General Anesthesia in a Patient With Urticaria Pigmentosa Referred for Electroconvulsive Therapy.

Urticaria pigmentosa is a rare disorder characterized by an abnormal systemic proliferation of mast cells. In this condition, various trigge … hesia must be managed carefully. In this review, we describe the safe use of general anesthesia for electroconvulsive therapy in a patient with urticaria pigmentosa and discuss the association between psychiatric disorders and mastocytoses.

Hypocapnia and hyperoxia induction using a hyperventilation protocol in electroconvulsive therapy.

INTRODUCTION: Hyperventilation in electroconvulsive therapy sessions has been associated with seizure threshold, seizure characteristics, and cognitive effects. There is no consensus on the optimal procedure of applying hyperventilation manoeuvres during electroconvulsive therapy. MATERIAL AND METHODS: Prospective evaluation of the effects of systematic use of hyperventilation manoeuvres with facial mask and capnography (protocolized hyperventilation [pHV]), on ventilation parameters and on seizures. The study included a sample of 130 sessions (65 performed according to hyperventilation stand…

[Cardiac safety of electroconvulsive therapy in an elderly patient--a case report].

Since electroconvulsive therapy (ECT) was introduced as treatment for psychiatric disorders in 1938, it has remained one of the most effective therapeutic methods. ECT is often used as a "treatment of last resort" when other methods fail, and a life-saving procedure in acute clinical states when a rapid therapeutic effect is needed. Mortality associated with ECT is lower, compared to the treatment with tricyclic antidepressants, and comparable to that observed in so-called minor surgery. In the literature, cases of effective and safe electroconvulsive therapy have been described in patients of…

The Practice of Electroconvulsive Therapy in US Correctional Facilities: A Nationwide Survey.

OBJECTIVES: There are little data regarding the practice of electroconvulsive therapy (ECT) in correctional settings in the United States. A survey was conducted to study the current practice of ECT in US prisons. We hypothesize that ECT is underutilized in the correctional setting. We also review the ethical aspects of using ECT for the treatment of mental illness in the prison population. METHODS: A 12-question survey via a Survey Monkey link was emailed to chiefs of psychiatry, or the equivalent, of each state's department of corrections. We examined the frequency of Likert-type responses,…

Maintenance electroconvulsive therapy for a neuroleptic-intolerant patient with disorganized schizophrenia.

Owing to unresolved questions concerning the efficacy and safety of electroconvulsive therapy (ECT) in the treatment of schizophrenia, and widespread negative attitudes toward ECT, maintenance ECT (mECT) is generally considered only as a last resort. Nevertheless, in some clinical situations, the advantages of mECT may outweigh the risks and associated concerns. We report the case of a patient suffering from disorganized schizophrenia who had life-threatening hematological side effects to treatment with antipsychotic agents. Long-term mECT was administered and the patient achieved remission wi…

Response of depression to electroconvulsive therapy: a meta-analysis of clinical predictors.

OBJECTIVE: Roughly one-third of individuals with depression do not respond to electroconvulsive therapy (ECT). Reliable predictors of ECT response would be useful for patient selection, but have not been demonstrated definitively. We used meta-analysis to measure effect sizes for a series of clinical predictors of ECT response in depression. DATA SOURCES: PubMed was searched systematically to identify studies published after 1980 that tested at least 1 clinical predictor of response to ECT. STUDY SELECTION: Of 51 studies identified, 32 were compatible with meta-analysis. DATA EXTRACTION: Th…

Drug resistant neuroleptic malignant syndrome and the role of electroconvulsive therapy.

AIM: Cytotoxic drugs have reduced the mortality in patients with ANCA-associated vasculitis (AASV) but their use carries a substantial risk of toxicity. Efforts are made to switch from cytotoxic drugs to less toxic maintenance regimens. In this study we aimed to assess the efficacy of mycophenolate mofetil (MMF) as maintenance therapy in patients with AASV and renal involvement. METHODS: 22 patients with newly diagnosed AASV, microscopic polyangiitis (MPA) (n = 16), Wegener's granulomatosis (WG, n = 4), renal limited vasculitis (RLV, n = 1) and Churg-Strauss syndrome (CSS, n = 1) and renal in

A randomized comparison of ketamine versus methohexital anesthesia in electroconvulsive therapy.

To assess the clinical utility of ketamine as an anesthetic agent for electroconvulsive therapy (ECT), based upon recent findings that ketamine may have antidepressant properties. Depressed ECT patients were randomly assigned to receive anesthesia with either ketamine or methohexital. Outcome measures included assessments of depressive severity, cognition, post-anesthesia side effects, and hemodynamics. Twenty one patients were treated with ketamine and 17 with methohexital. There were no significant differences in depression or cognitive outcomes between the two drugs. Additionally, there wer…

A within-subject comparison of propofol and methohexital anesthesia for electroconvulsive therapy.

OBJECTIVES: The optimal anesthetic for electroconvulsive therapy (ECT) is a frequently studied but unresolved issue. Methohexital and propofol are 2 widely used anesthetic agents for ECT. The purpose of this study was to determine which of the 2 agents was associated with superior clinical outcomes. METHODS: Records from all patients who had undergone separate ECT courses with methohexital and propofol between 1992 and 2008 (n = 48) were reviewed for a retrospective within-subject comparison of outcome measures. The clinical outcomes we examined were number of treatments required in a course …

Electroconvulsive therapy (ECT) in bipolar disorder: A narrative review of literature.

In many countries including India electroconvulsive therapy (ECT) is frequently used to treat different phases of bipolar disorder. The response to ECT is impressive in mania, depression and in mixed affective states. Preliminary evidence also suggests benefit from maintenance ECT in bipolar disorder. However, most of the literature on efficacy and adverse effects comes from case series, retrospective reports and open trials - controlled trials have been few and far between. Official guidelines recommend the use of ECT only when there is a dire emergency or when all other options have been exh…

The effect of electroconvulsive therapy using propofol and succinylcholine on the intraocular pressure.

PURPOSE: Electroconvulsive therapy (ECT) is a therapeutic procedure in many mood and psychiatric disorders. After induction of general anesthesia by administering an induction dose of an intravenous anesthetic such as Propofol, intravenous succinylcholine is often used to prevent bone fractures and joint dislocations during ECT. Intraocular pressure (IOP) is raised by succinylcholine and tonic-colonic convulsion, and decreased by propofol administration. To our knowledge, there is no published paper on the effect of ECT using propofol and succinylcholine on the IOP. This study for the first tim…

The use of electroconvulsive therapy in a cohort of forensic psychiatric patients with schizophrenia.

BACKGROUND: In Denmark, over 2500 people are in psychiatric treatment in forensic mental health services at any one time, most suffering from schizophrenia. Many of them have illnesses that are resistant to medication. There is evidence of the effectiveness of electroconvulsive therapy (ECT) for schizophrenia, but not explicitly for this complex forensic group. AIMS: The aim of this study was to describe the outcome of using ECT as augmentation therapy in a cohort of forensic psychiatric patients with schizophrenia who were failing to respond to antipsychotic medication. METHODS: In one univ…

Continuation and maintenance electroconvulsive therapy for mood disorders: review of the literature.

BACKGROUND: Electroconvulsive therapy (ECT) is a highly effective treatment for mood disorders. Continuation ECT (C-ECT) and maintenance ECT (M-ECT) are required for many patients suffering from severe and recurrent forms of mood disorders. This is a review of the literature regarding C- and M-ECT. METHODS: We conducted a computerized search using the words continuation ECT, maintenance ECT, depression, mania, bipolar disorder and mood disorders. We report on all articles published in the English language from 1998 to 2009. RESULTS: We identified 32 reports. There were 24 case reports and re…

A systematic review of the combined use of electroconvulsive therapy and psychotherapy for depression.

OBJECTIVE: Electroconvulsive therapy (ECT) is one of the most effective treatments for severe major depressive disorder. However, after acute-phase treatment and initial remission, relapse rates are significant. Strategies to prolong remission include continuation phase ECT, pharmacotherapy, psychotherapy, or their combinations. This systematic review synthesizes extant data regarding the combined use of psychotherapy with ECT for the treatment of patients with severe major depressive disorder and offers the hypothesis that augmenting ECT with depression-specific psychotherapy represents a pro…

The safety of electroconvulsive therapy in patients with prolonged QTc intervals on the electrocardiogram.

BACKGROUND: Prolonged QTc intervals have been associated with increased cardiac morbidity and mortality. We investigated whether pretreatment prolongation of the QTc interval was associated with increased cardiac-related events (CREs) in electroconvulsive therapy (ECT) treatment of psychiatric disorders. METHODS: The charts of 1437 ECT patients were reviewed for the presence of baseline QTc prolongation. Broadly defined CREs during the course of treatment were abstracted from patient charts. RESULTS: Presence of baseline QTc prolongation was not independent of increased risk of CREs during E…

Tics as signs of catatonia: electroconvulsive therapy response in 2 men.

OBJECTIVES: Tics have rarely been described in catatonia although tics are sudden and nonrhythmic variants of stereotypic or repetitive movement abnormalities that are considered cardinal symptoms of catatonia. We describe 2 men with tics and self-injurious behavior, who met criteria for catatonia. One patient met criteria for autism. CASE REPORTS: We reported 2 new cases and performed a literature review using PubMed to identify other cases of tics that were treated with electroconvulsive therapy. Tics along with other catatonic symptoms and self-injurious behavior responded to electroconvul…
