Tolerability of the dexamethasone-corticotropin releasing hormone test in major depressive disorder.

BACKGROUND: The dexamethasone-corticotropin releasing hormone (Dex-CRH) test may differentially predict which depressed patients will respond to antidepressant medication. However, a comprehensive analysis of the safety of this test in psychiatric patients has not previously been performed. METHODS: We conducted a pooled analysis of depressed patients in four clinical studies. Observed and subjectively reported side-effects in 454 patients were collected for 90 minutes following CRH administration. Pre-test electrocardiograms were available in 250 patients to assess cardiac safety. Descriptiv

Microstructural abnormalities in subcortical reward circuitry of subjects with major depressive disorder.

BACKGROUND: Previous studies of major depressive disorder (MDD) have focused on abnormalities in the prefrontal cortex and medial temporal regions. There has been little investigation in MDD of midbrain and subcortical regions central to reward/aversion function, such as the ventral tegmental area/substantia nigra (VTA/SN), and medial forebrain bundle (MFB). METHODOLOGY/PRINCIPAL FINDINGS: We investigated the microstructural integrity of this circuitry using diffusion tensor imaging (DTI) in 22 MDD subjects and compared them with 22 matched healthy control subjects. Fractional anisotropy (FA)…

Association of GSK3beta polymorphisms with brain structural changes in major depressive disorder.

CONTEXT: Indirect evidence suggests that the glycogen synthase kinase-3beta (GSK3beta) gene might be implicated in major depressive disorder (MDD). BACKGROUND: We evaluated 15 GSK3beta single-nucleotide polymorphisms (SNPs) to test for associations with regional gray matter (GM) volume differences in patients with recurrent MDD. We then used the defined regions of interest based on significant associations to test for MDD x genotype interactions by including a matched control group without any psychiatric disorder, including MDD. DESIGN: General linear model with nonstationary cluster-based …

When should psychotherapy be the treatment of choice for major depressive disorder?

Clinicians should think carefully before deciding on the initial treatment for a patient with major depressive disorder. The author argues that this does not happen often enough, and that psychotherapy may be overlooked too often as a first treatment option. Based on the research literature and the American Psychiatric Association Practice Guidelines, this article reviews potential cautions and relative indications for initiating treatment with evidence-based psychotherapies for major depressive disorder. Potential indications include 1) patient preference; 2) symptom severity; 3) relative con…

Population-based study of first onset and chronicity in major depressive disorder.

CONTEXT: There are no studies of the natural history of major depressive disorder that lack prevalence and clinic biases. OBJECTIVES: To estimate risk factors for first lifetime onset and parameters of chronicity following the first episode, including duration, recovery, and recurrence, and to search for predictors of each parameter. DESIGN: Prospective population-based cohort study with 23 years of follow-up. SETTING: East Baltimore, Maryland, an urban setting. PARTICIPANTS: Probability sample of 3481 adult household residents in 1981, including 92 with first lifetime onset of major depre…

Comparison of clinical characteristics of opium-induced and independent major depressive disorder.

Psychiatric disorders and among them depression are common in substance dependent patients. The aim of this study is to compare the clinical characteristics of those that appear to have substance-induced depression and those that have independent major depression. One-hundred eighty-four independent and 187 opium-induced (OID) depressed male patients that met the DSM-IV criteria for major depressive disorder were randomly selected. Standard demographic data, including age, marital, employment and education status, were collected. The primary measure of depressive signs and symptoms was Hamilto…

Diagnosing major depressive disorder: II: is there justification for compound symptom criteria?

The DSM-IV symptom inclusion criteria for the diagnosis of major depressive disorder (MDD) are constructed in three ways: single symptom criteria, compound criteria encompassing opposite variants of the same disturbance, and compound criteria encompassing related problems. In the present report from the Rhode Island Methods to Improve Diagnostic Assessment and Services project, we tested the following three hypotheses: (1) the components of compound-opposite criteria rarely occur simultaneously, (2) the components of the compound-related criteria frequently occur simultaneously, and (3) the co…

Major depressive disorder with psychotic features in an aviator after head trauma.

We present a case in which a Naval aviator suffered an unusual sequence of neuropsychiatric symptoms after head trauma. He demonstrated subtle deficits on several measures 1 mo after the trauma. Repeat testing at 8 mo showed apparent recovery, and the patient was cleared to return to flight status. Unbeknownst to medical staff, the patient was still experiencing difficulties. He was found in a severely debilitated state and exhibiting psychotic features 12 mo after his initial trauma. The patient was hospitalized in a psychiatric ward, and with prolonged inpatient and outpatient treatment, he

Increased prevalence of negative life events in subtypes of major depressive disorder.

The prevalence of negative life events is known to be increased among patients with depression. Little data exist on the specific subtypes of depression that are related to negative life events. Our study aimed to address this issue. We compare 115 patients with major depressive disorder (MDD) to 60 normal controls. MDD patients reported experiencing one (P = .0001) or two (P = .01) negative life events more frequently than controls. Patients reported marital, other personal problems, and medical events significantly more often than controls (P < .01). Patients did not report more positive lif…

Adinazolam, diazepam, imipramine, and placebo in major depressive disorder: a controlled study.

In summary, the clinical results of this double-blind study clearly show that imipramine, as expected, demonstrated significant antidepressive properties in outpatients suffering from major depressive disorders. In contrast, adinazolam showed rather mild and weak antidepressive properties, and in no measures did its response differ significantly from that of diazepam. These findings are quite in contrast to those obtained by the authors in an earlier study with alprazolam (Rickels et al., 1987) in which alprazolam and imipramine produced rather similar results and both were significantly bette…

Cross disorder comparisons of brain structure in schizophrenia, bipolar disorder, major depressive disorder, and 22q11.2 deletion syndrome: A review of ENIGMA findings.

This review compares the main brain abnormalities in schizophrenia (SZ), bipolar disorder (BD), major depressive disorder (MDD), and 22q11.2 Deletion Syndrome (22q11DS) determined by ENIGMA (Enhancing Neuro Imaging Genetics through Meta Analysis) consortium investigations. We obtained ranked effect sizes for subcortical volumes, regional cortical thickness, cortical surface area, and diffusion tensor imaging abnormalities, comparing each of these disorders relative to healthy controls. In addition, the studies report on significant associations between brain imaging metrics and disorder-relate…

Elevated C-reactive protein levels across diagnoses: The first comparison among inpatients with major depressive disorder, bipolar disorder, or obsessive-compulsive disorder.

OBJECTIVE: We addressed elevated C-reactive protein level (eCRP) specificity comparing, for the first time, eCRP (i.e., serum CRP > 3 and ≤10 mg/L) in patients with major depressive disorder (MDD), bipolar disorder (BD), or obsessive-compulsive disorder (OCD). We also assessed to what extent multiple variables that can potentially increase inflammation may have influenced eCRP in our sample. METHODS: We performed a retrospective, observational, cross-sectional study using information documented in the electronic medical records (EMRs) of patients hospitalized for a 4-week psychiatric rehabili…
