Clinicopathologic and genetic features of early- and late-onset FAP type I (FAP ATTR Val30Met) in Japan.

Type I (transthyretin Val30Met) familial amyloid polyneuropathy (FAP ATTR Val30Met) has been reported in relation to two endemic foci in Japan. These cases are characterized by a relatively young age at onset, between the second and third decade, high penetrance rate, concentration in endemic foci, predominant loss of superficial sensation, severe autonomic dysfunction, and atrioventricular nodal block requiring pacemaker implantation. In contrast to these endemic cases, because of advances in DNA diagnosis, late-onset cases of FAP ATTR Val30Met with symptoms appearing at or over 50 years of a

Parasympathetic ganglia innervating the canine atrioventricular nodal region.

Surgical disruption of the small (approximately 0.7 x 1.0 cm) epicardial fat pad situated at the junction of the inferior vena cava (IVC) and inferior surface of the left atrium (ILA) interrupts both right and left vagal input to the atrioventricular nodal (AVN) region of the canine heart. This intervention eliminates AV block during supramaximal stimulation of both cervical vagi, without interfering with sinus bradycardia normally associated with sinoatrial nodal (SAN) suppression. Independent modulation of SAN and AVN activities by the parasympathetic system is thereby revealed. Histology of

Predictors of Long-Term Response to Ganglion Impar Block With or Without Caudal Epidural Steroid Injection in Chronic Coccydynia: A Retrospective Study.

BACKGROUND: Ganglion impar block (GIB) is a commonly used interventional option for refractory chronic coccydynia. Caudal epidural steroid injection (CESI) targets the sacral and coccygeal roots and may theoretically augment the analgesic efficacy of GIB. Evidence comparing GIB alone with a combined GIB and CESI approach, however, remains limited. This study compared long-term pain outcomes after GIB monotherapy versus GIB combined with CESI. METHODS: In this retrospective single-center study (Jan 2020-May 2025), we evaluated 56 adults with chronic coccydynia who underwent GIB alone (Group A)

Efficacy of early use of percutaneous stellate ganglion block for electrical storms.

AIMS: Electrical storm (ES) is a life-threatening condition requiring a rapid management. Percutaneous stellate ganglion block (PSGB) is proved to be safe and effective on top of standard therapy, but no data are available about its early use. METHODS AND RESULTS: We considered all patients enrolled from 1 July 2017 to 30 April 2024 in the STAR registry (STellate ganglion block for Arrhythmic stoRm), a multicentre, international, observational, prospective registry. We aimed to assess the effectiveness of the first PSGB only. Patients were divided into two groups depending on whether they rec

Chronic persistent Horner's syndrome in trigeminal autonomic cephalalgia subtypes and alleviation with treatment: two case reports.

BACKGROUND: The trigeminal autonomic cephalalgias are a group of primary headache syndromes marked by severe head pain and associated cranial autonomic symptoms which can include a full or partial Horner's syndrome. Rarely, the eye-related symptoms will become fixed even between headache attacks. There is minimal documentation that the Horner's syndrome can be reversed if successful treatment of the underlying headache disorder is initiated. CASE REPORTS: Two cases are presented of trigeminal autonomic cephalalgia subtypes with chronic persistent Horner's syndromes that alleviated with treatm

Oximetry-derived perfusion index as an early indicator of CT-guided thoracic sympathetic blockade in palmar hyperhidrosis.

AIM: To test the hypothesis that the oximetry-derived perfusion index (PI) recorded in the index finger may provide earlier objective evidence for correct positioning of the needle tip during computed tomography (CT)-guided thoracic sympathetic blockade than skin temperature in palmar hyperhidrosis. MATERIALS AND METHODS: Forty-four CT-guided thoracic sympathetic blockades were prospectively performed in both hands of 22 patients. Prior to chemical blockade, PI and skin temperature were recorded at 1 min intervals until 20 min after lidocaine injection. Repeated measures analysis of variance

Minimal volume of local anesthetic required for an ultrasound-guided SGB.

BACKGROUND: Compared with the blind technique, ultrasound-guided stellate ganglion block (SGB) reduces the amount of local anesthetic needed for a successful block. The purpose of this study is to determine the minimal, optimal volume of local anesthetic required for successful ultrasound-guided SGB and to reduce its adverse effects. METHODS: Thirty-five patients with postherpetic neuralgia and complex regional pain syndrome of the upper extremity and the facial area were selected. For ultrasound-guided SGB by subfacial method, each patient was injected with 0.5% mepivacaine mixed with contra

Intraoperative hemodynamic measurements of the vertebral artery and common carotid artery.

The hemodynamics in the vertebrobasilar artery (VBA) system were investigated in patients with vertebrobasilar insufficiency (VBI). Vertebral artery (VA) stump pressure and blood flows in the VA and common carotid artery (CCA) were intraoperatively measured in 45 patients who underwent surgical correction of the first segment of the VA (V1) for angiographic tortuosity, kinking, and/or stenosis manifesting as symptomatic VBI. The effects of changes in the systemic arterial blood pressure (SABP) induced by trimethaphan, phenylephrine, and cervical epidural anesthesia were also investigated. The

MRI-guided celiac plexus block.

Celiac plexus block is used as a palliative procedure in cases of severe upper abdominal pain caused by pancreatitis or tumors of the pancreas. It can be guided by bony landmarks, fluoroscopy, ultrasound (US), or computed tomography (CT). To avoid severe complications, methods visualizing soft tissue, like CT and magnetic resonance (MR) imaging, are preferable. We describe celiac plexus blocks carried out in an open MR scanner, offering needle guidance with an optical tracking system and near real-time image acquisition. Eight patients with severe chronic abdominal pain were included. In these

[Anesthetic infiltration of the spermatic cord in surgery for voluminous hydrocele].

The use of a new technique in spermatic cord block in surgical treatment of large hydroceles is reported. Identification of the cord in these cases is often difficult due to the presence of the hydrocele. The reported technique consists in the percutaneous drainage of the hydrocele prior to the block, in order to allow an easier identification of the cord. The block is then performed by the usual method. 108 patients with large hydroceles (above 250 mls) underwent surgical repair employing this approach. In only one case the cord was not identified even after drainage due to the effects of a p

Regional intravenous sympathetic nerve blocks.

The assessment of the possible efficacy of regional intravenous blocks or sympathetic blocks, in general, raises a number of questions that may be answered after studying the techniques of regional pharmacologic segmental blocks and their effects on the sympathetic system. The authors present an original clinical study evaluating the capacity of regional intravenous guanethidine to induce a sympathetic blockade compared with other techniques.

The interpretation of pain relief and sensory changes following sympathetic blockade.

A comparative study of the effects of sympathetic blockade by stellate ganglion block (SGB) and intravenous phentolamine infusion (PhI) was carried out in 24 patients with presumed sympathetically maintained pain of an upper extremity. A total of 15 SGBs and 16 PhIs were performed, with seven patients undergoing both procedures. All patients developed a Horner's syndrome with SGB and nasal stuffiness and cardiovascular changes with PhI. Similar pain relief was obtained with SGB and PhI in six of the seven who underwent both procedures. Pre-procedure patient characteristics including age, sex,

Altered hepatic metabolic response to carbohydrate loads in rats with hepatic branch vagotomy or cholinergic blockade.

Hepatic pyruvate, lactate and glycogen concentrations were measured 40 min after gavage of 5 ml 35% fructose or glucose to rats with sham surgery or section of the hepatic branch of the vagus nerve. Other animals were treated with saline or the peripheral anticholinergic drug, atropine methyl nitrate, prior to gavage of fructose. The increase in pyruvate concentration produced by the carbohydrate loads was attenuated by both hepatic vagotomy and cholinergic blockade. The increase in lactate concentration after a fructose load was attenuated by hepatic branch vagotomy but not atropine injection
