Osteochondromas of the cervical spine in atypical location.

In the current article we review the available English literature by pubmed search engine on the topic of osteochondromas and their location in the cervical spine. The focus is to investigate the location of the lesions in cervical spine and to analyze clinical presentations and symptoms of patients, the aetiology and histopathology examination of the masses. In addition we report a rare illustrative case of a 62 year-old man with an osteochondroma of the cervical spine. The mass developed at a very slow rate for many years and produced no clinical symptoms. The location of the extradural mass

Modification of C1-C2 transarticular screw fixation by image-guided surgery.

STUDY DESIGN: This is a feasibility study of image-guided surgery for C1-C2 transarticular screw fixation comparing postoperative screw position in a nonrandomized prospective cohort with a historic control group in which fluoroscopic guidance was used alone. OBJECTIVES: To evaluate the potential benefits and disadvantages of image-guided surgery for C1-C2 screw placement. SUMMARY OF BACKGROUND DATA: C1-C2 transarticular screw fixation is biomechanically superior to other current surgical stabilization procedures. The original technique for C1-C2 screw placement relies on anatomic landmarks

Posterior atlanto-axial fixation with polyaxial C1 lateral mass screws and C2 pars screws.

PURPOSE: C1-C2 instability or painful osteoarthritis are recognised indications for posterior atlanto-axial fixation. In the traditional trans-articular C1-C2 screw fixation, up to 20% of patients cannot have safe placement of bilateral screws in the event of a medially located vertebral artery and a straight screw trajectory in the sagittal plane. The more recently developed C1-C2 fixation technique with individual C1 lateral mass screws and converging C2 pars screws can be employed in case of a medially located vertebral artery and has comparable biomechanical strength. This is a prospective

RANKL/OPG axis as a therapeutic target for microplastic-induced bone loss: Mechanistic insights from transcriptomic and functional validation.

Although plastic products have offered substantial benefits to modern society and daily life, their degradation into microplastics (MPs) has raised significant concerns owing to their adverse effects on ecosystems and human health. This study investigated MP deposition in human skeletal tissues and elucidated their effects on bone metabolism. Comprehensive analysis of human bone tissue using Nile red staining, Raman spectroscopy, and infrared microspectroscopy identified MP particles in 33 out of 40 samples (covering the cervical, thoracic, and lumbar vertebrae, as well as the upper and lower

Clinical efficacy of soft tissue micro-adjustment combined with traction in pediatric atlantoaxial subluxation: a randomised controlled study protocol using musculoskeletal ultrasound technology.

BACKGROUND: Atlanto-axial rotatory subluxation (AARS) in pediatric patients is characterized by abnormal or restricted motion between the atlas and axis vertebrae, typically presenting with neck pain, limited mobility, torticollis, and muscle stiffness. Cervical atlanto-occipital joint x-ray in open-mouth position or CT scan reveals an atlanto-occipital distance (AOD) of 2 mm < AOD < 5 mm, or a bilateral atlanto-occipital lateral distance (B-LAD) ≥ 2 mm. Although traditional manipulative and bone-setting techniques in Traditional Chinese Medicine have demonstrated clinical benefits, robust emp

How the spine differs in standing and in sitting-important considerations for correction of spinal deformity.

BACKGROUND CONTEXT: The current prevailing school of thought in spinal deformity surgery is to restore sagittal balance with reference to the alignment of the spine when the patient is standing. This strategy, however, likely accounts for increased rates of proximal junctional failure. PURPOSE: The purpose of this study was to investigate the differences between the spine in standing and sitting positions as these may elucidate reasons for deformity correction failure. STUDY DESIGN/SETTING: A prospective, comparative study of 58 healthy patients presenting to a tertiary hospital over a 6-mon

Spontaneous rotatory atlantoaxial dislocation without neurological compromise in a child with Down syndrome: a case report.

Spontaneous atlantoaxial dislocation is a rare recognised complication of Down syndrome. In the majority of cases, dislocation takes place in an anteroposterior direction and is often associated with abnormalities of odontoid development or ossification. Rotatory atlantoaxial dislocation is extremely rare in Down syndrome and this is to our knowledge the first reported case in which modern imaging methods have been described; surface shaded reformats derived from a multislice CT scan were of fundamental importance in making the diagnosis.

Atlanto-axial fusion for instability.

The problem of how best to treat a patient with instability of the atlanto-axial complex is still somewhat controversial. In this follow-up study of fifty-nine patients, nineteen were treated by a plaster jacket and brace; eleven, by single midline wiring and onlay bone grafts; and thirty, by four circumferential wires around the posterior elements of the axis and atlas with two bone grafts wedged between these elements on each side according to the method described by Brooks and Jenkins. (One patient had both types of fusion.) Although direct comparison of the results of treatment in three gr

Changes in Swallowing After Anterior Cervical Discectomy and Fusion With Instrumentation: A Presurgical Versus Postsurgical Videofluoroscopic Comparison.

PURPOSE: The purpose of this study was to explore the impact of anterior cervical discectomy and fusion (ACDF) with anterior instrumentation on swallowing function and physiology as measured on videofluoroscopic swallowing studies. METHOD: We retrospectively analyzed both functional measures (penetration-aspiration, residue) and physiological/anatomical measures (hyoid excursion, posterior pharyngeal wall thickness) in a series of 17 patients (8 men, 9 women, mean age 54 years). These measures were extracted from calibrated 5-ml boluses of thin radio-opaque liquids on both pre-ACDF and post-A

Tumour necrosis factor inhibitor therapy and infection risk in axial spondyloarthritis: results from a longitudinal observational cohort.

OBJECTIVES: Long-term data on infection risk in axial SpA (axSpA) are sparse. TNF inhibitors (TNFis) are increasingly being used in axSpA, with infection being the most important adverse event. We aimed to investigate the frequency of infections in axSpA and to identify factors predisposing to infection. METHODS: Data were extracted from a longitudinal observational cohort of patients with axSpA. Infection rates were calculated and multivariate analysis was performed to investigate the association of independent variables with infection. RESULTS: Data were analysed for 440 patients followed

[Traumatic atlanto-axial rotatory fixation with lateral mass fracture of the axis].

Traumatic atlanto-axial rotatory fixation (AARF) is a relatively uncommon injury, especially in association with fractures of the axis. The rotatory dislocation and fixation is normally caused by intercalated facet joints of axis and atlas. A traumatic AARF in a 21 year old female is presented with special emphasis on the diagnostic and therapeutic approaches. This high velocity injury was caused by a traffic accident. The trauma service which was initially involved made the correct diagnosis and tried to reduce the dislocation by skull traction during analgesia and sedation, but without succe

Validation and use of a finite element model of C-2 for determination of stress and fracture patterns of anterior odontoid loads.

OBJECT: The finite element (FE) method is a powerful tool for the analysis of stress patterns of anatomical structures. In this study a highly refined FE model of C-2 was created and validated. The model was then used to characterize stress patterns, predicted fracture patterns, and transitions between Type II and Type III odontoid fractures. METHODS: An anatomically accurate three-dimensional model of C-2 was created from computerized tomography data obtained from the Visible Human Project. The C-2 model was broken down into an FE mesh consisting of 32,815 elements and 40,969 nodes. For vali
