We tested the hypothesis that chronic hypercapnia contributes to the development of core/targetoid fibers in ventilatory muscles. Five Wistar rats were kept in 5% CO2 in air for 13 weeks. Arterial blood gases (mean +/- SE) were pH 7.402 +/- 0.017, PaCO2 47.4 +/- 2.4 Torr, and PaO2 105.2 +/- 13.2 Torr. Cervical magnetic stimulation (CMS) produces a greater twitch transdiaphragmatic pressure (TwPdi) than electrical stimulation. This may be because CMS produces rib cage muscle activation, thus producing an inspiratory action independent of the diaphragm. A case of the adenocarcinoma of the stomach coexisting with malignant granulomatosis is being presented. Clinical stage of the latter was defined, using also a specimen collected during laparotomy, as 11B below diaphragm. Cytostatics (MOPP/ABV hybrid.), started after gastrectomy, enabled complete remission within 6 months; no recurrence of the granulomatosis was noted. Relationship between these two neoplastic processes has been discussed.

Barium examination and upper endoscopy revealed the presence of a smoothly tapered stricture at the lower end of a dilated oesophagus typical of achalasia. Functional respiratory disorders in patients with systemic lupus erythematosus (SLE) have been previously reviewed. We report a patient with SLE who developed progressive dyspnea with a restrictive pattern in the functional respiratory tests, and diaphragmatic disfunction. Symptoms and functional tests improved after a treatment with steroids. We also review the pathophysiologic mechanisms of this disorder.
