BACKGROUND: Current guidelines suggest that patients should undergo colonoscopy after CT confirmed acute diverticulitis to outrule colorectal cancer (CRC). The aim of this study was to determine if flexible sigmoidoscopy (FS) could be a viable alternative to full colonoscopy following acute sigmoid diverticulitis.
METHODS: A retrospective study of 271 patients was performed who were diagnosed with acute sigmoid diverticulitis by CT and subsequently underwent full colonoscopy. Medical records, CT reports, endoscopy reports, and histopathological reports were reviewed.
RESULTS: Sigmoid diverti…

A 44-year-old woman presented with right upper abdominal pain and nausea with a 4-day duration. An abdominopelvic CT scan revealed a 2.3-cm sized cystic lesion in the duodenum with inflammatory changes and partial thrombosis in the distal main portal vein. Empirical antibiotics were initiated considering acute duodenal diverticulitis, but the symptoms worsened. Side-viewing duodenoscopy revealed purulent fluid draining spontaneously from a small diverticular opening. After enlarging the opening using an extraction balloon, sweeping followed by saline irrigation was performed, and pus and food …

INTRODUCTION: Colonic diverticulosis is common in western world. Fistula formation occurs in 10-20 % of patients, usually as a consequence of an acute inflammatory process (diverticulitis). Fistulas from diverticulitis occur mainly to bladder, small bowel and uterus. Communication between colon and ovary occurred more frequently in the context of primary neoplasms of ovary, ovarian abscess or Crohn's disease. However, colo-ovarian fistula after acute colonic diverticulitis is a rare entity with few cases reported in literature.
PRESENTATION OF CASES: In this article, we described two cases. W…

INTRODUCTION: Reduced port laparoscopic surgery using an umbilical zigzag incision is comparable to conventional multiport laparoscopic surgery. This method is associated with improved cosmesis and decreased wound pain.
PRESENTATION OF CASE: A 67-year-old man visited our hospital. He presented emergency room with a chief complaint of right lower abdominal pain by walking. The patient was diagnosed Meckel's diverticulitis by computed tomography (CT). At first, antibiotics therapy and fasting were performed. Three months later, the patient underwent resection of Meckel' diverticulum with zigzag…

BACKGROUND: Patients with diverticulitis are predisposed to hepatic abscesses via seeding through the portal circulation. Hepatic abscesses are well-documented sequelae of diverticulitis, however instances of progression to hepato-bronchial fistulization are rare. We present a case of diverticulitis associated with hepatic abscess leading to hepato-bronchial fistulization, which represents a novel disease course not yet reported in the literature.
CASE PRESENTATION: A 61-year-old Caucasian man presented with a history of unintentional weight loss and dyspnea both at rest and with exertion. He…

We report a very rare case of Meckel's diverticulitis with ileal stricture at the base of the diverticulum in a young adult as a cause of recurrent small bowel obstruction lasting for 7 years. None of the pre-operative investigations were able to diagnose the cause of obstruction. The patient had undergone appendicectomy 3 years ago but without any relief. Thereafter he was given a therapeutic trial of anti-tubercular drugs, but his symptoms rather worsened with this treatment. Finally, the diagnosis of Meckel's diverticulitis with ileal stricture was made on exploratory laparotomy. The patien…

AIM: To study the clinical features and computed tomography (CT) findings of appendiceal diverticulitis vs acute appendicitis.
METHODS: We retrospectively reviewed the records of 451 patients who had undergone appendectomy in our institution from January 2007 to September 2012. Patient demographics, clinical features, pathological findings, and surgical outcomes were analyzed. We also compared preoperative CT images of 25 patients with appendiceal diverticulitis with those of 25 patients with acute appendicitis.
RESULTS: Among 451 patients, 44 (9.7%) were diagnosed to have appendiceal divert…

INTRODUCTION: It is often postulated that younger patients with acute sigmoid diverticulitis (SD) have an increased risk of perforation which constitutes an indication for early surgery. The aim of this study was to correlate the severity of sigmoid diverticulitis with patient age in order to check the surgical indication in younger patients.
PATIENTS AND METHODS: Patients with acute SD from January 1998 to June 2009 were included. Two age groups were distinguished: group I (GI) ≤40 years in age and group II (GII) >40 years. The perforation risk associated with first episode SD was determined…

Chronic colonic diverticulitis may be associated with typical complications such as local abscesses, stenosis, bleeding, intraperitoneal bowel perforations or fistulas to other organs. Most commonly, fistulas exist between the colon and the bladder; nevertheless, they may also extend to the small intestine, other areas of the colon, ureter, uterus, salpinx, vagina, abdominal wall, portal- and mesenterial venous system, pleura, urachus, biliary system and the hip. We report on a patient with chronic colonic diverticulitis having an unusual sigmoido-gluteal fistula along the sacrum, the piriform…

Using a data base of 7,000 acute abdominal pains, we have described the assessed clinical features of acute diverticulitis of the sigmoid colon. Percentages of sensitivities have been replaced by adverbs or adjectives, applying a scale of equivalence. The modifications of the positive predictive values have been also replaced by verbs or typical expressions. In this article, abscesses, fistulas, generalized peritonitis and hemorrhage arising from an acute diverticulitis of the sigmoid colon were not studied.

During a 4-year period, eight patients 40 years old or younger had surgically proved diverticulitis at our institution. None of these patients had connective-tissue diseases or were on medication (i.e., steroids) that would predispose them to diverticulosis. The presenting clinical symptoms in this group of patients were often misleading, and in only one of the eight cases was the correct clinical diagnosis made at the time of admission. Of the three diagnostic studies that were performed (barium enema, sonography, and CT), barium enema was the most accurate, yielding evidence for diverticulit…

Giant duodenal diverticula are rare and often asymptomatic but may present with atypical clinical and imaging findings that resemble intra-abdominal or retroperitoneal collections, abscesses, or cystic neoplasms. We report the case of a 67-year-old male who presented with recurrent right-sided abdominal symptoms initially treated as pyelonephritis and subsequently diagnosed as a right retroperitoneal abscess. Further evaluation with multimodal imaging, including contrast-enhanced computed tomography (CECT) with oral contrast and retrospective ultrasound review, revealed a giant duodenal divert

INTRODUCTION: This study evaluated the role of interval drain studies following percutaneous abscess drainage in complicated diverticulitis. We sought to identify patients who would benefit from these studies by assessing predictive factors for a positive drain study (PDS).
METHODS: A single institution cohort study was performed on patients undergoing interval drain study following percutaneous abscess drainage for complicated diverticulitis. PDS was defined as persistence of abscess, increased size of abscess, or fistula formation. Demographics, admission data, drain study-specific findings…

Acute diverticulitis, which arises from inflammation of a colonic diverticulum, is a prevalent cause of acute abdominal pain and hospitalization in developed countries. Although most cases respond to conservative treatment, serious complications such as perforation and formation of large abscesses and fistulas can arise, which would require urgent intervention. In the United States, computed tomography (CT) is the diagnostic standard, offering sensitivity and specificity >95%, but ultrasound has also proven effective in identifying uncomplicated cases of diverticulitis.

PURPOSE: The main aim of this study was to determine the short- and long-term outcomes of the non-operative management of acute left-sided complicated diverticulitis (ALCD) in severely immunocompromised patients (IMS group) and compare them with immunocompetent patients (IC group). The secondary aim was to assess the necessity of an elective surgery following a successful prior non-operative management in the IMS group after a non-operative management of the first episode of ALCD.
METHODS: Patients presented with their first episode of ALCD between 2012 and 2018 were retrospectively reviewed.…
