High striatal amyloid beta-peptide deposition across different autosomal Alzheimer disease mutation types.

BACKGROUND: Supported by compelling genetic data regarding early-onset familial Alzheimer disease (AD), the amyloid beta-peptide (Abeta)-centric theory holds that Abeta is involved in the pathogenesis of sporadic AD. Mutations in the amyloid precursor protein (APP), presenilin 1 (PSEN1), and presenilin 2 (PSEN2) genes lead to increased Abeta levels before symptoms arise. OBJECTIVES: To evaluate the pattern of Pittsburgh Compound B (PiB) retention in subjects with different autosomal dominant mutations associated with familial AD vs that in healthy age-matched control subjects and subjects wit

AI-Assisted Decision-Making in Long-Term Care: Qualitative Study on Prerequisites for Responsible Innovation.

BACKGROUND: Although the use of artificial intelligence (AI)-based technologies, such as AI-based decision support systems (AI-DSSs), can help sustain and improve the quality and efficiency of care, their deployment creates ethical and social challenges. In recent years, a growing prevalence of high-level guidelines and frameworks for responsible AI innovation has been observed. However, few studies have specified the responsible embedding of AI-based technologies, such as AI-DSSs, in specific contexts, such as the nursing process in long-term care (LTC) for older adults. OBJECTIVE: Prerequis

A prospective study to assess the effectiveness and safety of the BlueWind System in the treatment of patients diagnosed with urgency urinary incontinence.

BACKGROUND: Overactive bladder (OAB) affects one in six adults in Europe and the United States and impairs the quality of life of millions of individuals worldwide. When conservative management fails, third-line treatments including tibial neuromodulation (TNM) is often pursued. TNM has traditionally been accomplished percutaneously in clinic. OBJECTIVE: A minimally invasive implantable device activated by a battery-operated external wearable unit has been developed for the treatment of urgency urinary incontinence (UUI), mitigating the burden of frequent clinic visits and more invasive thera

Reasons for Long-term Opioid Prescriptions After Guideline-directed Opioid Prescribing and Excess Opioid Pill Disposal.

OBJECTIVES: The aim of this study was to determine the frequency and reasons for long-term opioid prescriptions (rxs) after surgery in the setting of guideline-directed prescribing and a high rate of excess opioid disposal. BACKGROUND: Although previous studies have demonstrated that 5% to 10% of opioid-naïve patients prescribed opioids after surgery will receive long-term (3-12 months after surgery) opioid rxs, little is known about the reasons why long-term opioids are prescribed. METHODS: We studied 221 opioid-naïve surgical patients enrolled in a previously reported prospective clinical

Integrative analysis of blood biochemistry and sperm metabolome profiles reveals metabolic determinants of cryotolerance in Bali bulls under standardized nutritional conditions.

BACKGROUND: Enhancing the nation's beef supply and guaranteeing long-term food security require increasing the effectiveness of artificial insemination (AI) in Bali cattle (Bos javanicus). Cryotolerance, or post-thaw variability in semen quality, limits AI results and probably reflects systemic and cellular metabolic variations. To determine the metabolic determinants of cryotolerance under standardized nutrition and supervision, this study combined blood biochemistry with the sperm metabolome. METHODS: Ten healthy breeding bulls at the regional artificial insemination center (RAIC) were main

Clinical and radiological comparison of three different reverse shoulder arthroplasty designs for patients with primary osteoarthritis.

AIMS: In reverse shoulder arthroplasty (RSA), different implant designs range from medializing implants to strongly lateralizing onlay designs with different neck-shaft-angles (NSA). Thus different degrees of lateralization are currently used. Aim of this study was to compare clinical and radiological outcomes of three different implant designs in a homogeneous patient cohort with primary osteoarthritis (OA). METHODS: Patients with OA who underwent RSA between 03/2014 and 01/2020 were included and categorized into three groups based on RSA design: group MD (medialized-distalized design: eccen

A reappraisal of the role of the mammillothalamic tract in memory deficits following stroke in the thalamus.

The thalamus, traditionally viewed as a sensory relay station, is now recognized for its critical role in higher-order cognitive functions, including memory. While most research has focused on its distinct nuclei, the thalamus also contains crucial white matter tracts, such as the mammillothalamic tract (MTT), part of the Papez circuit. Despite its established role in memory, the MTT remains underappreciated in studies on the cognitive role of the thalamus, increasing the risk of misattributing memory functions to nearby thalamic nuclei. This study investigates the memory impact of thalamic st

A fine-tuned genomic language model captures nucleotide-level information overlooked by missense variant impact predictors.

Missense variant interpretation remains a major challenge in clinical genomics. Existing missense variant impact predictors achieve strong performance, but they emphasize protein-level consequences and often share overlapping annotation priors. A missense annotation specifies the encoded amino-acid substitution, but the underlying nucleotide change may also act through nucleotide sequence context that protein-centric predictors overlook. Whether genomic language models capture distinctive nucleotide-level information beyond established missense variant impact predictors remains unclear. Throug

Age-dependent diagnostic and correlational architecture of multiplex plasma biomarkers in Alzheimer's disease: a cross-ethnic, cross-platform validation study.

BACKGROUND: Plasma phosphorylated tau-217 (p-tau217) is one of the most accurate blood-based biomarkers for Alzheimer's disease (AD), yet whether its diagnostic and correlational properties vary with age at onset has not been systematically examined. METHODS: We measured eight plasma biomarkers (tau, amyloid, neurodegeneration, neuroinflammation) via automated chemiluminescence immunoassay in a Chinese memory clinic cohort (n = 604; amyloid positron emission tomography (PET) reference), with validation in the Alzheimer's Disease Neuroimaging Initiative (ADNI) (n = 1,615; amyloid and tau PET).

Nutrition care and Dietetic Practice in Managing Metabolic and Endocrine Dysfunction in Polyendocrine Metabolic Ovarian Syndrome (PMOS): Implications and Opportunities in the Context of the Name Change of Polycystic Ovarian Syndrome (PCOS) to PMOS.

Polyendocrine metabolic ovarian syndrome (PMOS) affects 8-13% of women across the lifespan. Those with PMOS have reduced quality of life and increased risk of co-morbidities including elevated weight, eating disorders, gestational diabetes, type 2 diabetes and cardiovascular disease. PMOS is a major population in clinical dietetic practice and dietary intervention is the first-line approach recommended to target metabolic risk in the international evidence-based guidelines for assessment and management of PCOS. The past name, polycystic ovary syndrome (PCOS) was ovarian-centric and did not acc

New-Onset Depression Is Associated With Low Income and Adverse Arthroplasty-Related Complications.

Background and objective Depression may manifest in total joint arthroplasty (TJA) patients even in the absence of any prior history. While research has demonstrated depression to be associated with low socioeconomic status and TJA complications, no studies have explored these associations related to new-onset depression in post-TJA settings. Hence, this study aimed to address that by conducting a retrospective, database-centric study. Methods A national database was queried for data on primary total hip arthroplasty (THA) and total knee arthroplasty (TKA) patients ≥18 years with hip/knee oste

An Insight into Survivin in Relevance to Hematological, Biochemical and Genetic Characteristics in Tobacco Chewers with Oral Squamous Cell Carcinoma.

BACKGROUND: Survivin is an inhibitor of apoptosis protein (IAP), encoded by the Baculoviral IAP Repeat Containing 5 (BIRC5) gene located on q arm (25.3) on chromosome 17. It is expressed in various human cancers and involved in tumor resistance to radiation and chemotherapy. The genetic analysis of the BIRC5 gene and its protein survivin levels in buccal tissue related to oral squamous cell carcinoma (OSCC) in South Indian tobacco chewers has not been studied. Hence, the study was designed to quantify survivin in buccal tissue and its association with pretreatment hematological parameters and
