Endovascular and Endoscopic Treatment of Hemobilia: A Report of Two Cases.

Hemobilia is a term used to describe bleeding caused by abnormal communication between blood vessels and bile ducts. Some vascular anomalies, such as aneurysms or arterio-biliary fistulas, facilitate the appearance of this type of biliary bleeding. Other causes have been described such as iatrogenic causes secondary to percutaneous procedures, infections, tumors, and trauma. We report two cases of hemobilia. The first one presented with acute biliary bleeding with secondary hypovolemic shock. Bleeding was controlled after percutaneous interventions with a selective embolization technique. The …

Clinical Analysis of C-Shaped Embedded Pancreaticojejunostomy in Pancreaticoduodenectomy.

BACKGROUND: Comparing the effects of C-shaped embedded anastomosis and pancreatic duct-jejunal mucosal anastomosis on the incidence of pancr … ological type, intraoperative blood loss, pancreaticojejunostomy time, combined pancreatic fistula, biliary fistula, hemorrhage, and abdominal infection were observed and compared. According to the different methods of pancreaticojejunostomy during operation, they were divided into group A: C-shaped embedded pancreaticojejunostomy group (38 cases), and group B: p …

Cholecystoappendiceal fistula associated with xanthogranulomatous cholecystitis.

Xanthogranulomatous cholecystitis is a rare chronic inflammatory disease of the gallbladder associated with complications such as perforation, dense adhesions and fistulation. We present a case of a female patient in her 20s who presented with three episodes of recurrent cholecystitis over 3 months. Her medical history included an endoscopic retrograde cholangiopancreatography for obstructive jaundice due to choledocholithiasis. As there was no possibility of performing an urgent cholecystectomy during her initial admission, she was listed for an elective operation. Laparoscopic inspection rev

Bronchobiliary fistula: a rare postoperative complication of spilled gallstones from laparoscopic cholecystectomy.

Bronchobiliary fistula (BBF) is defined as the abnormal connection between the biliary system and the bronchial tree, which presents clinically as an irritant cough with bilioptysis. Many conditions can lead to its development. We present a case of an acquired BBF in a 61-year-old man with a significant history of spilled gallstones from a prior laparoscopic cholecystectomy and subsequent presentation of intermittent right upper quadrant pain and recurrent pneumonia. Imaging studies revealed a liver and subdiaphragmatic abscess with right middle lobe pneumonia and a BBF traversing the right he…

Cholecystocutaneous fistula containing multiple gallstones in a dog.

A 7-year-old dog was presented with a history of an open lesion on the right thoracic wall, discharging honey-like fluid and small stones. Ultrasonography and computed tomographic fistulography identified a cholecystocutaneous fistula; cholecystectomy was curative. Veterinarians should consider this disease in patients with long-term discharging lesions on the right thoracic or abdominal wall.

[Cholecistocolic fistula: an uncommon cause of lower gastrointestinal bleeding].

Cholecystocolic fistula is an uncommon biliary-enteric fistula with a variable clinical presentation that usually appears as a rare complication of gallstone disease. It can present with abdominal pain, nausea, weight loss, diarrhoea with or without associated steatorrhea, and dyspeptic symptoms. Rare cases have been reported with lower gastrointestinal haemorrhage and even with a gallstone ileous. The most useful techniques for diagnosis are CT, barium studies, and ERCP. We report a case of a cholecistocolic fistula in an elderly woman with multiple medical comorbidities that presented as low

Fistulization of hepatic hydatid cyst into duodenum.

Though rupture of hepatic hydatid cyst is not uncommon, fistulization into the gut following rupture is rare. We report a case in whom a hepatic hydatid cyst ruptured into the first part of the duodenum.

Successful preoperative diagnosis of a cholecystogastric fistula using endoscopic retrograde cholangiography: report of a case.

A 64-year-old woman who presented with pyrexia and severe right upper quadrant abdominal pain was suspected of a having a cholecystogastric fistula by the abdominal plain radiograph and gastrointestinal endoscopic findings. The diagnosis was established preoperatively by endoscopic retrograde cholangiography (ERC), following which a cholecystectomy and fistulectomy with resection of the extrahepatic bile duct and hepaticojejunostomy were performed. The patient has remained well since her discharge from hospital.

[Bilio-digestive fistula with an unusual etiology].

On the basis of one case, the authors give the highlights on the various clinicopathological features of malignant gastric perforations. While medical imaging does not seem to contribute much in the positive diagnosis of such complications, it does remain essential to establish the local extension of the tumour process and thus to allow a better therapeutic management.

[Gallstone ileus].

From 1965 to 1985 gallstone ileus was diagnosed and treated in 19 patients. The mean preoperative period of symptoms was five days; the correct diagnosis was made in only half the cases. Enterolithotomy alone has proven to be, in comparison with other methods, a reliable procedure with the fewest postoperative complications. Treatment of bilioenteric fistula was limited to symptomatic cases. Improvement of the results can be expected once the interval from first symptoms to operation is reduced.

Cholecystoduodenal fistula.

An elderly woman with a history of cholelithiasis was seen for right upper quadrant pain and jaundice. During her evaluation, she was found to have extensive choledocholithiasis and a cholecystoduodenal fistula. Successful management was accomplished with cholecystectomy, removal of the common bile duct stones, and by creating a choledochoduodenostomy where the fistula had been present.
