PURPOSE OF THE STUDY: Neurogenic paraosteoarthropathy of the elbow is a frequent complication in head injury patients. The functional impairment may be severe. The purpose of this work was to evaluate the efficacy of our medico-surgical approach and to determine indications for surgery.
MATERIAL AND METHODS: We analyzed retrospectively a continuous series of 44 patients (51 elbows) with neurogenic heterotopic new bone of the elbow joint managed from 1993 to 2000. The heterotopic bone had developed on a central deficit limb in 70% of the patients. Flexion-extension was limited in all patients,…

BACKGROUND: Individuals with a brachial plexus birth injury have changes in optimal muscle length of affected muscles and diminished motion at the elbow and shoulder, impacting active shoulder elevation. The long head of the biceps, a bi-articular muscle, is of particular interest in this population due to its reduced optimal muscle length and simultaneous shortening at the shoulder and elbow during active shoulder elevation. Prior studies have shown immobilization of a joint may improve the moment-generating capacity of a biarticular muscle crossing that joint. The aim of this study was to in

Flexor pronator muscles (FPMs) play a key role in stabilizing the elbow joint against valgus forces. However, no studies have investigated the in vivo kinematics of FPMs against these forces on the elbow. This study aimed to clarify the in vivo contribution of each FPM as a dynamic stabilizer in a clinical situation.Twelve healthy volunteers participated in this study. Verbal informed consent was obtained from all subjects. The elbow was flexed to 90 degrees, and the forearm was placed in the neutral position. Manual valgus stress was applied to the elbow joint until maximal shoulder external …

Monteggia lesions and Monteggia-like lesions involve ulna and radius injuries, which can not be successfully treated using the surgical principles of isolated fractures. Proximal ulnar fracture, radio-humeral dislocation, and additional dislocation in the proximal radioulnar joint result in the disintegration of the functional unit, and there is a complex injury across the elbow to the forearm, resulting in poor clinical outcomes. Thus, addressing all osteo-ligamentous injuries is essential for the long-term course.

Elbow osteoarthritis is occasionally primary and most often post-traumatic. Nonoperative treatment is frequently the best course of action. Available surgical options are governed by the location of cartilage deterioration, bone structure, and preoperative joint instability. Debridement of osteophytes can achieve modest, temporary increases in motion and comfort. Surgical management benefits from knowledge of the substantial varus torque that is exerted when the arm is abducted. In the elbow that is resurfaced or replaced, these forces may strain the soft tissue support and lead to premature s

BACKGROUND: Changes in the dominant shoulder and elbow of professional pitchers have been noted on radiographs, magnetic resonance imaging scans, and ultrasound studies. The relationship of these findings to future injury and to time lost from play has not been identified.
HYPOTHESIS: Degenerative changes of the shoulder and elbow are common findings on the radiographs of asymptomatic Major League Baseball pitchers. The changes are cumulative with increased pitching. These findings are not predictive of time on the disabled list.
STUDY DESIGN: Cohort study (prognosis); Level of evidence, 4.

INTRODUCTION: Avoiding overlengthening in radial head arthroplasty (RHA) is essential for the prognosis of the elbow joint. An overlengthening from 2 mm is visible due to widening of the anterolateral ulnohumeral joint space but intraoperatively, this particular joint space is difficult to visualize. The commonly used Kocher approach allows visualization of the posterolateral joint space without additional instruments or further surgical release of the already unstable elbow. The aim of our study was to investigate whether the visualization of the posterolateral joint space is also a reliable …

Posttraumatic limitation of range of motion in the elbow joint may be of extraarticular, intraarticular or mixed origin. If conservative management fails to improve elbow motion, arthroscopic arthrolysis, open arthrolysis and distraction arthroplasty are available as therapeutic options. Joint distraction for gradual lengthening of shortened ligaments and capsules is used to disimpact the humero-ulnar articular surface. Under distraction fixator controlled mobilization is carried out. The use of transarticular external fixation is particularly helpful in cases of subluxation in order to restor…

A standardized and systematic approach for the management of an acute complex elbow dislocation involving the radial head and coronoid will maximize clinical outcomes. The cornerstones of surgical management include primary restoration of the ulnohumeral joint by reduction of the intact joint or coronoid fracture fixation, followed by radial head fixation or replacement. The lateral ulnar collateral ligament complex is then repaired before assessing elbow stability within a functional sagittal arc of motion.

Arthroscopy of the elbow was performed in 35 cases of suspected loose bodies. Loose bodies were found in 13 cases. Preoperative radiographs were falsely negative in two cases and falsely positive in 12. In ten cases arthroscopic removal was attempted; it succeeded in six cases. When no loose bodies were found, the most common diagnosis was degenerative joint disease. Arthroscopy is recommended for diagnosis and treatment of loose bodies in the elbow joint.

Coronoid fractures are rarely isolated and are much more frequently associated with other osseous or ligamentous structures injuries. On the basis of the coronoid fracture patterns, described by the O'Driscoll classification, it is possible to recognize three main patterns of injury that differ on traumatic mechanism and on associated lesions: posterolateral rotatory instability, posteromedial rotatory instability, and axial load injuries. The management of coronoid fractures is challenging and varies according to characteristics of the fracture, associated lesions, and amount of elbow instabi

We present three pediatric cases of hemophilic arthropathy (HA) of the elbow that had already progressed to advanced stage with osteochondral lesions at the time of diagnosis-Case 1: 13-year-old boy, severe hemophilia A; Case 2: 13-year-old boy, moderate hemophilia A; Case 3: 15-year-old boy, severe hemophilia A. HA results from repeated hemarthrosis. Notably, Cases 2 and 3 had no history of clinically recognized elbow hemarthrosis before HA diagnosis, and Case 1 had only two episodes of hemarthrosis. Cases 2 and 3 did not report elbow extension restriction and pain for 1 or 2 years, as their

BACKGROUND AND IMPORTANCE: Snapping of the triceps muscle occurs when a portion of the medial head dislocates over the medial epicondyle with elbow flexion. Resection or redirection of a portion of the triceps muscle is the main surgical treatment. The extent of triceps resection/redirection is difficult to evaluate. A novel intraoperative technique, stimulating the musculocutaneous nerve, to simulate active elbow flexion is proposed to help ensure that the snapping triceps has been adequately treated.
CLINICAL PRESENTATION: A patient presented with a several year history of bilateral elbow p

INTRODUCTION: The aim of the present study was to develop a technical process to reproducibly generate terrible triad injuries (TTI) in fresh-frozen human cadaveric specimens, while leaving the skin intact. Such "pre-fractured" specimens, used for scientific analysis and for surgical education, might help to improve current treatment, which is complex and prone to complications.
MATERIALS AND METHODS: To induce the desired fractures, a custom-made fracturing unit was used to apply an axial force on the extended cadaveric elbow specimens, with the forearm pronated and under valgus load. To sim

BACKGROUND: Luxation of the elbow occurs around the age of 7 years instead of a supracondylar humeral fracture. The most common complication is a fracture of the medial epicondyle of the ulna in the sense of a bony avulsion of the ligament. There are only few case reports regarding collateral injuries of the brachial artery. All authors have described this complication as being very rare.
CASE REPORT AND LITERATURE SEARCH: This article reports the case of a 10-year-old boy, who suffered a dorsal luxation of the left elbow. After reduction the left hand was noticeably pale and cold. The diagno

A case report of a closed posterior elbow dislocation with brachial artery rupture treated with a reversed saphenous vein graft, fasciotomy, and medical collateral ligament (anterior oblique component) repair is presented. A literature review of 21 other similar cases is discussed. Three patients were treated with direct arterial suturing, four with ligation, four closed reductions, nine vein grafts, and two were undescribed. Early postoperative complications included anastomosis failure in two patients (9%) and thrombosis in three patients (14%). At final follow-up, 12 patients (55%) had a mo

We describe a case of a 6-year-old girl with a posterolateral elbow dislocation and a concomitant fracture of the lateral humeral condyle. After reduction of the dislocation, the fracture was diagnosed and treated by open reduction and fixation, with a good functional result. In doubtful cases, oblique, heterolateral, and varus stress films, or even arthrography may be necessary.

The anatomical relations of the semilunar notch of the ulna were studied in radiographs, taken in a strict lateral view, from 100 patients with elbow dislocations. The results were compared with those from a control group of 150 patients without dislocation or other radiographic pathology. We found that the coronoid process and an angle (a) formed between the axis of the ulna and a line going through the tip of the coronoid process and the tip of the olecranon were significantly smaller than those in the control group (P = 0.02). The central angle of the semilunar notch was significantly large
