Prevention strategies of transfusion-transmitted parasitic infections (TTPIs): Strengths and challenges of current approaches, and evaluation of the strategies implemented in Iran.

BACKGROUND: Several strategies are being implemented in blood transfusion centers of the world to prevent the transfusion-transmitted parasitic infections (TTPIs). The objective of this study was to determine and describe the strategies to minimize the transmission risk of parasitic agents via blood transfusion in Iran. METHODS: This study was conducted in the Iranian blood transfusion organization (IBTO). The data were extracted from the latest version of the "medical interview" standard operating procedure (SOP). RESULTS: The donor selection is the first and only step to reduce the risk of…

KIR B haplotype donors confer a reduced risk for relapse after haploidentical transplantation in children with ALL.

We analyzed the influence of donor killer-cell immunoglobulin-like receptor (KIR) gene haplotypes on the risk for relapse and the probabilit … g-rank test for trend, P = .026). These data indicate that KIR genotyping should be included in the donor selection algorithm for haploidentical transplantation in children with acute lymphoblastic leukemia with the aim of choosing, whenever possible, a KIR haplotype B donor with a high KIR B-content score.

Changing typology of brain death liver donors: characteristics and impact of risk factors on short-term outcome.

BACKGROUND/AIMS: An effective recruitment plan and the use of donors with risk factors are essential means of compensating for the lack of organs, but mean a significant change in the type of donor. This study defines the characteristics of the donors currently used, evaluates their impact on short-term outcome and underlines the need for a different approach to organ selection. METHODOLOGY: Data concerning 244 patients were collected for the study of donor characteristics by age groups, and the multivariate analysis of the associated risk factors. RESULTS: There were no significant differen

Occipital Donor Area Grading and Profile in Indian Population.

BACKGROUND: Safe donor area (SDA) in hair transplant surgery had been categorized by various studies. We designed a study to profile the occipital donor area in our population and devise a grading scale the donor area. AIM: To profile and grade the pattern of receding hair over the occipital donor area among men in the age group of 50-55 years with androgenetic alopecia (AGA). MATERIALS AND METHODS: A total of 681 men with AGA (grade 3 and above Norwood scale, diffuse unpatterned AGA, and retrograde AGA) in the age group between 50 and 55 were included in the study group. Their occipital don

Living donor liver transplantation from a donor with definite obstetric antiphospholipid syndrome: a case report with perioperative risk mitigation strategy.

Antiphospholipid syndrome (APS) is an acquired autoimmune thrombophilia associated with venous, arterial, and microvascular thrombosis. Living donor liver transplantation (LDLT) from a donor with definite APS is exceptionally uncommon and raises a difficult balance between recipient benefit and donor safety. We report LDLT using a right lobe graft from a 47-year-old female spouse donor with recurrent spontaneous abortions and persistent lupus anticoagulant positivity documented on repeat testing more than 12 weeks apart, fulfilling criteria for definite obstetric APS. Anticardiolipin immunoglo

Program interventions to build access to living donor transplantation.

PURPOSE OF REVIEW: Living donor transplantation is the optimal treatment for end-stage renal disease and end-stage liver disease. For the recipient, the benefits of living donation include improved duration of graft survival compared to deceased donation and avoidance of lengthy waiting time and risks of dialysis. For transplant centers, a robust living donor program results in consistent transplant case volumes, daytime cases and a decreased reliance on potentially unfavorable allocation policy changes. Transplant programs require a playbook of actionable initiatives based on consensus best p

Exploring predisposing factors and pathogenesis contributing to injuries of donor lungs.

INTRODUCTION: Lung transplantation (LTx) remains the only therapeutic strategy for patients with incurable lung diseases. However, its use has been severely limited by the narrow donor pool and potential concerns of inferior quality of donor lungs, which are more susceptible to external influence than other transplant organs. Multiple insults, including various causes of death and a series of perimortem events, may act together on donor lungs and eventually culminate in primary graft dysfunction (PGD) after transplantation as well as other poor short-term outcomes. AREAS COVERED: This review

Transplantation of highly sensitized patients on the basis of acceptable HLA-A and B mismatches.

We think that the determination of acceptable mismatches is an efficient approach to increase the chance of finding a crossmatch-negative donor for highly sensitized patients. This approach has several advantages (4,5) namely: 1. There is no need for distribution of patient sera to other tissue typing centers. 2. Rather than performing crossmatches with all donors, most of which will be positive, selection is based on a predictable negative crossmatch. 3. Selection of potential donors is based on data obtained by the recipient centers, which has all the information concerning the patient's imm

Complete correction of murine phenylketonuria by selection-enhanced hepatocyte transplantation.

Hepatocyte transplantation for genetic liver diseases has several potential advantages over gene therapy. However, low efficiency of cell engraftment has limited its clinical implementation. This problem could be overcome by selectively expanding transplanted donor cells until they replace enough of the liver mass to achieve therapeutic benefit. We previously described a gene therapy method to selectively expand hepatocytes deficient in cytochrome p450 reductase (Cypor) using acetaminophen (APAP). Because Cypor is required for the transformation of APAP to a hepatotoxic metabolite, Cypor defic

Lack of effect of donor-recipient Rh mismatch on outcomes after allogeneic hematopoietic stem cell transplantation.

BACKGROUND: A recently published study has reported that donor-recipient Rhesus (Rh)-mismatched allogeneic hematopoietic stem cell transplantation independently led to significantly poorer survival. This suggests that donor-recipient Rh mismatching is a risk factor in allogeneic hematopoietic stem cell transplantation and should be a criterion for donor selection. STUDY DESIGN AND METHODS: To further evaluate this issue, 258 consecutive patients who underwent myeloablative or submyeloablative allogeneic hematopoietic stem cell transplantation at our institution were analyzed to determine the …

[Bypass Surgery for Common Carotid Artery Occlusion].

Bypass surgery for common carotid artery occlusion presents significant challenges. In cases of internal carotid artery occlusion, superficial temporal artery-to-middle cerebral artery (STA-MCA) bypass is the standard approach, as the STA blood flow remains intact. However, in common carotid artery occlusion, the STA blood flow is generally insufficient for a donor artery, necessitating alternative and more complex surgical strategies. This paper discusses the surgical approach for common carotid artery occlusion, focusing on donor artery selection and the relevant surgical anatomy.

Strategies in multiple toe transplantation for bilateral type II metacarpal hand reconstruction.

Multiple toe transplantation has been established as an effective means of restoring prehensile function in the metacarpal hand. The attainable outcome, however, must be weighed against potential donor site morbidity. Restoration of prehensile function in bilateral type II metacarpal hands can be achieved by reconstructing three opposable digits in the dominant hand and two opposable digits in the nondominant hand. This article outlines the strategy for optimizing functional outcome and discusses the rationale behind toe selection to reduce donor morbidity.

Preoperative Drip Infusion Cholangiography With Computed Tomography to Identify the Duct of Luschka in Living-Donor Hepatectomy: A Case Report.

The Duct of Luschka represents a rare form of accessory biliary ducts. There are no reported cases of the Duct of Luschka in living donor hepatectomy. We report a 36-year-old man with the Duct of Luschka who underwent living-donor hepatectomy. Preoperative drip infusion cholangiography with computed tomography (DIC-CT) identified the Duct of Luschka originating from the right hepatic duct to the cystic duct. By choosing the left and caudate lobe hepatectomy, the Duct of Luschka was left intact during the procedure. The donor recovered uneventfully posthepatectomy, and the recipient also did no

Long-term follow-up following bone marrow transplantation for Hunter disease.

Bone marrow transplantation (BMT) was performed in 10 patients with Hunter disease (mucopolysaccharidosis type II, iduronate-2-sulphatase deficiency). The donor was an HLA-identical sibling in 2 cases, an HLA-nonidentical relative in 6 cases, a volunteer unrelated donor in 1 case, and details were not available in 1 case. Only three patients have survived for more than 7 years post BMT; however, this high mortality probably resulted from poor donor selection. In two, there has been a steady progression of physical disability and mental handicap. One patient has maintained normal intellectual d…

[Donor nephrectomy].

Using 76 protocols of anaesthesia for donor nephrectomy from 1976 to 1982, the early selection of a possible cadaver kidney-donor and the requirements for donation are reported as well as measures of ascertaining cerebral death, possibilities to obtain the consent for donation and the course and organization of donor nephrectomy. Own experiences show that cadaver nephrectomy can be carried out without any disturbance of the routine operative program. The task of the anaesthesiologist is not specific but related to the fundamental knowledge of his specialty. Amelioration of the organization and

Evaluation of Kidney Donors: Core Curriculum 2018.

Nearly 100,000 patients are waiting for a kidney transplant, yet each year only 11,000 undergo transplantation with a deceased donor kidney. Annual death rates among waitlist registrants range from 5% to 15%; many die before receiving a transplant. Not surprisingly, registrants turn to family and friends to become living kidney donors on their behalf. Living kidney donor selection practices aim to quantify lifetime risk for kidney failure based on a candidate's predonation demographic and health characteristics. It has been established that estimated lifetime risk for kidney failure varies con…

Confronting complexities of uterus transplantation: balancing innovation, risk, and access.

Uterus transplantation (UTx) has evolved from an experimental procedure to an increasingly established infertility treatment for women with absolute uterine-factor infertility. Despite procedural progress, UTx remains a very complex infertility treatment. Here, we examine key complexities and challenges that will shape the future of UTx. Donor-related considerations include selection criteria for both live and deceased donors to maximize the likelihood of transplantation success. The implications of exposing healthy live donors to risks of postoperative complications, organ availability, and l

Complications in Donors Using Right Liver Graft: Analysis of 280 Consecutive Cases.

INTRODUCTION: Living donor liver transplantation (LDLT) is performed with increasing frequency worldwide due to the shortage of donated organs. It is a life-saving procedure for the recipient, but, on the other hand, a major surgical procedure for healthy donors and it may cause morbidity and even mortality. PATIENTS AND METHODS: This research was completed at Dokuz Eylül University Faculty of Medicine Hospital General Surgery Department Liver Transplant Unit and included 280 cases (4 with simultaneous liver and kidney transplants from living donors) who underwent donor right hepatectomy for
