Midfacial advancement by bone distraction for treatment of craniofacial deformities.

Craniofaciostenosis is often associated with midfacial hypoplasia and has been treated traditionally using Le Fort advancement osteotomies and bone grafts. The surgical procedure requires a prolonged operating time, several osteotomies with a significant blood loss, and wide surgical exposure. According to the principles of bone lengthening, we performed midfacial advancement by bone distraction in 4 patients with midfacial hypoplasia to reduce the operative time and complication rate. In 2 patients with Crouzon's syndrome we performed a Le Fort III osteotomy and placed the distraction device …

Use of non-vascularized autologous fibula strut graft in the treatment of segmental bone loss.

BACKGROUND: Fractures resulting in segmental bone loss challenge the orthopedic surgeon. Orthopedic surgeons in developed countries have the option of choosing vascularized bone transfers, bone transport, allogenic bone grafts, bone graft substitutes and several other means to treat such conditions. In developing countries where such facilities or expertise may not be readily available, the surgeon has to rely on other techniques of treatment. Non-vascularized fibula strut graft and cancellous bone grafting provides a reliable means of treating such conditions in developing countries. MATERIA

Oblique osteotomy for the correction of tibial malunion.

Fifteen patients had an oblique osteotomy of the tibia for the correction of a multiplanar deformity between January 1989 and March 1991; twelve were followed for an average of twenty-five months (range, twelve to forty-two months). Preoperatively, the average deformity in the coronal plane was 14 degrees (range, 30 degrees of valgus to 25 degrees of varus) and the average deformity in the sagittal plane was 13 degrees (range, 40 degrees of recurvatum to 23 degrees of procurvatum [angulation convex anteriorly]). The average leg-length discrepancy was 2.2 centimeters (range, one to six centimet

Effect of asymmetrical configuration of pins in the TSF external fixator used for tibial lengthening in a pediatric population.

BACKGROUND: The Taylor Spatial Frame (TSF) is a relatively new circular external fixator linked with a computer program used in limb reconstruction surgery. It is routinely used in our unit (Royal Manchester Children's Hospital) for pediatric tibial lengthening. METHODS: The case notes and radiographs of 17 children who underwent 18 tibial lengthenings were reviewed. The TSF was attached to the tibia using a hybrid fixation method in the safe zone with two 1.8 mm tensioned smooth wires and 2 anteriorly placed hydroxyapatite-coated pins. Mechanical compression testing of our TSF construct was

Proximal toe phalanx transplantation for bony stabilization and lengthening of partially aplastic digits.

In congenital malformations the lengthening of partially aplastic digits, by on-top plasty of a bone graft or the bony stabilization of finger stumps consisting of soft tissues without skeletal elements, is extremely difficult. The usual bone graft, taken from the iliac crest or the fibula, will undergo resorption and will have disappeared a few months postoperatively. Experience has shown that the proximal toe phalanx will with-stand any resorption if it is taken whole, covered by its periosteum. Another advantage is the possibility of the construction of a new joint between the metatarsophal

Treatment of sequestra, pseudarthroses, and defects in the long bones of children who have chronic hematogenous osteomyelitis.

We reviewed the results, after follow-up ranging from twenty-three months to six years, in thirty-four patients who were treated during childhood for hematogenous osteomyelitis of a major long bone complicated by sequestration of a portion of the diaphysis and by pseudarthrosis or segmental bone loss, or both. Of the thirty-four lesions, twenty-four were in the tibia, eight were in the femur, and two were in the humerus. In twenty-three patients (Group I), the infection was still active, while in the other eleven (Group II), it was quiescent at the time of admission to the hospital. In nine of

External skeletal fixation. Secondary indications.

In addition to open tibial fractures, external fixation has many other applications to the leg. By keeping the method in mind, the surgeon will find the occasional application in which external fixation may serve as a convenient, safe, and comfortable alternative method of treatment.

Trochanteric Entry for Femoral Lengthening Nails in Children: Is It Safe?

BACKGROUND: Antegrade intramedullary (IM) nailing for skeletally immature femur fractures can damage the capital femoral epiphysis blood supply, leading to avascular necrosis (AVN) of the femoral head. Reported AVN rates are 2% for piriformis entry and 1.4% for trochanteric entry. None of previous reports described IM lengthening nails for limb lengthening procedures. We have used self-lengthening telescopic nails with a proximal Herzog bend and standard trochanteric entry for femoral lengthening in children. The purpose of this study is to determine whether trochanteric entry IM lengthening n

Clinical implication of sagittal translation in lengthening over nail of tibia.

PURPOSE: During the surgery of lengthening over nail (LON), we sometimes observe sagittal translation of proximal tibia (STPT) when the nail is inserted into the canal. We investigated the factors that lead to STPT and its clinical significance. METHODS: We reviewed 35 consecutive patients who underwent bilateral tibial lengthening with LON. The following parameters were assessed to evaluate the factors that can lead to STPT; the level of tibia osteotomy (OT) in sagittal plane, the type of the nail, the nail entry point in sagittal/coronal planes, and the type of the OT. Then, the followings

The role of distraction osteogenesis in the reconstruction of the mandible in unilateral craniofacial microsomia.

Laboratory and clinical studies have shown that mandibular distraction is an effective and powerful reconstructive surgical technique. It is safely performed without the need for blood transfusion or bone graft. Because of concomitant expansion of the functional matrix (the associated skin, subcutaneous fat, and muscles of mastication), there is a multidimensional expansion of the lower jaw with minimal, if any, evidence of relapse. Moreover, the surgical reconstruction can be commenced by 2 years of age, and the vector of distraction can be controlled by preoperative design. Finally, the incr

Clinical variation in dyschondrosteosis. A report on 13 individuals in 8 families.

Thirteen patients with dyschondrosteosis from eight families are reviewed and their clinical and radiographic variation noted. Inheritance is likely to be autosomal dominant but with only 50 per cent penetrance. Stature was moderately reduced, due to shortening of the bones of the leg. Radio-ulnar shortening could either involve both bones equally or the radius predominantly, in which case a typical Madelung deformity was seen. Tibio-fibular disproportion was present in half the patients, two of them having severe deformity associated with tibia varum and a long fibula. The treatment of one of

Metabolic regulation of skeletal cell fate and function.

Bone development and bone remodelling during adult life are highly anabolic processes requiring an adequate supply of oxygen and nutrients. Bone-forming osteoblasts and bone-resorbing osteoclasts interact closely to preserve bone mass and architecture and are often located close to blood vessels. Chondrocytes within the developing growth plate ensure that bone lengthening occurs before puberty, but these cells function in an avascular environment. With ageing, numerous bone marrow adipocytes appear, often with negative effects on bone properties. Many studies have now indicated that skeletal c…

Horizontal anchorage in the ascending ramus--a technical note.

The ascending ramus provides a volume of bone that can receive horizontal screws of large sizes in the retromolar area, between the internal and external oblique lines. This area is not prone to disuse resorption and the only vital structure to consider is the neurovascular bundle. In this article two systems that make use of this anatomic opportunity are presented. One is an intraoral distractor, used for mandibular lengthening by gradual distraction. The other is a screw anchor that provides absolute anchorage for orthodontic tooth movement.

Diagnostic and treatment challenge in adult presentation of congenital pseudoarthrosis of the tibia: A case report.

INTRODUCTION: Congenital pseudoarthrosis of the tibia is a rare congenital disease. Late presentation in adult, makes the diagnostic far more challenging and often misdiagnosed as a common non-union fracture with high re-operation rate. In long courses of repeated surgery, non-union persisted along with severe leg length discrepancy. CASE PRESENTATION: A 19-year-old male presented with history of left tibia fracture with repeated surgery. Current problems were progressed bowing of the left lower leg and length discrepancy without recent injury. There was a sign of neurofibroma and pseudoarthr

Evaluation of the extent of bone healing in mandibular distraction osteogenesis using pre-operative and post-operative imaging pixel value ratio: a cohort study.

BACKGROUND: The pixel value ratio (PVR) is a metric measured on X-ray images that has been used in many studies to assess the time of final external fixator removal. The aim of this study was to investigate the value and potential influencing factors of PVR in the early phase of mandible healing by distraction osteogenesis (DO) in Pierre Robin sequence. METHODS: We performed a retrospective analysis on 342 Pierre Robin sequence patients with bilateral mandibular lengthening surgery, measured PVRs in the osteotomy position of children before surgery, and monitored PVRs in the early stage of bo

Short-term outcomes of gradual ulnar lengthening in multiple hereditary osteochondromas of the forearm with monolateral external fixator.

Hereditary multiple exostoses is an autosomal dominant genetic condition primarily affecting long bones. Forearm deformities, including wrist ulnar deviation, ulnar shortening, radial or ulnar bowing, and radial head dislocation, are common manifestations. Gradual ulnar lengthening is suggested as a viable treatment option for managing these deformities. This retrospective single-center study included 15 forearms treated with gradual ulnar lengthening using a monolateral external fixator by a single surgeon. Clinical and radiological characteristics were recorded preoperatively and during foll

Sciatic palsy after total hip arthroplasty associated with vascular graft occlusion.

Sciatic nerve palsy is a well-recognised complication of total hip arthroplasty, and causes include direct injury during surgery (crushing or electrocautery), compression or stretching of the nerve, thermal damage caused by leaked bone cement, trauma during dislocation or reduction of the hip, haematoma, traction caused by leg lengthening or inadvertent intraneural injection from nerve blocks. We describe what we believe to be a case of sciatic nerve ischemia due to intra-operative arterial occlusion, and we discuss the vascular anatomy which may have contributed.

[Hallux Extensus as a Rare Complication of Crural Fracture and Its Surgical Treatment].

Hallux extensus is a persistent dorsiflexion of the first metatarsophalangeal joint. Apart from many other causes, it can also be caused by contracture of the extensor hallucis longus muscle, in our case resulting from surgical treatment of crural fracture. The authors present the case of a 17-year-old soccer player who sustained a fracture of both shin and calf bone during a soccer match and underwent surgical treatment with intramedullary nailing. While compartment syndrome wasn't observed, a postoperative complication in the form of progressive development of a dynamic type of hallux extens
