Unveiling the rare: biliary tract ascariasis complicated by liver abscess and haemothorax.

Ascariasis, caused by Ascaris lumbricoides, is a common parasitic infection in developing countries, whereas biliary ascariasis is an uncommon manifestation that may present as cholangitis, biliary obstruction, or cholecystitis. We report the first documented case of biliary ascariasis in a 43-year-old Saudi man presenting with right upper quadrant pain and initially diagnosed with acute cholecystitis. Imaging revealed tubular filling defects within the biliary tree consistent with ascariasis, complicated by a liver abscess. The patient was treated with albendazole, followed by percutaneous dr

Staging laparoscopy with para-aortic lymph node sampling for biliary tract cancer.

BACKGROUND: Biliary tract cancer (BTC) is often diagnosed at an advanced stage. Para-aortic lymph node (PALN) metastasis represents distant disease and contraindicates curative resection. It is frequently occult on preoperative imaging and identified only at laparotomy, resulting in futile surgery. PALN sampling during staging laparoscopy (SL) may therefore be useful. We evaluated the feasibility and clinical impact of SL for BTC. METHODS: This retrospective single-centre study included 23 patients with perihilar or intrahepatic cholangiocarcinoma or gallbladder cancer without distant metasta…

Lipid Metabolism Alterations in Hyperlipidemic Dogs with Biliary Tract or Endocrine Diseases.

Fasting hyperlipidemia results from lipid metabolism defects associated with alterations in specific lipoprotein classes. These changes may originate from genetic predispositions or underlying metabolic disorders, including cholestasis and endocrine diseases. This retrospective study aimed to analyze variations in lipoprotein electrophoresis (LPE) profiles in hyperlipidemic dogs and investigate the associations between biliary tract diseases, endocrine disorders, and lipid metabolism. Sixty-five dogs that underwent LPE were classified into non-hyperlipidemic (NHL) and hyperlipidemic (HL) group…

Sex-related disparities in outcomes of survival in biliary tract cancer patients.

INTRODUCTION: Biliary tract cancer (BTC) is a rare and aggressive cancer with a poor prognosis. Despite treatment, overall survival is less than 12 months. It is a proven fact that women have better chemotherapy responses and survival than men in almost all cancer types. We believe that gender is one of the important factors affecting the prognosis of BTC. In this study, we aimed to investigate the effect of gender on prognosis in this type of cancer. METHODS: This study was designed as a single-centre retrospective analysis of patients with BTC. All patients, regardless of operability, were …

Enhancing biliary tract cancer diagnosis using AI-driven 3D optical diffraction tomography.

Biliary tract cancer is associated with distinct metabolic alterations, particularly in lipid metabolism. This study aimed to classify biliary tract cancer cells automatically based on lipid droplet (LD) characteristics using three-dimensional (3D) optical diffraction tomography (ODT) combined with convolutional neural networks (CNNs). Human biliary tract cancer cell lines (SNU1196, SNU308, and SNU478) and a normal cholangiocyte cell line (H69) were cultured to evaluate the LD volume, mass, and count. We generated 3D refractive index tomograms and developed a CNN-based diagnostic system for au…

Utilizing Patient-Derived Xenografts to Model Precision Oncology for Biliary Tract Cancer.

PURPOSE: Biliary tract cancers, which are rare and aggressive malignancies, are rich in clinically actionable molecular alterations. A major challenge in the field is the paucity of clinically relevant biliary tract cancer models that recapitulate the diverse molecular profiles of these tumors. The purpose of this study was to curate a collection of patient-derived xenograft (PDX) models that reflect the spectrum of genomic alterations present in biliary tract cancers to create a resource for modeling precision oncology. EXPERIMENTAL DESIGN: PDXs were derived from biliary tract cancer samples…

Comprehensive genetic analysis in biliary tract cancer: a prospective single-center experience.

BACKGROUND: Recent data reported a significant incidence of germline aberrations in biliary tract cancer (BTC) patients, even if conclusive data on that and on its clinical implication are lacking. METHODS: We selected patients for genetic counselling basing on four criteria: personal history of oncologic disease other than BTC; familial history of oncologic disease (considering relatives of first and second grade); patients ≤50 years old; patients presenting a somatic mutation in genes involved in DNA damage repair pathways and mismatch repair. RESULTS: A total of 22/150 patients met at lea…

Update for: New developments in systemic therapy for advanced biliary tract cancer.

Biliary tract cancer, carcinoma of the extrahepatic bile ducts, carcinoma of the gallbladder, ampullary carcinoma, and intrahepatic cholangiocarcinoma are often identified at advanced stages. The standard therapy for advanced biliary tract cancer has been a combination of cytotoxic agents. Globally, gemcitabine plus cisplatin has been the standard first-line regimen, whereas gemcitabine plus cisplatin plus S-1 and gemcitabine plus S-1 have also been the standard regimens in Japan. Recently, treatment strategies have been updated. As first-line systemic therapy, the addition of an immune checkp…

Prognostic impact of peritoneal washing cytology in patients with biliary tract cancer.

PURPOSE: To elucidate the clinical significance of peritoneal washing cytology (PWC) in patients with resectable biliary tract cancer (BTC). METHODS: Clinical data of patients with BTC, who received PWC at curative intent surgery from March 2009 to December 2021, were retrospectively analyzed. Eligible patients were stratified into two groups according to positive or negative PWC. Recurrence-free survival and overall survival were compared between the two groups. Independent factors associated with positive PWC were investigated using multivariate analysis. RESULTS: Among the 284 patients an…

Concordance of ctDNA and tissue genomic profiling in advanced biliary tract cancer.

BACKGROUND & AIMS: Recent advances in molecular profiling have enabled the identification of potential therapeutic targets for biliary tract cancer (BTC). However, in patients with BTC, molecular profiling is hindered by challenges in obtaining adequate tissue samples. Circulating tumor DNA (ctDNA) may offer an alternative to tissue-based analysis. Herein, we aimed to assess the concordance between ctDNA and tissue genomic profiling in a large cohort of Asian patients with advanced BTC, and to evaluate the feasibility of liquid biopsy in BTC treatment. METHODS: This study included patients wi…

Fibroblast growth factor therapies in biliary tract cancers: current and future state.

INTRODUCTION: Cholangiocarcinoma is the rare and aggressive tumor with poor prognosis and limited therapeutic options. Recently, there have been promising developments in molecular targeted therapies for patients following the progression of first-line chemotherapy and immunotherapy combinations. Dysregulation of fibroblast Growth Factor Receptor (FGFR) signaling is significantly associated with tumorigenesis of intrahepatic cholangiocarcinoma and has been identified as a targetable alteration. This was possible through the discovery of crucial insights into the biochemical mechanisms and path

Transmission of oral microbiota to the biliary tract during endoscopic retrograde cholangiography.

BACKGROUND: Endoscopic retrograde cholangiography (ERC) possesses a translocation risk of microbes to the biliary system. We studied bile co … me ERC in 33% of all cases and in 45% in the non-cholangitis group. Transmission of microbes to the biliary tract did not result in more frequent cholangitis, longer hospital stays, or worse outcome. CONCLUSIONS: During ERC bile samples are regularly contaminated with microbes of the oral cavity but it did not affect clinical outcome.

Descriptive epidemiology of biliary tract cancer incidence and geographic variation in Japan.

OBJECTIVE: The epidemiologic features of each biliary tract cancer (BTC) subtype have not been studied and disclosed in detail. The objective of this study was to provide an up-to-date description of the epidemiologic features of BTC by subtypes, especially in terms of the geographic variation of its incidence. METHODS: We calculated the age-standardized incidence rate (ASR) of each BTC subtype at national and prefectural levels using the data from the national cancer registry in 2016 and 2017. The geographic distribution of each BTC subtype incidence was assessed by calculating the ASR ratio…

Establishment of Patient-Derived Organoids and Drug Screening for Biliary Tract Carcinoma.

Biliary tract carcinomas (BTCs) are among the most aggressive malignancies and have a poor prognosis. Here, we successfully established organoid lines derived from intrahepatic cholangiocarcinoma, gallbladder cancer, and neuroendocrine carcinoma of the ampulla of Vater. These organoids derived from BTCs were cultured stably for >1 year and closely recapitulated the histopathology, gene expression, and genetic alterations evident in the primary tumors. Gene expression profiling of the organoids revealed that SOX2 could be a potential prognostic biomarker for patients with BTC. We screened a com…
