Role of greater occipital nerve blocks and trigger point injections for patients with dizziness and headache.

BACKGROUND: The trigeminocervical system is integral in cervicogenic headache. Cervicogenic headache frequently coexists with complaints of dizziness, tinnitus, nausea, imbalance, hearing complaints, and ear/eye pain. Controversy exists as to whether this constellation of symptoms may be cervically mediated. OBJECTIVES: To determine whether a wider spectrum of cervically mediated symptoms exist, and to investigate a potential role of greater occipital nerve blocks (GON) and trigger point injections (TPI) in these patients. METHODS: Retrospective review of GON/TPI performed in a tertiary oton…

Nystagmus assessments documented by emergency physicians in acute dizziness presentations: a target for decision support?

OBJECTIVES: Dizziness is a common presenting complaint to the emergency department (ED), and emergency physicians (EPs) consider these presentations a priority for decision support. Assessing for nystagmus and defining its features are important steps for any acute dizziness decision algorithm. The authors sought to describe nystagmus documentation in routine ED care to determine if nystagmus assessments might be an important target in decision support efforts. METHODS: Medical records from ED visits for dizziness were captured as part of a surveillance study embedded within an ongoing popula…

Risk of vascular events in emergency department patients discharged home with diagnosis of dizziness or vertigo.

STUDY OBJECTIVE: Dizziness and vertigo are common reasons for visiting the emergency department (ED), but many patients are discharged home without a specific diagnosis. Given the concern that important diagnoses could be missed, we measure the incidence of subsequent major vascular events in patients after discharge home. METHODS: We identified all adults discharged home from California EDs with a primary diagnosis of dizziness or vertigo from January to June 2005, using comprehensive encounter records. Events were captured with linked statewide hospital discharge and national mortality data…

Stroke among patients with dizziness, vertigo, and imbalance in the emergency department: a population-based study.

BACKGROUND AND PURPOSE: Dizziness, vertigo, and imbalance are common presenting symptoms in the emergency department. Stroke is a leading concern even when these symptoms occur in isolation. The objective of the present study was to determine the "real-world" proportion of stroke among patients presenting to the emergency department with these dizziness symptoms (DS). METHODS: From a population-based study, patients >44 years of age presenting with DS to the emergency department, or directly admitted to the hospital, were identified. Demographics, the frequency of new cerebrovascular events, …

Vertigo is an underestimated symptom of ocular disorders: dizzy children do not always need MRI.

Vertigo, instability, dizziness, or equilibrium disorders are not usually considered as consequences of ophthalmologic problems. We present data indicating that ocular disorders can be responsible for these symptoms in children. In a population of 523 pediatric patients with vertigo or disequilibrium and referred for vestibular testing in our otolaryngology department during a 5-year period, 27 children presented with normal vestibular and somatic neurologic examinations but with ophthalmologic disorders (vergence insufficiency or latent strabismus with binocular vision in 70% and anisometropi…

An evaluation of falls, syncope, and dizziness by prolonged ambulatory cardiographic monitoring in a geriatric institutional setting.

Fifty-nine patients living in a geriatric residential setting underwent prolonged ambulatory cardiographic monitoring with the Holter monitor (AHM) as part of an investigation of falls, dizziness, and syncope. In 16 instances, cardiac arrhythmias were a contributing factor; in 12, the diagnostic AHM recordings were associated with unrevealing standard electrocardiographic (ECG) tracings. In 12 patients who had fallen, cardiac arrhythmias played a contributing role, but in only two of these cases was the ECG diagnostic. The value of the AHM for use in managing elderly patients, whose cardiac co…

[Effect of body mass index on residual dizziness after repositioning in benign paroxysmal positional vertigo].

Objective:To explore the influence of the body roundness index (BRI) on residual dizziness (RD) in patients with benign paroxysmal positional vertigo (BPPV). Methods:A total of 137 patients with BPPV were enrolled and divided into an RD group(n=44) and a non residual dizziness (NRD) group(n=93). Differences in demographic characteristics, comorbidities, and disease feature were compared between the groups. Multivariate logistic regression and restricted cubic spline models were used to analyze the relationship between BRI and residual dizziness. Results:Compared with the NRD group, patients in…

Neuroanatomical correlates of stroke-related dizziness and vertigo: secondary analysis from the INSPiRE-TMS trial.

Dizziness and vertigo are common and debilitating neurological symptoms among patients visiting the emergency department (ED). Possibly because the symptoms are not captured by standard stroke severity tools such as the NIHSS (National Institutes of Health Stroke Scale), their network-level neuroanatomical correlates-particularly potential supratentorial contributions-remain incompletely characterized. Therefore, we aim to investigate the neuroanatomical substrates of post-stroke dizziness and vertigo. This retrospective analysis included patients with acute ischemic stroke from the randomized…

Can cervical exercises alleviate residual dizziness? A retrospective study of patients after successful BPPV repositioning.

BACKGROUND: Residual dizziness affects a substantial proportion of patients after successful repositioning for benign paroxysmal positional vertigo (BPPV), posing a challenge for post-treatment management. OBJECTIVES: This retrospective study aimed to evaluate the effects of a four-week cervical exercise program on dizziness resolution and psychological status in patients with residual dizziness. MATERIAL AND METHODS: Sixty-three eligible patients, one week after successful repositioning, were alternately assigned to a cervical exercise group (n = 35) or a control group (n = 28). The interve…

Subjective persistent dizziness handicap and psychological factors in vestibular schwannoma patients: a cross-sectional study.

Persistent dizziness is a frequent and disabling symptom after retrosigmoid microsurgery for vestibular schwannoma (VS), negatively affecting quality of life. While vestibular outcomes are well studied, the contribution of psychological factors to persistent dizziness remains insufficiently understood. This cross-sectional study included 93 patients with VS, of whom 80.6% reported postoperative dizziness. Psychological factors were assessed using validated self-report questionnaires covering premorbid mental health conditions, personality traits, somatization, anxiety, depression, and psycholo…

Residual Dizziness Characteristics of Idiopathic Sudden Sensorineural Hearing Loss Patients with Benign Paroxysmal Positional Vertigo.

INTRODUCTION: ISSNHL, a common clinical condition, can be accompanied by vertigo. Initially, research on sudden deafness primarily focused on the hearing loss itself, with less emphasis on episodic vertigo. However, as vertigo research has advanced, it has been recognized that BPPV is a frequent accompaniment to ISSNHL-associated vertigo. Even after treatment, some patients may experience residual dizziness. This study investigates the characteristics of patients with ISSNHL accompanied by BPPV and the impact of residual dizziness on their lives. METHODS: This study is being conducted on pati…

Visual vertigo and motion sickness is different between persistent postural-perceptual dizziness and vestibular migraine.

INTRODUCTION: Persistent postural-perceptual dizziness (PPPD) and vestibular migraine (VM) share symptoms of visual vertigo and motion sickness that can be confusing for clinicians to distinguish. We compare the severity of these symptoms and dynamic subjective visual vertical (dSVV) in these two common vestibular conditions. METHOD: Twenty-nine patients with PPPD, 37 with VM, and 29 controls were surveyed for subjective symptoms using the visual vertigo analogue scale (VVAS) and motion sickness susceptibility questionnaire during childhood (MSA) and the past 10 years (MSB). dSVV is a measure…

Do monosymptomatic stroke patients with dizziness present a vestibular syndrome without nystagmus? An underestimated entity.

BACKGROUND AND PURPOSE: Vestibular symptoms are common in emergency department (ED) patients and have various causes, including stroke. Accurate identification of stroke in patients with vestibular symptoms is crucial for timely management. We conducted a prospective cross-sectional study from 2015 to 2019 to determine stroke prevalence and associated symptoms in ED patients with vestibular symptoms, aiming to improve diagnosis and outcomes. METHODS: As part of the DETECT project, we screened 1647 ED patients with acute vestibular symptoms. Following a retrospective analysis of 961 head and n

Dizziness-Related Disability One Year after a Mild-to-Moderate TBI-A Follow-Up Study.

Persisting dizziness and balance problems after mild-to-moderate traumatic brain injury (mmTBI) may result in considerable disability. The primary aim of this study was to explore the factors associated with dizziness-related disability one year post-injury. Data from 64 participants (mean age 39.4 [SD 13.0] years; 45 [70.3%] women) with mmTBI from a previous randomised controlled trial were analysed using simple and multiple regression analyses (Clinical Trials Registry #NCT01695577). The Dizziness Handicap Inventory one year (12.1, [SD1.6] months) post-injury was the dependent variable. Demo…

Relationship between periodontal disease and dizziness in Taiwanese adults: A nationwide population-based cohort study.

Periodontal disease is often neglected and overlooking its initial symptoms can lead to tooth loss and systemic diseases. Patients with otitis media are at high risk of vestibular and balance dysfunction, consequently, and vertigo. Vertigo and dizziness are conditions with high reported incidences; they worsen with age and can burden health systems. The present study investigated whether periodontal disease causes dizziness. Research data covering 2008 through 2013 were retrieved from the National Health Insurance Research Database of Taiwan. Patients who were newly diagnosed as having periodo…

Clinical Reasoning: A 37-Year-Old Man Presenting With Intermittent Dizziness and Brief Choreiform Movements.

In this report, we describe the case of a 37-year-old man presenting with intermittent episodes of dizziness and choreiform movements. MRI of the head showed an acute infarction in the left cerebellar hemisphere and multiple chronic infarctions in the cerebellum and pons. Vessel imaging also showed dynamic stenoses in the posterior circulation. This case provides a framework for evaluating young patients with intermittent dizziness and recurrent strokes and highlights the need to combine anatomical angiographies or hemodynamic studies with head rotational behavior.
