We had 126 patients with inner ear malformation diagnosed with temporal bone computed tomography (CT) scans at Azabu Triology Hospital between 1996 and 2002. External ear canal cholesteatoma is a rare condition in otologic practice. A case in a 43-year-old woman is presented in which despite the extensive nature of the lesion, minimal symptoms and absence of signs delayed diagnosis. The cause of the lesion and its treatment are discussed.

Autoimmune deafness represents a heterogeneous group of disorders in which there is though to be inner ear and/or middle ear damage mediated by various immunological abnormalities. We describe a patient with Wegener's granulomatosis in whom there was evidence of significant inner ear damage caused by vasculitis, which was completely reversed by cyclophosphamide and prednisolone. We propose that Wegener's granulomatosis is a cause of autoimmune deafness.

Feeding practices were studied in 128 infants aged 18 months and below with middle ear infection, belonging to the low socio-economic group. By the age of one year, 117 (91.4%) had acquired the infection. At the onset of the first episode, 105 (82.0%) were breast-fed, 42 not receiving any other milk. A case of ME effusion in a patient with MG is reported. The likely pathogenesis is of TVP muscle weakness causing Eustachian tube dysfunction. ME effusion should be suspected in a myasthenic complaining of sudden hearing loss. Tympanometry with measurement of the ME pressures and reflexes is a useful tool for assessing a myasthenic's response to treatment.
