Preoperative lifestyle intervention in bariatric surgery: a randomized clinical trial.

BACKGROUND: Studies on the impact of presurgery weight loss and lifestyle preparation on outcomes following bariatric surgery are needed. OBJECTIVE: To evaluate whether a presurgery behavioral lifestyle intervention improves weight loss through a 24-month postsurgery period. SETTING: Bariatric Center of Excellence at a large, urban medical center. METHODS: Candidates for bariatric surgery were randomized to a 6-month behavioral lifestyle intervention or to 6 months of usual presurgical care. The lifestyle intervention consisted of 8 weekly face-to-face sessions, followed by 16 weeks of face

Bariatric Surgery Patients' Response to a Chronic Pain Rehabilitation Program.

BACKGROUND: Chronic pain (CP) is a prevalent and disabling diagnosis in obese individuals, but how bariatric surgery patients respond to chronic pain rehabilitation treatment programs has not previously been described. OBJECTIVES: The aim of this study was to compare treatment outcomes of a chronic pain rehabilitation program (psychological and pain variables, medication use, treatment completion rates) for post-bariatric surgery patients to those of a non-bariatric surgery control group. SETTING: Three week outpatient multidisciplinary chronic pain program in an academic medical center. ME

Metabolic effects of bariatric surgery in mouse models of circadian disruption.

BACKGROUND/OBJECTIVES: Mounting evidence supports a link between circadian disruption and metabolic disease. Humans with circadian disruption (for example, night-shift workers) have an increased risk of obesity and cardiometabolic diseases compared with the non-disrupted population. However, it is unclear whether the obesity and obesity-related disorders associated with circadian disruption respond to therapeutic treatments as well as individuals with other types of obesity. SUBJECTS/METHODS: Here, we test the effectiveness of the commonly used bariatric surgical procedure, Vertical Sleeve Ga

[Anastomosis and suture insufficiency after interventions for bariatric and metabolic surgery].

BACKROUND: Metabolic surgery is internationally well-established for the treatment of obesity and its comorbidities. The numbers of procedures performed is steadily increasing. The results of surgery are superior in comparison to conservative treatment options regarding weight loss and resolution of comorbidities. The insufficiency of suture lines is a well-known and feared complication in the stapling procedures and is associated with an increased morbidity as well as mortality, especially in super obese patients. MATERIAL AND METHODS: The current literature was reviewed and the results are

ERCP after bariatric surgery--literature review and case report.

Obesity is a disease which has become more prevalent in Romania. Bariatric surgical procedures are among the treatment options for obese patients. Obesity and the metabolic disorders induced by it are risk factors for gallstones formation and their complications. ERCP is a minimally invasive therapeutic procedure indicated in the treatment of choledochal lithiasis and its complications. ERCP is generally considered the most difficult endoscopic procedure from the technical point of view. The authors have proposed to consider the possibility of performing therapeutic ERCP in patients who have u

The safety and efficacy of bariatric surgery for obese, wheelchair bound patients.

INTRODUCTION: The aim of this study was to evaluate outcomes of bariatric surgery performed in order to improve mobility in patients with severe mobility limitations. METHODS: Patients with severe mobility impairment (wheelchair bound) who underwent laparoscopic Roux-en-Y gastric bypass (LRYGB) or laparoscopic adjustable gastric banding (LAGB) surgery to improve their mobility were included in this study. Patients were identified between July 2009 and October 2011 using an electronic prospective bariatric database. Mobility was assessed by questionnaire during clinic follow-up appointments.

Rapid resolution of hidradenitis suppurativa after bariatric surgical intervention.

We report the case of a 52-year-old man who underwent rapid improvement of long-standing treatment-refractory hidradenitis suppurativa (HS) following bariatric surgical intervention and subsequent dramatic weight loss. This morbidly obese man had previously shown little response to multiple treatment avenues, including an extended course of antibiotic therapy and treatment with acitretin. He had developed marked genital lymphoedema consequential to HS, which had further complicated his HS treatment. However, his disease regressed significantly within weeks of undergoing laparoscopic gastric sl

Medical management of patients after bariatric surgery: Principles and guidelines.

Obesity is a major and growing health care concern. Large epidemiologic studies that evaluated the relationship between obesity and mortality, observed that a higher body-mass index (BMI) is associated with increased rate of death from several causes, among them cardiovascular disease; which is particularly true for those with morbid obesity. Being overweight was also associated with decreased survival in several studies. Unfortunately, obese subjects are often exposed to public disapproval because of their fatness which significantly affects their psychosocial behavior. All obese patients (BM

A holistic assessment of bariatric surgical outcomes in a Northern Irish cohort.

The King's Obesity Staging system was developed to evaluate the effect of obesity treatments in multiple physical, psychological and functional domains. In this prospective cohort study, a Northern Irish cohort was scored using the King's Obesity Staging system before and 1 year after bariatric surgery. 71 individuals underwent surgery and 31 (44%) had type 2 diabetes. Bariatric surgery improved each health domain (p < 0.05). A subgroup with type 2 diabetes showed a significantly greater improvement in gonadal disease (polycystic ovarian syndrome and sub-fertility) (p = 0.02), and a trend towa

Functional assessment of older obese patients candidates for bariatric surgery.

CONTEXT: Obesity in the elderly is associated with exacerbation of functional decline (dependency), that occurs with aging, because of decreased muscle mass and strength, and increased joint dysfunction. Consequently, there is progressive loss of independence, autonomy, chronic pain and impaired quality of life. The weight loss can bring benefits in all these aspects, especially when accompanied by exercises. Elderly patients with morbid obesity may be submitted to surgical treatment, taking into account that the massive weight loss, eventually caused by bariatric surgery, may exacerbate the l

Kidney function in severely obese adolescents undergoing bariatric surgery.

OBJECTIVE: Determine objective measures of kidney function and analyze factors associated with kidney dysfunction in severely obese adolescents undergoing weight loss surgery were described. METHODS: Cross-sectional data from 242 adolescent participants in the Teen-Longitudinal Assessment of Bariatric Surgery (Teen-LABS) study before weight loss surgery were analyzed. Kidney status was assessed by measuring urine albumin creatinine ratio to determine microalbuminuria and by calculating serum cystatin C-based estimated glomerular filtration rate (eGFR) to assess kidney function. RESULTS: Mean

Pulmonary function tests in obese people candidate to bariatric surgery.

Obesity is considered one of the most serious public health problems of the modern world. Because it alters the relationship between the lungs, chest wall and diaphragm, it is to be expected that it impacts on the respiratory function. In Portugal, there is not much data about the relationship between obesity and pulmonary function. The aim of this study was to characterize respiratory function in morbidly obese patients and to evaluate whether weight loss in patients submitted to bariatric surgery affects pulmonary function tests (PFT). We conducted a retrospective study with 36 morbidly obes

Nonalcoholic steatohepatitis in bariatric patients with a diagnosis of obstructive sleep apnea.

OBJECTIVE: To study a possible association between obstructive sleep apnea (OSA) severity, managed with noninvasive ventilation, and nonalcoholic steatohepatitis (NASH) in bariatric surgical patients. METHODS: Medical records of 218 bariatric surgical patients who underwent liver biopsy were reviewed. OSA severity was determined from preoperative polysomnography (apnea-hypopnea index (AHI) ≤ 15 no/mild OSA vs. AHI ≥ 16 moderate/severe OSA). Patients diagnosed with OSA were prescribed noninvasive ventilation. Patients were categorized according to liver histopathology into 3 groups: (i) no liv

Improved renal function 12 months after bariatric surgery.

BACKGROUND: Obesity is a risk factor for developing chronic kidney disease (CKD) that may be improved with bariatric surgical weight reduction. The objective of this study was to investigate changes in the glomerular filtration rate (GFR) in severely obese patients 1 year after bariatric surgery. METHODS: GFR was measured in 233 severely obese patients before and more than 12 months after bariatric surgery. Patients were separated by baseline GFR: hyperfiltration (GFR>125 mL/min), normal (GFR 125-90 mL/min), CKD stage 2 (GFR 89-60 mL/min), and CKD stage 3 (59-30 mL/min). The groups were reana

Bariatric surgery and the kidney-much benefit, but also potential harm.

Bariatric surgery is increasingly performed on overweight individuals. A significant benefit with respect to cardiovascular (CV) events and survival has been documented. After weight loss, reduction of albuminuria/proteinuria is almost consistently seen; small studies documented retardation of the glomerular filtration rate (GFR) loss after bariatric surgery; reduction of blood pressure (BP) is less consistent. It has been known for a long time that the frequency of oxalate stones is increased after bariatric surgery. The main renal threat of hyperoxaluria is renal oxalosis, often irreversible
