Pneumonia in residents of long-term care facilities: epidemiology, etiology, management, and prevention.

Pneumonia is a leading cause of morbidity and mortality among patients in long-term care facilities; the median reported incidence is 1 per 1,000 patient-days. Risk factors include functional dependency, chronic pulmonary disease, and conditions causing aspiration. The frequency of etiologic agents varies widely among reports; for example; Streptococcus pneumoniae ranges from 0% to 39% of cases, and gram negative bacilli ranges from 0% to 51% of reported cases. Viral respiratory infections, particularly influenza and respiratory syncytial virus, typically occur in outbreaks. Mortality varies f

[Laparoscopy and salpingitis. For whom? When? How?].

The apparent decrease of the incidence of pelvic inflammatory disease (PID) in industrial countries comes from the increase of less symptomatic forms. Diagnosis requires more often non clinical diagnostic technics. Laparoscopy remains the gold standard for the diagnosis and grading of PID but its indications should be on an accurate strategy.

Gestational diabetes mellitus.

Diabetes sometimes appears for the first time during pregnancy. It is important that all cases are detected, so all pregnant woman should be screened for this condition. Screening protocols vary but usually involve urinalysis at all clinic visits plus a blood glucose test, taken after a meal, at 24-28 weeks. Risk factors include increasing maternal age (25 years upwards); obesity; family history of diabetes; and previous unexplained stillbirths or babies with congenital abnormalities. There is an increased frequency of gestational diabetes in Oriental women and those from the Indian subcontine

Recognizing elder abuse and neglect.

The signs and symptoms of elder abuse and neglect may mimic the signs and symptoms of many common chronic medical conditions in elderly persons. A complete assessment for elder abuse and neglect is time consuming and is best performed as a regimented evaluation that includes separate histories from the patient and suspected party and a clinical assessment that emphasizes function, cognition, and specific aspects of the physical examination. The management of elder abuse and neglect should be multidisciplinary with several key personnel participating. An elderly person in immediate danger shoul

Hypoplastic left heart syndrome.

HLHS is a complex developmental malformation of the left ventricle, aorta, and associated valves. Survival beyond neonatal life is the exception in untreated infants. The prognosis, however, is no longer hopeless. Newer surgical strategies of staged palliative reconstruction and heart replacement by transplantation appear promising. These options on occasion will become complementary in the management of individual patients. Those children surviving Fontan operations are frequently able to lead NYHA Class I or II lives without drugs, while transplant recipients will require one or more immunor

Computer-aided decision system for the clubfeet deformities.

A computer-aided decision system (CADS) based on ontology in pediatric orthopedics was developed to assess, without assumptions performed, the abnormalities of the musculoskeletal system of lower limbs. The CADS consists of four components. The first component is a diagnosis-based ontology, called Ontologie du Système Musculosquelettique des Membres Inférieurs (OSMMI). The second component is a database for collecting clinical observations, e.g., the birth classification of the clubfeet deformities. The third component uses statistical methods (principal component analysis and decision tree) f

Application of Principal Component Analysis to Newborn Screening for Congenital Adrenal Hyperplasia.

PURPOSE: Newborn screening laboratories are challenged to develop reporting algorithms that accurately identify babies at increased risk for congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency (21OHD). Screening algorithms typically use cutoff values for a key steroid(s) and include considerations for covariates, such as gestational age or birth weight, but false-positive and false-negative results are still too frequent, preventing accurate assessments. Principal component analysis (PCA) is a statistical method that reduces high-dimensional data to a small number of componen

Objective: To explore high-risk sexual behaviors of HIV/AIDS and related factors in young students in Guangzhou. Methods: A cross-sectional survey was conducted in 5 different types of Guangzhou colleges by convenience sampling with minimum number of classes per grade and 600 samples per school from September to November 2021. The R 4.2.2 software was used to consolidate databases. Simultaneously, a logistic regression model and a decision tree algorithm model, stratifying by whether sexual behaviors had occurred before, were constructed. In each layer, the prediction performance of the two mo

Simplified Machine Learning Models Can Accurately Identify High-Need High-Cost Patients With Inflammatory Bowel Disease.

INTRODUCTION: Hospitalization is the primary driver of inflammatory bowel disease (IBD)-related healthcare costs and morbidity. Traditional prediction models have poor performance at identifying patients at highest risk of unplanned healthcare utilization. Identification of patients who are high-need and high-cost (HNHC) could reduce unplanned healthcare utilization and healthcare costs. METHODS: We conducted a retrospective cohort study in adult patients hospitalized with IBD using the Nationwide Readmissions Database (model derivation in the 2013 Nationwide Readmission Database and validati

Management of Cardiovascular Disease Patients With Confirmed or Suspected COVID-19 in Limited Resource Settings.

In this paper, we provide recommendations on the management of cardiovascular disease (CVD) among patients with confirmed or suspected coronavirus disease (COVID-19) to facilitate the decision making of healthcare professionals in low resource settings. The emergence of novel coronavirus disease, also known as Severe Acute Respiratory Syndrome-Coronavirus-2 (SARS-CoV-2), has presented an unprecedented global challenge for the healthcare community. The ability of SARS-CoV-2 to get transmitted during the asymptomatic phase and its high infectivity have led to the rapid transmission of COVID-19 b

Primary care and upfront computed tomography scanning in the diagnosis of chronic rhinosinusitis: a cost-based decision analysis.

OBJECTIVES/HYPOTHESIS: To diagnose chronic rhinosinusitis (CRS), current guidelines require either endoscopic or computed tomography (CT) findings of sinus disease. To a primary care physician, this means a referral to an otolaryngologist or obtaining a CT scan. Unfortunately, the sensitivity of endoscopy for detecting CRS is low, and examination by the Otolaryngologist may not yield a definitive diagnosis. This leaves CT scanning. However, this is contradicted by recommendations to limit CT scanning for only preoperative planning purposes due to cost concerns. This study aims to provide an ev
