[Bronchiolitis. Our clinical experience in the 4 years from 1981 to 1984].

The clinical experience of 661 children with bronchiolitis is reported in four-years period to gain a better understanding of diagnosis and pathogenesis of bronchiolitis. Upper airways infections, expiratory dyspnea, clear sound by chest percussion, vesicular rales and whistling by chest auscultation, air trapping on the chest radiography were considered as essential data of diagnosis. It was found in 595 patients: expiratory dyspnea, air trapping, vesicular r. and whistling in 85% and whistling only in 15%; hypoxemia in 20% combined with hypercapnic acidosis in 10%; normoxemia in 80% combined…

Continuing respiratory problems three and a half years after acute viral bronchiolitis.

We reviewed the clinical progress of 81 children as part of a prospective study three and a half years after admission to hospital with acute viral bronchiolitis in infancy. Fifty six (69%) reported episodes of lower respiratory symptoms continuing over the preceding year, 25 (31%) had symptoms lasting for longer than two weeks on two or more occasions, 14 (17%) had symptoms for more than 100 days, and six (7%) required readmission to hospital with acute respiratory illness. Two years previously, these percentages had been 82%, 36%, 33%, and 13% respectively. Forty six (57%) children were said…

Atopy predisposing to acute bronchiolitis during an epidemic of respiratory syncytial virus.

Thirty-one infants admitted to hospital with acute bronchiolitis during an epidemic of respiratory syncytial virus were compared with a control group of 32 infants to establish whether the two groups differed in atopic background. Past history of respiratory illness, eczema, and present reactions to skin testing differed significantly between the two groups. Thus, infants with acute bronchiolitis had a significantly higher atopic predisposition than the controls.

Slowly progressive bronchiolitis obliterans. A case report with detailed pulmonary function studies.

A case of idiopathic bronchiolitis obliterans in a 40-year-old man is presented. The chest radiograph showed miliary nodules, which on histological examination were found to consist of granulation tissue in and around bronchioles causing narrowing or obliteration of the airway lumen. The pulmonary function tests indicated a milk degree of airway obstruction, ventilation/perfusion inequality, hypoxemia which increased during exercise, and a well-preserved diffusion capacity suggesting absence of significant damage to the interstitium of the lung. Radiospirometry suggested that the functional de…

Standardizing HFNC for bronchiolitis: a quality improvement protocol in an Italian children's hospital.

UNLABELLED: Bronchiolitis is the leading cause of non-elective hospitalizations in infants, with an increasing use of high-flow nasal cannula (HFNC) therapy outside the Pediatric Intensive Care Unit (PICU). At our hospital, HFNC was used to treat pediatric bronchiolitis patients without a standardized protocol for initiation, flow titration, escalation, or a PICU alert system in cases of clinical deterioration. This quality improvement project aimed to evaluate the feasibility of a multidisciplinary protocol designed to standardize HFNC use for bronchiolitis in patients under 24 months of age,…

Intravenous Terbutaline as Rescue Therapy in Severe Bronchiolitis: A Report of Two Cases.

Bronchiolitis is a lower respiratory tract infection most frequently caused by viral infections and is usually managed with supportive care. The role of bronchodilators particularly intravenous (IV) β2-agonists remain uncertain. Emerging evidence suggests that bronchiolitis comprises heterogeneous phenotypes, with a subset demonstrating bronchodilator responsiveness. We report two infants (ten and three months old) with RSV confirmed severe bronchiolitis who exhibited respiratory distress and hypoxemia despite maximal supportive therapy, including oxygen, continuous positive airway pressure (C…

Lung transplantation in a patient with severe bronchiolitis obliterans secondary to Castleman disease.

Castlemann disease (CD) is a rare but often benign condition of the lymph nodes. Seldomly coinciding with CD are other conditions, such as paraneoplastic phemhigus (PMP) and bronchiolitis obliterans (BO). When these occur, patients are at risk of developing severe complications, such as respiratory failure, which may subsequently result in death. Diagnosing and managing conditions related to CD poses a challenge and may cause diagnostic delay. In this case report, we describe the course of illness in a 31-year-old female diagnosed with CD. Prior to the diagnosis of CD, the patient had been dia…

The severity of the first occurrence of bronchiolitis increased the risk of developing asthma symptoms.

AIM: The relationship between bronchiolitis and asthma is complex. We assessed whether patients admitted to a paediatric intensive care unit (PICU) with bronchiolitis had a greater risk of developing asthma than patients admitted to a paediatric ward. METHODS: We retrospectively included children under 1 year of age, who were hospitalised for bronchiolitis for the first time at the University Hospital of Caen, France, between 2010 and 2014. The children were divided into two groups: 89 were admitted to the paediatric ward and 89 were admitted to the PICU. We wanted to assess which group devel…

The Role of Pulmonary Genetic Variations in the Pathogenesis of Pediatric Postinfectious Bronchiolitis Obliterans.

OBJECTIVE: Postinfectious bronchiolitis obliterans (PIBO) is a chronic airway disease. The severity of the damage and the subsequent obstructive and inflammatory processes varies from one individual to another. The objective was to identify genetic variations that may be associated with pulmonary diseases in patients with PIBO. MATERIAL AND METHODS: This retrospective descriptive study was carried out to define potential genetic changes that may be associated with PIBO. Medical records were used to obtain sociodemographic characteristics. Neutrophil, lymphocyte, platelet counts, immunoglobuli…

Proximal and Distal Bronchioles Contribute to the Pathogenesis of Non-Cystic Fibrosis Bronchiectasis.

Rationale: Non-cystic fibrosis bronchiectasis (NCFB) may originate in bronchiolar regions of the lung. Accordingly, there is a need to characterize the morphology and molecular characteristics of NCFB bronchioles. Objectives: Test the hypothesis that NCFB exhibits a major component of bronchiolar disease manifest by mucus plugging and ectasia. Methods: Morphologic criteria and region-specific epithelial gene expression, measured histologically and by RNA in situ hybridization and immunohistochemistry, identified proximal and distal bronchioles in excised NCFB lungs. RNA in situ hybridization a

The Use of Evaluation Methods for Physiotherapy Treatment in Infants With Bronchiolitis-a Survey Study.

AIMS: The aim of this study was to identify and describe methods that physiotherapists use to evaluate the immediate effects of their interventions for infants in hospital with acute respiratory tract infections. METHODS: We constructed an anonymous digital survey to physiotherapists working with infants aged 0-24 months in Sweden. The survey was distributed by e-mail and posted on web pages for seven weeks, which included a reminder. RESULTS: Replies were obtained from 88 respondents, and 52 completed surveys remained to be analyzed. All 21 counties in Sweden were represented. The most prev

Management of Acute Bronchiolitis in Spoke Hospitals in Northern Italy: Analysis and Outcome.

Bronchiolitis is an acute viral infection of the lower respiratory tract that affects infants and young children. Respiratory syncytial virus (RSV) is the most common causative agent; however, other viruses can be involved in this disease. We retrospectively reviewed the clinical features of infants aged less than 12 months hospitalized for acute bronchiolitis in our Pediatric Units of Chivasso, Cirié, and Ivrea in Piedmont, Northern Italy, over two consecutive bronchiolitis seasons (September 2021-March 2022 and September 2022-March 2023). Patient-, disease-, and treatment-related variables w…

New paradigms in acute viral bronchiolitis: Is it time to change our approach?

Viral bronchiolitis is the most common pediatric acute respiratory infection leading to hospitalization, and it causes a significant healthcare burden worldwide. Current guidelines recommend supportive management after many clinical trials on specific therapies failed to demonstrate benefits. However, several studies in the past decade have revealed that bronchiolitis may not be a homogeneous disease, but instead may constitute an umbrella comprised of different "endotypes" and "phenotypes" based on patient characteristics, etiology, pathophysiological mechanisms, and clinical presentation. In…

Probabilistic Linkage Creates a Novel Database to Study Bronchiolitis Care in the PICU.

OBJECTIVES: Lack of a comprehensive database containing diagnosis, patient and clinical characteristics, diagnostics, treatments, and outcomes limits needed comparative effectiveness research (CER) to improve care in the PICU. Combined, the Pediatric Hospital Information System (PHIS) and Virtual Pediatric Systems (VPS) databases contain the needed data for CER, but limits on the use of patient identifiers have thus far prevented linkage of these databases with traditional linkage methods. Focusing on the subgroup of patients with bronchiolitis, we aim to show that probabilistic linkage method…

Mucin 4 is a cellular biomarker of necrotizing bronchiolitis in influenza A virus infection.

Influenza A virus (IAV) in the human and swine host infects epithelial cells lining the respiratory tract causing a necrotizing bronchitis and bronchiolitis. These epithelial surfaces are protected by large glycoproteins called mucins. Mucin 4 (MUC4) is a transmembrane mucin that consists of an alpha subunit responsible for surface protection and intracellular beta subunit involved in signal transduction which repress apoptosis and stimulate epithelial proliferation. This study was designed to determine the expression and potential role of MUC4 during IAV infection. We used immunohistochemistr…

Nasal CPAP in the Pediatric Ward to Reduce PICU Admissions for Severe Bronchiolitis?

In treating acute bronchiolitis in infants, the decision to use continuous positive airway pressure (CPAP) often involves infant referral from the pediatric ward to the pediatric intensive care unit (PICU). We present our experience of CPAP use in a general pediatric ward, aiming to reduce the pressure on the PICU in recent outbreaks of bronchiolitis. Clinical data of patients less than 12 months of age and admitted for bronchiolitis from 1 October 2021 to 31 March 2023 were retrospectively collected. Of 82 infants admitted for bronchiolitis, 16 (19%) were treated with nasal CPAP (nCPAP group)…
