Comparison of continuous subcutaneous and intravenous hydromorphone infusions for management of cancer pain.

To compare the safety and efficacy of subcutaneous and intravenous infusion of opioid analgesics, a randomised, double-blind, crossover trial was carried out in inpatients. 15 patients with severe cancer pain received two 48 h infusions of hydromorphone--one subcutaneously and one intravenously in randomly allocated order. The study was made double-blind by the use of two infusion pumps throughout; during the active subcutaneous infusion the intravenous pump delivered saline and vice versa. Serial measurements of pain intensity, pain relief, mood, and sedation by means of visual analogue scale…

[Prevention by naloxone of adverse effects of epidural morphine analgesia for cancer pain].

Forty cancer patients were randomly assigned to two groups (n = 20). All had incapacitating pain unresponsive to the usual non opioid analgesic drugs. An epidural catheter was set up at the level of the most painful metamere, and made to pass subcutaneously so as to exit either in the supraclacicular fossa, or on the patient's flank. At T0, the patients were given 4 mg morphine hydrochloride diluted in 10 ml normal saline. Thirty min later, patients in the naloxone group (group N) were given a 0.4 mg bolus, followed by a constant rate infusion of 5 micrograms.kg-1.h-1, of naloxone hydrochlorid

Treatment of diffuse metastatic cancer pain by instillation of alcohol into the sella turcica.

The technique of "chemical hypophysectomy" was modified for the management of pain due to metastatic cancer. Using stereotaxic control, a needle is introduced via the nose into the sella turcica. Absolute alcohol is then injected into the pituitary. Of 13 patients who had severe uncontrollable pain, 11 obtained marked symptomatic relief. The longest follow-up period to date is seven months, with results persisting. Sequelae are those associated with destruction of the pituitary gland, the most significant being diabetes insipidus. Several cerebrospinal fluid leaks prompted us routinely to inst

"A lot of bureaucracy": Structural barriers to prescription opioids in cancer pain management.

Well-intentioned policies to reduce opioid prescribing can conflict with efforts to manage advanced cancer pain, resulting in structural barriers to opioid pain management that complicate the delivery of guideline-concordant care. This study examined how patients with advanced cancer and their support people perceive barriers to opioid access, how those barriers affect their cancer pain management, and the extent to which they recognize and attribute them to policy guidance. Results from an analysis of in-depth qualitative interviews with 20 patients with advanced cancer and 11 support people,…

Nonpharmacologic Approaches for Pain Management in Patients With Cancer Treated With Radiation Therapy.

Radiation therapy (RT) is central to cancer treatment, yet pain from RT-related toxicities remains common and often inadequately controlled with pharmacologic care alone. Integrative medicine modalities are increasingly used to complement standard analgesic strategies, but their role in RT-related pain has not been comprehensively synthesized. In this narrative review, we summarize clinical evidence for nonpharmacologic approaches to RT-related pain, including acupuncture, massage, hypnosis, yoga, and exercise. We further include strategies for incorporating these modalities across the RT care

Macrophages as central mediators of perineural invasion in cancer: Implications for pain and prognosis.

Perineural invasion (PNI) is associated with tumor progression, therapeutic resistance, and poor prognosis. However, the mechanisms driving tumor-nerve interactions remain incompletely understood. Emerging evidence suggests that macrophages function as key regulators within the perineural niche by linking inflammation, neural remodeling, extracellular matrix reorganization, and immunometabolic signaling. This review proposes a macrophage-centered framework for understanding PNI progression and summarizes macrophage-mediated mechanisms involved in nerve invasion, perineural homeostasis disrupti

Arts in Medicine for the Reduction of Pain and Stress in Cancer Patients During Chemotherapy.

CONTEXT: Rates of pain and stress are found in greater numbers in cancer patients than in the general population. Cancer patients often are concerned about unwanted side effects that can arise from taking medications to lower their pain and stress. As doctors are reaching for evidence-based, nonpharmaceutical, adjunct modalities, previous research indicates that visual art-making shows promise to help improve perceptions of pain and stress caused by a cancer diagnosis and treatment. OBJECTIVES: This mixed-methods pilot study was conducted at the Orlando Health Cancer Institute and included 54

Bilateral Cingulotomy as a Therapeutic Option for Intractable Cancer Pain: a Systematic Review.

PURPOSE OF REVIEW: Cingulotomy, a stereotactic neurosurgical procedure targeting the anterior cingulate cortex, has been explored for various refractory pain and neuropsychiatric conditions. However, while prior reviews have largely focused on chronic non-cancer pain, there remains a significant knowledge gap regarding its role in managing intractable cancer-related pain. This systematic review therefore aimed to evaluate the efficacy and safety of bilateral cingulotomy as a therapeutic option for cancer-associated pain, addressing a highly underexplored but clinically relevant area of neurosu…

Administration of Intrathecal Ketamine in the Treatment of Refractory Cancer Pain: A Case Series.

OBJECTIVES: Intrathecal opioids may not be effective for severe refractory pain associated with cancer. Intrathecal ketamine may be effective in reducing pain in these cases, however, there is legitimate concern regarding ketamine neurotoxicity. We present our experience using intrathecal ketamine in 17 consecutive patients in one clinic, as well as, a case report of one patient who received intrathecal ketamine for 36 wk. MATERIALS AND METHODS: A retrospective case review included all patients treated with intrathecal ketamine at one center. The primary outcome was dosing required to change

Efficacy and safety of hydromorphone for cancer pain: a systematic review and meta-analysis.

BACKGROUND: Cancer pain significantly impacts individuals' quality of life, with opioids being employed as the primary means for pain relief. Nevertheless, concerns persist regarding the adverse reactions and effectiveness of opioids such as morphine. Hydromorphone, recognized as a potent opioid, is a viable alternative for managing cancer-related pain. The goal of this systematic review and meta-analysis was to determine the effectiveness and safety characteristics of hydromorphone in comparison to other opioids, as well as different methods of administering this medication within the scope o…

[On the difficulties and pains in managing peritoneal metastasis from gastrointestinal cancer in China].

Modern clinical oncology has made great achievements over the last century. However, peritoneal metastasis from gastrointestinal cancer, as one of three most common metastasis modalities, was not re-recognized until the end of the last century, and a normative diagnosis and treatment system has been gradually beginning to be formed until today. This comment is to review the development history, reflect on the lessons and experiences in clinical practice, analyze the difficulties on redefinition, deep understanding and clinical management, and pain points on theory construction, technique pract

Painful ischemic monomelic neuropathy: An unusual complication in a lung cancer patient.

Ischemic monomelic neuropathy (IMN) is a rare type of acute axonal neuropathy which results from ischemia of multiple nerves in affected limb. The electroneuromyography is useful in detecting characteristic features of this neuropathy. It usually occurs after vascular interventions. Here, we present the first case who has IMN secondary to lung cancer and/or chemo-therapy and aim to draw attention to this infrequently recognized entity.
