactions to purified protein derivative of mycobacterium tuberculosis. Bilateral renal arterial bypass grafts performed in two children resulted in prolonged normalization of their blood pressures, but the grafts clotted 12-18 months later. Primary renal autotransplantation was unsuccessful in two children, one…

Relation of giant cell arteritis to polymyalgia rheumatica.

Polymyalgia rheumatica and temporal arteritis appear to be separate syndromes rather than two manifestations of an underlying giant cell arteritis. Polymyalgia rheumatica is a synovitis that may be persistent or recurrent, while temporal arteritis is almost always a single episode; documented recurrences are rare. The two syndromes frequently occur in the same patient although not necessarily at the same time and they may be separated by a long interval. In some patients with polymyalgia rheumatica, giant cell arteritis is found on biopsy of an asymptomatic temporal artery. The frequency of th…

Prognosis of polymyalgia rheumatica and giant cell arteritis.

Polymyalgia rheumatica and giant cell arteritis are amongst the most satisfying conditions for clinicians to diagnose and treat because the unpleasant effects and serious consequences of these conditions can be almost entirely prevented by corticosteroid treatment; the fact that the side-effects of this treatment sometimes seem to be more serious than the complications of the disease is an indication of its effectiveness. Unfortunately, there is no objective way of determining the prognosis in the individual, and decisions concerning duration of treatment remain empirical.

Anticardiolipin and antineutrophil antibodies in giant cell arteritis.

IgG anticardiolipin antibodies (aCL) were found in a patient with Takayasu's arteritis and then in 11 of 22 patients with temporal arteritis (TA) studied prospectively. In comparison IgG aCL were found in only one of 18 elderly controls with rheumatoid arthritis and one of 47 healthy elderly controls. In TA IgG aCL were more frequent in those with acute disease (8/12), in those with an accompanying history of polymyalgia rheumatica (9/13), p less than 0.05); and levels normalized with corticosteroid treatment. Antineutrophil cytoplasmic antibodies (c-ANCA) were present in one patient with TA w…

The peripheral neuropathy of necrotizing arteritis: a clinicopathological study.

We have reviewed the clinical and morphological data from 100 patients with necrotizing arteritis in muscle and/or in nerve samples taken by biopsy. The neuropathy occurred in the context of a multisystem disorder (Group 1) or in apparent isolation (Group 2). The average age of patients was 59 in Group 1 and 61 in Group 2. Females were more commonly affected than males, especially in the first group. Necrotizing arteritis complicated the course of rheumatoid arthritis in 25 patients. In 3 patients necrotizing arteritis was associated with infection with the human immunodeficiency virus, the ag…

Cutaneous manifestations of Takayasu's arteritis. A clinicopathologic correlation.

Takayasu's arteritis is a chronic, granulomatous, large-vessel arteriopathy of unknown cause. We retrospectively reviewed the medical records of 38 patients with Takayasu's arteritis and identified 21 with cutaneous findings. Seven patients had lesions that were related to their systemic vasculitis. We found a Churg-Strauss granuloma, a pyodermatous leg ulcer, and inflammatory leg nodules in these patients. Biopsy specimens from three patients with presumed "erythema nodosum" did not support the clinical diagnosis but did show arteritis. In patients with Takayasu's arteritis, small-vessel infl…

Benign isolated arteritis of the central nervous system.

We present a case of benign isolated arteritis of the CNS. The clinical and angiographic features of this rare disease are discussed and compared with similar cases published in the recent literature. The importance of the angiographic pattern in the diagnosis of this treatable disease is stressed.

[Giant cell arteritis of the uterus. Apropos of a case].

Histopathologic examination can sometimes provide unexpected indications. In this report giant cell arteritis of uterine vessels was found. It permitted to explain biological abnormalities presented by the patient who suffered of a particularly inflammatory syndrome. These abnormalities had been initially attributed to repetitive urinary infections due to third-degree uterine prolapse (associated bladder prolapse). The diagnosis of periarteritis nodosa (P.A.N.) was supported with the results of muscular biopsy. This diagnosis was confirmed by evolution: it showed secondary renal and hepatic da…

[Giant cell arteritis as a cause of tongue necrosis].

A 79 year old woman is reported who has suffered pain in the jaws and mouth for several years. Giant cell arteritis was diagnosed by temporal artery biopsy after the appearance of severe temporal and frontal headache. Despite treatment with cortisone, sudden pain in the tongue appeared preventing speaking and swallowing and leading to ischemic necrosis of the anterior two thirds of the tongue. At the same time she had a transitory ischemic cerebral attack. Six almost identical case histories could be found in the literature: in three tongue necrosis was caused by histologically proven giant ce…

Direct involvement of intracerebral arteries in Takayasu's arteritis.

A man aged 20 with sudden onset of headaches and rapidly evolving spastic quadriparesis died within 3 weeks. Pathologic examination revealed intensive inflammation confined mainly to the adventitia of the aorta, the internal carotids, and all the major intracranial arteries. The gross changes and the lymphocytic and plasma cell nature of the inflammatory infiltrates were typical of Takayasu's arteritis. Thrombosis of the right internal carotid artery resulted in ischemic necrosis of the ipsilateral hemisphere. Quite recent thrombus occluded the left internal carotid artery. This seems to be th…
