Integration of yaws control and primary health care.

Where a primary health care (PHC) system (e.g., a community health worker and a health center as the first referral level) is fully operational, control activities for yaws should be part of PHC. Where the whole population is adequately covered, the PHC system is responsible for early detection of cases of active clinical yaws, treatment and follow-up of these cases, treatment and follow-up of contacts, and health education related to yaws control. In circumstances where PHC is not as yet fully implemented, the structure for a yaws control program must be different. In areas with adequately fu

Moving towards culturally competent health systems for migrants? Applying systems thinking in a qualitative study in Malaysia and Thailand.

BACKGROUND: Cultural competency describes interventions that aim to improve accessibility and effectiveness of health services for people from ethnic minority backgrounds. Interventions include interpreter services, migrant peer educators and health worker training to provide culturally competent care. Very few studies have focussed on cultural competency for migrant service use in Low- and Middle-Income Countries (LMIC). Migrants and refugees in Thailand and Malaysia report difficulties in accessing health systems and discrimination by service providers. In this paper we describe stakeholder

Tuberculosis and latent tuberculosis infection among healthcare workers in Kisumu, Kenya.

OBJECTIVE: To assess prevalence and occupational risk factors of latent TB infection and history of TB disease ascribed to work in a healthcare setting in western Kenya. METHODS: We conducted a cross-sectional survey among healthcare workers in western Kenya in 2013. They were recruited from dispensaries, health centres and hospitals that offer both TB and HIV services. School workers from the health facilities' catchment communities were randomly selected to serve as the community comparison group. Latent TB infection was diagnosed by tuberculin skin testing. HIV status of participants was a

A cluster randomized trial for improving mental health and well-being of persons affected by leprosy or buruli ulcer in Nigeria: A study protocol.

This protocol describes a study in which we would assess the effect of using community lay counselors, self-help groups (SHGs), and trained frontline health workers to reduce mental disorders and improve quality of life (QOL) of persons affected by leprosy or Buruli ulcer (BU). A cluster randomized controlled study design will be employed. The study will involve persons affected by leprosy or BU. Ten local government areas (clusters) with the highest number of notified leprosy or BU cases between 2014 and 2018 in Southern Nigeria will be purposively selected. The clusters will be randomized in

Using Normalisation Process Theory to Develop a Complex Intervention: A Qualitative Case Study of Screening for Homelessness in Healthcare Settings.

BACKGROUND AND AIMS: Normalisation Process Theory (NPT) seeks to describe and explain how a complex intervention can be embedded into an existing health and social care system. NPT is frequently used to evaluate interventions, and less frequently to design them.We demonstrate how NPT can be applied to exploratory qualitative data to understand the potential barriers and facilitators to the implementation of a new intervention in a health setting using the NPT framework. We illustrate this application using a case study of screening for homelessness and housing instability in Australian emergen

A cost consequence analysis of outreach strategies for high risk pregnant women.

This retrospective, cost consequence analysis examined three distinct outreach strategies used by a community-based public health agency to enroll uninsured, low-income pregnant women in perinatal health care services. The three strategies included outreach workers, liaison workers, and case managers. Data analysis revealed that costs per enrollee by strategy were: outreach workers: $429; liaison workers: $98; and case managers: $187. Cost per strategy must be considered cautiously, however, because outreach and enrollment strategies at this agency are not mutually exclusive. Data analysis als

"We witnessed medicines help our children, then we became proud": experiences and acceptability of seasonal malaria chemoprevention in Aweil, South Sudan.

BACKGROUND: Malaria remains a significant public health concern, particularly in resource-constrained settings and regions affected by ongoing humanitarian crises, such as Northen Bahr el Ghazal in South Sudan. For successful implementation and uptake of malaria interventions, it is important to understand community perceptions in these contexts. Therefore, we present the findings of the qualitative study examining the feasibility of implementing seasonal malaria chemoprevention (SMC) and acceptability of the intervention by the community in this complex setting. METHODS: Five monthly cycles

A rapid increase in coverage of COVID-19 vaccination, Central African Republic.

PROBLEM: In 2021, Central African Republic was facing multiple challenges in vaccinating its population against coronavirus disease 2019 (COVID-19), including inadequate infrastructure and funding, a shortage of health workers and vaccine hesitancy among the population. APPROACH: To increase COVID-19 vaccination coverage, the health ministry used three main approaches: (i) task shifting to train and equip existing community health workers (CHWs) to deliver COVID-19 vaccination; (ii) evidence gathering to understand people's reluctance to be vaccinated; and (iii) bundling of COVID-19 vaccinati…

Potential Resilience to Post-traumatic Stress Disorder and Common Mental Disorders among Lay Health Workers Working on the Friendship Bench Programme in Zimbabwe.

PURPOSE: The Friendship Bench is a successful task-shifting intervention for addressing common mental disorders in Zimbabwe. The intervention takes a cognitive behavioural therapy and problem-solving approach provided by lay health workers (LHWs). The LHWs live in the same environment as their clients and are thus exposed to the same traumas and stressors as are their clients. Little is known about the long-term psychological effects on LHWs involved in this type of work. METHODS: A random sample of LHWs (n=182) was assessed using the Posttraumatic Stress Disorder Checklist (PCL-5) and the Sh

Optimizing Myanmar's community-delivered malaria volunteer model: a qualitative study of stakeholders' perspectives.

BACKGROUND: In parallel with the change of malaria policy from control to elimination and declines in the malaria burden in Greater Mekong Sub-region, the motivation and social role of malaria volunteers has declined. To address this public health problem, in Myanmar, the role and responsibilities of malaria volunteers have been transformed into integrated community malaria volunteers (ICMV), that includes the integration of activities for five additional diseases (dengue, lymphatic filariasis, tuberculosis, HIV/AIDS and leprosy) into their current activities. However, this transformation was

The Ashodaya PrEP project: Lessons and implications for scaling up PrEP from a community-led demonstration project among female sex workers in Mysore, India.

To inform PrEP roll out, Ashodaya Samithi, a sex workers' collective, conducted a community-led prospective demonstration project among female sex workers in Mysore and Mandya, India. Following a community preparedness phase and pre-screening, participants were recruited for clinical screening and enrolment, provided PrEP as part of combination HIV prevention, and followed for 16 months. Adherence was measured by self-reported pill intake and by tenofovir blood level testing among a subset of participants. Of the 647 participants enrolled, 640 completed follow-up. Condom use remained stable an

The use of counting beads to improve the classification of fast breathing in low-resource settings: a multi-country review.

To decrease child mortality due to common but life-threatening illnesses, community health workers (CHWs) are trained to assess, classify and treat sick children. For pneumonia, CHWs are trained to count the respiratory rate of a child with cough and/or difficulty breathing, and determine whether the child has fast breathing or not based on how the child's breath count relates to age-specific respiratory rate cut-off points. International organizations training CHWs to classify fast breathing realized that many of them faced challenges counting and determining how the respiratory rate relates …

Low dosage diethylcarbamazine administered by villagers for the control of timorian filariasis.

The results of mass treatment using low dosage diethylcarbamazine (DEC) in three small villages in West Flores, Indonesia, endemic for Brugia timori are described. DEC was distributed on a weekly basis by motivated persons in the community to nearly all villagers for 18 months. An educational programme was developed which focused on describing the relationship between microfilaraemia, vector and disease manifestations, the danger of repeated attacks of adenolymphangitis, and the efficiency of DEC in eliminating the parasites and attacks. During the consolidation phase motivated persons were as

Structural barriers and facilitators to accessing HIV services for marginalized working populations: insights from farm workers in South Africa.

Farm workers are vulnerable working populations who face significant inequalities in accessing health services, including those for human im … facilitators to HIV healthcare included the availability of mobile health services, the presence of community health workers and a supportive work environment. The findings suggest disparities in farm workers' access to HIV services, with work being the main determinant of access. We, therefore, recommend a review of HIV policies and programmes for the agricultural sector …

Empowering communities against AMR: Evaluating the one health-based antibiotic-wise village (SAJAKA) in Bali, Indonesia.

Antimicrobial resistance (AMR) is a critical global health issue, especially in low-resource settings. A One Health-based community engagement approach, such as the Antibiotic-Wise Village (SAJAKA) model, offers a potential solution. This study assessed SAJAKA's implementation in four Bali villages, evaluating its impact on knowledge and antibiotic use behavior. The intervention included workshops, school programs, poultry feed/water analysis, and medical audits. Using a pre-test/post-test design, 769 participants including household mothers, schoolchildren, farmers, healthcare workers were su

Integrating Case Detection of Visceral Leishmaniasis and Other Febrile Illness with Vector Control in the Post-Elimination Phase in Nepal.

Nepal has completed the attack phase of visceral leishmaniasis (VL) elimination and now needs active case detection (ACD) and vector control methods that are suitable to the consolidation and maintenance phases. We evaluated different ACD approaches and vector control methods in Saptari district. We assessed 1) mobile teams deployed in villages with VL cases in 2015 to conduct combined camps for fever and skin lesions to detect VL/PKDL (post-kala-azar dermal leishmaniasis) and other infections; 2) an incentive approach by trained female community health volunteers (FCHVs) in villages with no V

From implementation to sustainability? A 5-year follow-up on a physical activity counseling program in Uganda for people with HIV and mental health conditions.

Physical activity (PA) interventions benefit people living with HIV and co-morbid mental health conditions, but sustaining them in low-resource settings remains difficult. This study explored the long-term sustainability of a multi-stakeholder PA counseling program in a rural Ugandan fishing community, five years post-implementation. The program integrated a PA counseling into routine HIV care, combining motivational interviewing and self-determination theory. It began with sessions led by health care providers and later transitioned to lay-led group sessions that focused on goal-setting, iden
