The placement of a headless screw within the scaphoid is important for successful fixation of fractured scaphoids. We ascertained the safe position of a screw using virtual collision experiments on a three-dimensionally reconstructed scaphoid bone and screw model, and applied measured angles to simple radiographs. We used images from computed tomograms of the wrists taken from 13 men to reconstruct a three-dimensional cortical image of the scaphoid. Inside the bone, we inserted a virtually reconstructed Herbert screw model and calculated the safe angle along the long axis of the scaphoid at wh

BACKGROUND CONTEXT: Spinal deformity surgery in elderly patients is associated with an increased risk of implant loosening due to failure at the screw-bone interface. Several techniques can be used to increase the screw anchorage characteristics. Cement-augmented screw fixation was shown to be the most efficient method; however, this technique is associated with a risk of complications related to vertebral cement deposition and leakage. Hence, there is a need to further elaborate the alternative screw augmenting techniques to reduce the indications for bone cement.
PURPOSE: To analyze surgica

BACKGROUND: The atlantoaxial vertebral model was established in order to compare the biomechanical properties of C2 pedicle and translaminar screws from the perspective of the screws themselves.
METHODS: A finite element model of the screw-vertebrae was developed. The screw load-displacement ratios were analysed under up/down and left/right load conditions; the vertebral load-displacement ratios under flexion/posterior extension (FLX/EXT), left/right lateral bending (LLB/RLB), and left/right rotation (LAR/RAR) load conditions; the bone-screw interface stress values and screw load-displacement

PURPOSE: Maxillomandibular fixation (MMF) can be performed using various techniques. Two common approaches used are arch bars and bone screws. Arch bars are the gold standard and inexpensive, but often require increased procedure time. Bone screws with wire fixation is a popular alternative, but more expensive than arch bars. The differences in costs of care, complications, and operative times between these 2 techniques are analyzed.
METHODS: A chart review was conducted on patients treated over the last 12 years at our institution. Forty-four patients with CPT code 21453 (closed reduction of…

Plate osteosynthesis is one treatment option for the stabilization of long bones. It is widely accepted to achieve bone healing with a dynamic and biological fixation where the perfusion of the bone is left intact and micromotion at the fracture gap is allowed. The indications for a dynamic plate osteosynthesis include distal tibial and femoral fractures, some midshaft fractures, and adolescent tibial and femoral fractures with not fully closed growth plates. Although many lower limb shaft fractures are managed successfully with intramedullary nails, there are some important advantages of open

Displaced medial malleolus fractures are considered unstable and typically require open reduction and internal fixation for anatomic reduction and early joint range of motion. These fractures are usually fixated with either compression lag screws or tension band wiring depending on the fracture pattern, size of the distal fragment, and bone quality. When fracture fixation fails, it is typically in pullout strength. Failure of primary bone healing can result in nonunion, malunion, and need for revision surgery. The current study wished to explore a potentially stronger fixation technique in reg

PURPOSE: Posterior reduction and pedicle screw fixation is a widely used procedure for thoracic and lumbar vertebrae fractures. Usually, the pedicle screws would be removed after the fracture healing and screw tunnels would be left. The aim of this study is to evaluate the effect of screw tunnels on the biomechanical stability of the lumbar vertebral body after pedicle screws removal by finite element analysis (FEA).
METHODS: First, the CT values of the screw tunnels wall in the fractured vertebral bodies were measured in patients whose pedicle screws were removed, and they were then compared

Screw fixation of the tibial component offers advantages in fixation in cementless total knee arthroplasty (TKA). In a group of 447 cementless TKA tibial components fixed without screws, 24% had partial undersurface radiolucent lines and 3% had complete radiolucent lines. Both of these parameters were significantly decreased (p < 0.01) when screws were used. In a group of 1442 cementless tibial trays fixed with screws, 4% had partial and 0.1% had complete radiolucent lines. Cyst formation around smooth pegs was also more common in trays fixed without screws. Two of the 5758 screws inserted in

The surgical fracture fixation of the odontoid process (dens) of the second cervical vertebra (C2/axis) is a challenging procedure, particularly in elderly patients affected by bone loss, and includes screw positioning close to vital structures. The aim of this study was to provide an extended anatomical knowledge of C2, the bone mass distribution and bone loss, and to understand the implications for anterior screw fixation. One hundred and twenty standard clinical quantitative computed tomography (QCT) scans of the intact cervical spine from 60 female and 60 male European patients, aged 18-90

The utilization of a plate-screw system is a prevalent osteosynthesis technique widely employed for the fixation of mandibular fractures. While comparable findings exist regarding the impact of using locking and non-locking screws on the mandibular bone, conflicting results have been reported concerning their effect on stress distribution in the plate and screws. This study investigates the influence of locking and non-locking plate-screw systems on mandibular fracture fixation using finite element analysis, incorporating a 3D scanned jawbone geometry. Results indicate that locking screws exhi

BACKGROUND: The bicortical or tricortical fixation technique with purchase into the anterior sacral wall or promontory has been recommended to achieve rigid sacral pedicle screw fixation, which carries the potential risk of neurovascular injuries. The penetrating endplate screw (PES) technique was proposed as an alternative screw trajectory to facilitate both strong fixation and safety. However, there has been no report on the practical significance of using the PES technique. The aim of the present study was to investigate radiological outcomes using the PES technique for lumbosacral fusion b

Shortcomings of fixation have been reported as a source of graft failure in anterior cruciate ligament (ACL) reconstruction. While interference screws have long been used as fixation devices for ACL reconstruction, they are not without complications. Previous studies have highlighted the use of bone void filler as a fixation method; however, no biomechanical comparisons using soft tissue grafts with interference screws exist to our knowledge. The purpose of this study is to evaluate the fixation strength of a calcium phosphate cement bone void filler compared with screw fixation in an ACL reco

Fixation techniques of periprosthetic fractures are far from ideal although the number of this entity is rising. The presence of an intramedullary implant generates its own fracture characteristics since stiffness is altered along the bone shaft and certain implant combinations affect load resistance of the bone. Influencing factors are cement fixation of the implant, intramedullary locking and extramedullary or intramedullary localization of the implant and the cortical thickness of the surrounding bone. Cerclage wires are ideally suited to fix radially displaced fragments around an intramedu

Study DesignExperimental ex vivo biomechanical cadaver study.ObjectivesThoracic pedicle screws are essential for stabilizing spinal injuries, but misplacement remains a frequent complication. While many studies focus on neurovascular injuries, the impact of the degree of lateral misplacement (offset) on the biomechanical stability of thoracic pedicle screws has not been examined. Therefore, this study aimed to evaluate the influence of lateral offset on pullout force (POF) and peak insertion torque (PIT), and to analyse additional factors such as screw diameter, screw length, and bone quality.

STUDY DESIGN: Computer simulation of adolescent idiopathic scoliosis instrumentation.
OBJECTIVE: To test the hypothesis that different screw densities would result in different apical vertebral rotation (AVR) corrections and bone-screw forces in adolescent idiopathic scoliosis instrumentation.
SUMMARY OF BACKGROUND DATA: The "Minimize Implants Maximize Outcomes" Clinical Trial revealed that the use of more versus fewer screws resulted in similar coronal plane correction for Lenke 1A curves. However, the biomechanical impact of screw density on transverse plane correction is still unclear. Fu

The compressive force generated by a 3.5 mm ASIF cannulated cancellous screw with a 5 mm head was compared with that generated by a standard 3.5 mm ASIF screw (6 mm head), a 2.7 mm ASIF screw (5 mm head), and a Herbert screw. The screws were evaluated in the laboratory with the use of a custom-designed load washer (transducer) to the maximum compressive force generated by each screw until failure, either by thread stripping or by head migration into the specimen. Testing was done on paired cadaver scaphoids. To minimize the variability that occurs with human bone, and because of the cost and d
