Commissural symmetry in unilateral transverse facial cleft patients: an anthropometric study.

PURPOSE: To evaluate the commissural symmetry and commissural migration among patients with unilateral transverse facial cleft. PATIENTS AND METHODS: From 2006 to 2009, 31 patients with unilateral transverse facial cleft were repaired with superiorly based vermillion-mucosal flap and orbicularis oris reconstruction. Linear closure and superiorly rotated and inferiorly rotated Z-plasty were randomly chosen for skin closure. An anthropometric method was used immediately after the repair and at the follow-up examination to evaluate the commissural symmetry in both horizontal and vertical directi

Advances in collision detection and non-linear finite mixed element modelling for improved soft tissue simulation in craniomaxillofacial surgical planning.

BACKGROUND: There is a huge demand to develop a method for assisting surgeons in automatically predicting soft tissue deformation in terms of a bone-remodelling plan. METHODS: This paper introduces several novel elements into a system for the simulation of postoperative facial appearances with respect to prespecified bone-remodelling plans. First, a new algorithm for efficient detection of collisions, using the signed distance field, is described. Next, the penalty method is applied to determine the contact load of bone on facial soft tissue. Finally, a non-linear finite mixed element model i

Antibiotic prophylaxis in cutaneous surgery.

BACKGROUND: Postoperative wound infections, endocarditis, and late contamination of prosthetic implants are potential complications of cutaneous surgery. OBJECTIVE: To review current American Heart Association guidelines and their application to cutaneous surgery, and to determine how practicing physicians approach this problem. METHOD: Surveys were sent to dermatologists to determine what kind of preoperative screening was being done and how antibiotics were being used. RESULTS: The most common indications for prophylaxis were manipulation of infected tissue in patients undergoing any proc

Widening of scars: foe coaxed into a friend? The Millard technique revisited.

Widening of scars is a frustrating event that most if not all plastic surgeons have encountered. Several therapeutic modalities have been proposed, but frequently the result is disappointing. During revision surgery, it is advocated that the dermal scar be used in providing support to the tissues instead of being discarded. The author proposes a technique by which tension on the wound is carried by tough scar tissue instead of the suture line, thereby reducing the incidence of postoperative widening. This technique was used to treat 35 nonburn scars that had widened: 31 scars were facial (24 p

[Reconstruction of the penis after necrosis due to circumcision].

Total necrosis of the penis is a very serious complication of circumcision with an electrical scalpel. Reconstruction of the penis was achieved by mobilization of the corpora cavernosa and detachment from the ischiopubic rami, which was then placed in a scrotal tunnel to avoid recurrent adhesion to ischiopubic rami and to ensure skin cover of the penis. After a follow-up of 6 months, this child urinated with a good stream and presented spontaneous erections.

Asymmetric sinus excision and primary closure with additional skin excision technique. Effect of reduction of dead-space with Karydakis modification.

OBJECTIVE: Pilonidal sinus is one of the common diseases, although there is still no gold standard of treatment available. The aim of this study was to reduce the residual dead-space volume with a modification following the standard Karydakis procedure. METHODS: A total of 100 patients were included in a randomised controlled trial, who were divided into two groups. Each group included 50 patients, and the patients in Group-1 were treated with the new technique, whereas in Group- 2, the standard Karydakis technique was performed. In the new technique, following total sinus excision, an advanc

Laparostomy closure in stage 4S neuroblastoma.

Laparostomy in children is a rare procedure, and there is little in the literature as to the best method for closure in children with an open abdomen. We describe 2 cases with stage 4S neuroblastoma that required emergency decompressive laparostomy. Both cases highlight advantages and disadvantages with known techniques for achieving skin and fascia closure. We believe this to be the first report of a combination of extracellular matrix mesh and vacuum therapy for successful fascial and skin closure in stage 4S neuroblastoma.

Dressing the part.

Wound care after cutaneous surgery can play an integral role in wound healing. Wound care regimens have changed dramatically over the last 35 years as the physiology of wound healing has become better understood. Foremost is the improvement in wound healing achieved by keeping the wound occluded and moist. This observation has led to an explosion of a whole new category of occlusive dressings at the surgeon's disposal in healing postoperative wounds. These dressings have numerous applications as discussed previously. Generally, for acute surgical wounds, occlusive dressings are most useful for

[Nitrous oxide - oxygen analgesia in aesthetic dermatology].

BACKGROUND: Local anaesthesia often is insufficient for more extensive procedures. Instead of general anaesthesia or sedation, pediatricians, gynaecologists and dentists increasingly use nitrous oxide (N2O). This study evaluates the suitability of this form of anesthesia in dermatology. PATIENTS AND METHODS: In 24 patients (18 w, 6 m, mean age 49 y.) N2O/O2 inhalation (Livopan®) was used during 46 procedures with indications including fractional RF/wrinkle reduction, IPL/rosacea, q-sw. laser/tattoos and hemosiderosis as well as fractional Er:Glass laser for scars and hypopigmentation. Simplified cellular grafting for treatment of vitiligo and piebaldism: the "6-well plate" technique.

BACKGROUND: Vitiligo is a disfiguring depigmenting dermatosis that affects approximately 0.5% to 1% of the general population regardless of race and sex. In patients with stable vitiligo who fail conventional therapies, surgical transplant offers a viable alternative. Noncultured cellular grafting offers the advantage of repigmenting vitiligo 5 to 10 times the size of the donor skin and can be completed on the same day on an outpatient basis. In recent years, ways to simplify this procedure have been explored, including the use of commercially available kits. OBJECTIVES: To simplify the extra

Re-epithelialization of the skin following CO2 laser resurfacing.

Wound healing following CO(2) laser resurfacing of the face is a complex process which must involve a rapid replacement of the ablated epidermis to protect the underlying structures in the skin from desiccation and deeper injury. The majority of cells that regenerate the epidermis come from the hair follicles, and cell movement out of the follicles is monitored using immunofluorescence with antibodies to keratin 17, an intermediate filament protein expressed in the migrating front of cells. This migration is enhanced with occlusive dressings used immediately after the resurfacing procedure. Sk

Behçet's disease: treatment of popliteal pseudoaneurysm by an endovascular stent graft implantation.

Behçet's disease is a multisystemic chronic autoimmune process that can be responsible for some vascular lesions, in addition to its typical dermatologic lesions. Arterial pseudoaneurysm formation can be diagnosed in this group of patients and generally surgery is indicated. In this report, we discuss an alternative endovascular intervention and its advantages. An endovascular stent graft was placed in the popliteal artery pseudoaneurysm of a 37-year-old male with Behçet's disease. Early results of this procedure are good thus far. Implantation of an endovascular stent graft, an alternative mo

Postoperative Complications and Risk Factors in Skin Tumor Excision With Simple Suturing Under Local Anesthesia: A Multicenter Retrospective Study in Japan.

Skin tumor excision under local anesthesia with simple suturing is widely performed in outpatient dermatological practice. Although generally considered safe, postoperative complications such as wound dehiscence, infection, and contact dermatitis can negatively impact patients' quality of life and healthcare resources. However, limited data are available on the complication rates and associated risk factors in procedures without reconstructive techniques. We retrospectively analyzed 1291 patients who underwent local excision and simple closure of skin tumors at three institutions in Japan betw

Dermatologic and Cosmetic Procedures in Pregnancy.

BACKGROUND: Patients may develop a need to undergo procedures while being pregnant and this requires a certain risk-benefit profiling to be done by the clinician. Skin changes during pregnancy such as melasma, striae, varicose veins, hirsutism, and increased skin growths may raise concerns for the lady. Although pregnancy-induced physiologic changes may prompt a surgeon to delay nonessential procedures until after delivery, certain skin conditions may require urgent intervention. Others that may be nonurgent, elective, or cosmetic may need careful analysis. MATERIALS AND METHODS: Data were ex

[Soft tissue reconstruction by dynamic skin suture].

Often the necessity is given, that large soft tissue defects of most diverse genesis have to be supplied. Because it means always substantial disadvantages for the person affected, it applies to perform the reconstructions so fast and as carefully as possible. If there is spare tissue needed for the compensation of the defect this has to come from the direct neighbourhood so that the tissue matches in colour and texture with the surrounding skin. This procedure concerns in particular the face, which as communication level of first degree may not be disturbed by repulsive stigmas and defects. F

Replantation of total degloving of the hand: case report.

This article reports a patient who had a total degloving of the hand, caused by a roller injury, up to the distal forearm level. The authors replanted the degloved skin using arteriovenous anastomosis of the radial artery at the wrist to the cephalic vein in the degloved skin in an end-to-side manner. To enhance the survival of the replanted skin, it was deepithelialized and buried in an abdominal pocket created specifically for this purpose. The replanted skin survived, except at the terminal portions of the fingers. The patient had reasonable function and cosmesis after the wound healed. The

Circumcision and 'uncircumcision'.

The origin of circumcision is shrouded in antiquity. Mummies, 6 000 years old, have been reported to show evidence of circumcision but the first definite account appears in Genesis, chapter 17. Many nations practise it as a rite, the time of its performance varying from soon after birth to early manhood. Antiochus Epiphanes prohibited it in Palestine and the introduction of Greek gymnasia where nudity was the practice, led to many seeking to become uncircumcised. Celsus has described such an operation which appears to have been successful. In Rome under Hadrian and in 16th century Spain a heav
