The temporal bone histopathologic findings of a patient having received a Nucleus 22-channel cochlear implant (Cochlear Corp, Englewood, Colo) following bilateral transverse otic capsule fractures is presented. The case was complicated by perilymph fistulas, labyrinthitis, meningitis, and death due to drowning. In brief Whitewater paddlers have very few injuries other than bruises and minor scrapes, and major injuries are the exception rather than the rule. Many of the injuries occur to capsized boaters, who are susceptible to hematomas, contusions, and abrasions from bumping into unseen rocks and obstructions as the river current carries them downstream. Two major chronic complaints are tendinitis of the wrist extensors and low back pain. Major injuries include head injuries, fractures, and shoulder dislocations and subluxations. As with other water sports, drowning is the major cause of fatalities.

Sequelae of smallpox infection on the ocular surface are still seen, including corneal scars adherent leukoma and phthisical globes. This paper will report another sequel of smallpox infection causing inadvertent bleb in a 62-year-old diabetic female with no history of ocular surgery or trauma in either eye. The patient had smallpox infection during her childhood. Her follow up extended from May 1997 until August 2007 with a constant eye examinations including controlled intraocular pressure, avascular cystic inadvertent bleb, and up drown peaked pupil.

The case of a 77-year-old woman who was found dead in her bathtub with her head clearly above the water line is presented. The decedent had a medical history of depression, liver disease, spinal stenosis, and diabetes mellitus. An empty medication bottle of triazolam was found in the trashcan. In a 10 year survey of deaths during sports and recreation, 110 deaths occurred and 80% of these were due to unnatural causes, the majority being drowning. Twenty-two cases were due to natural causes mainly due to failure of the circulatory system. The mechanism of death was briefly discussed and it was deemed advisable for those who are going to undertake a training programme in running or athletics to have a medical examination. This is especially important if one is over 30 years of age and have family history of heart disease and high blood pressure and those who led a sedentary life.

During an infusion of thiopentone to control status epilepticus secondary to hypoxic brain damage, thiopentone and pentobarbitone levels were measured in the serum and cerebrospinal fluid (CSF). Pentobarbitone was found to be present in the serum between 7.8 and 11.1% of the thiopentone levels. There was a prompt response in serum levels of thiopentone to changes in the infusion rate. The CSF thiopentone varied between 15 and 40% of the serum levels. The CSF anticonvulsant threshold for thiopentone for this patient was between 5 and 15 mg/litre.
