Modelling disease outbreaks in realistic urban social networks.

Most mathematical models for the spread of disease use differential equations based on uniform mixing assumptions or ad hoc models for the contact process. Here we explore the use of dynamic bipartite graphs to model the physical contact patterns that result from movements of individuals between specific locations. The graphs are generated by large-scale individual-based urban traffic simulations built on actual census, land-use and population-mobility data. We find that the contact network among people is a strongly connected small-world-like graph with a well-defined scale for the degree dis

[Chlamydia-like symptoms can have another etiology. Mycoplasma genitalium--an important and common sexually transmitted disease].

The aetiological importance of Chlamydia (C.) trachomatis in non-gonococcal urethritis (NGU) is undisputed. Mycoplasma (M.) genitalium has been shown to be strongly associated with NGU and with mucopurulent cervicitis and also with acute endometritis independent of C. trachomatis. In this prevalence study we examined 946 patients, 445 women and 501 men, attending the STD clinic, for M. genitalium as well as C. trachomatis and Neisseria gonorrhoeae. M. genitalium was detected in urethral samples from 17 (13.4 per cent) of 127 men with both symptoms and signs of urethritis and from 2 (1.3 per ce

Measles in Thai adults: three case reports and management of contact cases.

We report on three adults (one nurse and two medical students) diagnosed as having measles. All the patients presented with fever, cough, conjunctival injection and rash. They contracted measles from pediatric patients who had been treated at Chulalongkorn Hospital in the previous two weeks. Physical examination revealed Koplik's spots on the oral mucosa and typical maculopapular rash. The diagnosis was confirmed by viral isolation. Measles IgG antibodies were measured in 36 medical students who were in close contact with patient 2 and measles vaccines were given. Thirty-three specimens had po

[Prevention of sexually transmitted diseases or control of prostitutes and patients with venereal diseases? An exemplary, epicritical study in Bremen].

Data derived from laboratory and activity statistics of the Venereal Disease" department of the Public Health Office in Bremen were analysed between 1961 and 1991. The traditional concept to fight venereal diseases" aimed at controlling persons "at risk" for syphilis, gonorrhoea, chancroid or lymphogranuloma. This "high-risk" approach of STD prevention stressed the compulsory, at least twice monthly screening of female prostitutes and neglected other population groups like clients of prostitutes and young people, who might have an elevated risk in acquiring STD. The spread of STD like chlamydi

Remote health diagnosis and monitoring in the time of COVID-19.

Coronavirus disease (COVID-19), caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), is rapidly spreading across the … arantine site at home, or dedicated quarantine locations (e.g. hotels). In particular, facilitating contact tracing with proximity and location tracing apps was adopted in many countries very rapidly. Thus, the pandemic has driven incentives to innovate and enhance or create new routes for providing healthcare services at distance. In particular, this has created …

Likelihood of SARS-CoV-2 transmission in multi-bedded patient rooms by infecting variant.

OBJECTIVE: Sharing a room with an unrecognized contagious patient poses the greatest in-hospital risk of transmission of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). This study quantified secondary attack rates associated with exposure to the SARS-CoV-2 Omicron subvariant in an oncology centre. METHODS: Patients with hospital-onset SARS-CoV-2 infection from 2021 to 2024 were evaluated for roommate exposure, with secondary attack rates stratified by the predominant circulating variant. RESULTS: Among 299 patients who shared a room with a patient with hospital-onset SARS-CoV-2

Implementation of ML Flow for leprosy contacts in Brazil: Opportunities, pitfalls, and safeguards.

Brazil has the second-highest leprosy burden worldwide, with approximately 20,000 new cases reported annually, many diagnosed with advanced disease and disability. To support earlier detection, the Ministry of Health recently approved the ML Flow rapid test for contact evaluation. ML Flow detects immunoglobulin M antibodies against phenolic glycolipid-I of Mycobacterium leprae, can be performed at the point of care using finger-prick blood, and yields results within minutes. ML Flow offers important operational advantages. It enables same-visit counseling and may support risk-stratified househ

HIV self-testing and partner notification strategies for key populations in low- to upper-middle-income countries: A mixed-methods systematic review.

BACKGROUND: HIV self-testing (HIVST) enhances case diagnosis, but information on its integration for index testing to partner notification is limited. Assessing the acceptability and feasibility of index HIVST and partner testing among people living with HIV (PLHIV) and undiagnosed key populations is critical to ending the AIDS epidemic. METHODS: A mixed-methods systematic review using a convergent segregated approach was conducted using the Joanna Briggs Institute's methodology. Four databases were used to conduct a literature review from October 2023 to March 2024, which included studies pu

Effects of improved partner notification on the transmission of HIV and N. gonorrhoea among men who have sex with men: a modelling study.

Objectives Men who have sex with men (MSM) are disproportionally affected by HIV in the Netherlands. Partner notification (PN) is an important element in controlling the transmission of sexually transmitted infections (STIs) and HIV. We investigated the effects of improving PN on the transmission of HIV and Neisseria gonorrhoeae (NG) among MSM in the Netherlands. Methods We developed an agent-based model that describes the transmission of HIV and NG among MSM. In the baseline scenario, 14.3% and 29.8% of casual and steady partners of the index case get notified and tested for HIV/STI after 3 w

The impact of screening for tuberculosis exposure in the household in children with tuberculosis disease: A difficult riddle to solve.

BACKGROUND: Tuberculosis (TB) infection is transmitted by sharing the same airway with people with active TB. Children are often not considered the source of TB bacilli, and index case investigation is carried out after diagnosis. Here, we describe the impact of the presence of a household index case on childhood TB disease. METHODS: The data of patients aged between 1 month and 18 years who were diagnosed with TB were collected. We compared patients according to whether they had an index case in the household or not. RESULTS: A total of 202 TB patients were enrolled, of whom 62 (30.7%) had

Prevention of sexual transmission of mpox: a systematic review and qualitative evidence synthesis of approaches.

BACKGROUND: The ongoing multi-country mpox outbreak in previously unaffected countries is primarily affecting sexual networks of men who have sex with men. Evidence is needed on the effectiveness of recommended preventive interventions. To inform WHO guidelines, a systematic review and qualitative evidence synthesis were conducted on mpox preventive behavioural interventions to reduce: (i) sexual acquisition; (ii) onward sexual transmission from confirmed/probable cases; and (iii) utility of asymptomatic testing. METHODS: Medline, EMBASE, PubMed, Cochrane and WHO trial databases, grey literat
